Large artery properties in primary and secondary - endocrine hypertension

Arterial stiffness represented by carotid-femoral pulse wave velocity (PWV) is considered to be an independent cardiovascular risk factor. This study was focused on large artery properties investigation in specific forms of hypertension using applanation tonometer Sphygmocor (Atcor Medical). PWV was significantly higher in resistant hypertension patients when compared to moderate essential hypertension (EH) patients. This difference appears to be independent of clinical blood pressure (BP). Night-time BP appears to be a more accurate predictor of PWV in EH. In another study we demonstrated that primary hyperparathyroidism (PH) (both hypertensive or non-hypertensive forms) might be associated with higher PWV when compared to EH patients or to normotensive controls and that this difference is independent of age and clinical BP. Neither calcium serum level, nor parathyroid hormone has been associated with PWV. Specific treatment by parathyroidectomy (PTX) seems to be beneficial for PWV decrease, which might be mainly determined by improved BP control after surgery. Bioimpedance signal processing

Theoretical part of this Bachelor thesis deals with properties measurement and use of bioim-pedance signal for medical diagnostics. The practical part deals with detection of important points in derivate bioimpedance signal that may be further used in various medical applicati-ons. This work compares three detection methods. The best results were achieved by a de-tection with fixed interval there were excluded only 1,09 % values. Very similar results had detector dependent on constant bioimpedance. Here was excreted 1.18 % of detections. Algo-rithms that are dependent on length of RR interval perform worse. Furthermore, there is cal-culated the Pulse wave velocity in six parts of the human body in a horizontal and tilt patient position. In the tilt position were the pulse wave velocity about 38.59 % faster than in hori-zontal.

Evaluation of pulse wave velocity in the human body

This Mater's thesis deals with the evaluation of pulse wave velocity using multi-channel whole-body impedance cardiography. Data were taken from the group of healthy volunteers whose impedance changes were measured during rest, respiratory maneuvers, tilt and stress exercise. The result of this measurement are values of peaks of pulse wave time shifts towards R-wave. The velocity values towards the thorax electrodes were recalculated on the basis of knowledge about the pulse wave time shifts and the distances from the heart to the scanned locations, which were measured using the arterial segment's lengths.
Properties of pulse wave velocity in human body during various excitations

The thesis is concerned with the analysis of measuring pulse wave velocity in human body with the aid of whole-body multichannel bioimpedance which was developed at ISI AS CR, v.v.i.. The evaluation of pulse wave velocity can provide us with important information about the state of vessel compliance which is one of the basic parameters informing on their physiological state. The examination of the state of vascular system is a very important part of early diagnostics because its pathological states are the main contributor to the rise of cardiovascular diseases and disease mortality. The thesis is concerned with the theoretical analysis together with the available methods of valuation of the state of vascular system that use measuring of pulse wave velocity. The main part of the thesis deals with the analysis of the whole-body multichannel bioimpedance measurement. The proposed and programmed protocol that summarizes and visualizes all obtained information is a part of this thesis. This is currently used as an output of the experimental measurement by this method. Data file for statistical processing contains the values of the pulse wave velocity of 35 healthy volunteers and subsequently the properties of pulse wave are assessed at various excitations.

Measurements of the pulse wave velocity (PWV) in human arteries are important for two different reasons: an early diagnostics of atherosclerosis and identification of material properties of the vessel wall for numerical simulations. The PWV in blood vessels is not constant and classical methods of pressure wave shift between two points are not sufficient. We consider three methods: optical measurements of vessel wall displacement in several points by high speed camera, continual optical measurements of vessel wall displacement by correlator Q-450 and measurements of flow rate and displacements by ultrasonic device. The first method can determine the PWV only between several points. The second method can show 3D reconstruction of displacements with very good resolution. The ultrasound measurements need combination of Doppler ultrasound velocimetry and B-scan imagining. However, the common devices used in medicine do not have any data output. It causes bad resolution.

The pulse wave velocity (PWV) in blood vessels is one of the important hemodynamical parameters for detection of artery health condition. In our laboratory, the PWV in elastic tubes and arteries is measured in vitro. The method is based on optical measurements of displacement of the pulsating tube wall. A high frame rate camera was used. The obtained data were evaluated by several methods and compared. The measured pulse wave velocities were in the same order of magnitude as values based on theoretical calculations and published physiological data.
Detection of the pulse wave velocity in an elastic tube

The velocity of the pulse wave in blood vessels is one of the important hemodynamical parameters to detect health condition or a degree of damage of a diseased artery. To understand the problem, the physiological values must be known. In our laboratory, the velocity of pulsewave in elastic tubes and arteries is measured in vitro by non-invasive method. The method is based on optical measurements of displacement of the pulsating pipe wall. A crash test camera with high frame rate of 1000 Hz (fps) was used.
