The influence of botulotoxin application in lower extremities on physiotherapy of children with a spastic form of infantile cerebral palsy

Cerebral Palsy is one of the most frequent neurodevelopmental illnesses occuring in the child age. Main manifestations of the illness include a disorder of muscle tone leading to the development of spasticity. One of contemporary methods of Cerebral Palsy's therapy has been local application of Botulinum Toxin A. This toxin effects a blockade of neuromuscular transmission on neuromuscular junction and in short term limits excessive contractions of spastic muscles. This thesis deals with the influence of physical therapy of children suffering from Cerebral Palsy following the application of Botulotoxin A and compares physical therapic methods applied in the Czech Republic and abroad. Function of physical therapy in comprehensive rehabilitation within Evidence Based Medicine

This thesis called "Function of physical therapy in comprehensive rehabilitation within Evidence Based Medicine" firstly speaks about neuroanatomy and neurophysiology of peripheral nerves. This work also deals with symptomatology of peripheral nerve lesions and muscle dysfunctiones. It deals with indications of rehabilitation therapy including physical therapy by electrostimulation too. The aim of this thesis is to gather and make critical assessment of the piece of knowledge about objectification of physical therapy by electro stimulation. On the basis these piece of knowledge this work denominates crucial issue of objectification of electrostimulation and physical therapy as a whole. The aim of this thesis is to find out what are therapeutical possibilities of selective electrostimulatin in comprehensive rehabilitation within Evidence Based Medicine. Tato práce popisuje proces tvorby multi-platformního interaktivního systému a demonstruje jeho použitelnost na prototypu aplikace simulující rehabilitaci. Je zde zahrnut SW návrh, 3D programování a produkce animací za použití hloubkového senzoru Kinect. Jádro aplikace bylo vytvořeno v herním enginu Unity 3D a bylo otestováno na Linux Ubuntu, Windows 7, mobilním zařízení Android a Unity Web Playeru. První polovina tohoto dokumentu obsahuje můj průzkum nejmodernějších nástrojů a zdrojů vhodných pro realizaci interaktivní multi-platformní aplikace, pracující s 3D obsahem. Nejprve jsou popsány tři herní enginy (Unity 3D, UDK, Unreal Engine 4), jakožto hlavní vývojová prostředí vhodná k realizaci aplikace. Poté jsou popsány možnosti ukládání dat obecné pro všechny zmíněné enginy - integrovaná úložiště dat, databázové systémy a serializace. Nakonec jsou v práci popsány výhody a nevýhody různých přístupů, jak vytvořit 3D animace. Je zde naznačeno použití 3D softwaru určeného k manuální produkci, ale také různé možnosti snímání pohybu Motion Capture. Druhá část dokumentu popisuje mé zhodnocení informací popsaných v předchozí části a rozhodnutí, které prostředky jsem použil k vytvoření prototypu. Také je zde zkonkretizováno zadání a formulovány základní parametry aplikace. Dále je popsán SW návrh, produkce a zpracování 3D animací a samotná implementace. Nakonec je výsledný prototyp zhodnocen a naznačena rozšíření, které je možné aplikovat v budoucnosti. Co se týče využití samotné aplikace, tato práce předvádí mé nápady, jak mohou být informační technologie využity v oboru fyzioterapie a zdravotnictví obecně. Výsledkem nemělo být nahrazení fyzioterapeutů, nýbrž poskytnutí pomoci jim a jejich pacientům. Tento prototyp je příkladem, který nabízí 3D vizualizaci cvičení a lidského pohybového systému. 3D animace, ukazující, jak by měl být každý cvik správně proveden, mimo jiné, mohou učinit rehabilitace příjemnější a zároveň účinnější.
The effect of physiotherapy on cardio-respiratory parameters in patients with chronic heart failure indicated for cardiac surgery intervention

The bachelor thesis deals with the preoperative physiotherapy in patients with chronic heart failure who are indicated for cardiac surgery intervention and with the description of physiotherapy procedures utilized before and after the operation. The cardiovascular disease is in developed countries still the main cause of morbidity and mortality. (UNIFY, 2008) The improved treatment of acute conditions allows so that more patients reached a stage of the chronic heart failure. (Špinar, Vítovec, Hradec, 2012) The chronic heart failure is a sign for a number of symptoms that are caused by disruption of the heart work. It is a disorder in which despite adequate ventricular filling drops a cardiac output and the heart is unable to cover metabolic needs of the tissues (supply of oxygen and nutrients and removing of carbon dioxide and metabolic waste products). (Štejfa a kol., 2007) In the theoretical part is described the anatomy of the heart, the physiology of the heart and the blood circulation, the chronic heart failure, the response and the adaptation of the organism to physical load, the cardiovascular rehabilitation and physiotherapy procedures carried out before and after the operation. The practical part of the bachelor thesis is performed by method of the qualitative research with the using techniques: the anamnesis, the kinesiology analysis and the data analysis. The research set was formed by 2 patients of the cardiac surgery department of Hospital České Budějovice a. s. who were indicated for cardiac surgery intervention. The total time of the research was 4 weeks. The first purpose of this thesis was chart the effect of the preoperative physiotherapy on cardio-respiratory parameters and the condition of the patient. The second purpose was describe physiotherapy procedures carried out before and after the operation. I chose 2 research questions for the research: 1. What effect has the preoperative physiotherapy on cardio-respiratory parameters in patients with the chronic heart failure? 2. What are possibilities of physiotherapy procedures before and after the operation? The preoperative physiotherapy was carried out 5 times a week for 1 month in the home of the patient. It included a physical therapy, a respiratory physiotherapy and relaxation methods. Selected cardio-respiratory parameters were taken before and after each training unit. Results of therapy are processed through case studies. The component of the bachelor thesis is also a list of references, key words and abbreviations. Next it includes image attachments and graphs. The thesis can be a benefit for physiotherapists, general and professional public and for possible further research in this issue.
The use of physiotheraphy procedures for treating inflammatory rheumatic disease selected from the perspective of patients.

Rheumatology is a part of internal medicine that deals with diseases of musculoskeletal system. It is primarily devoted to diagnosis and treatment of these diseases. Rheumatoid diseases primarily affect joints and muscles, but other organs can also be affected endocrine glands, the nervous system, internal organs such as the kidneys, liver, lungs, human gastrointestinal tract, and skin. The diseases are classified as follows: inflammatory, degenerative, metabolic bone and joint diseases, extra-articular rheumatism, septic arthritis, etc. The causes of these diseases are unknown, but internal and external factors play an important role. They may be infectious, immunogenetic factors and effects of the external environment such as drugs and chemicals. The aim of this work is to map the use of physical procedures in the treatment of inflammatory rheumatic diseases. To do this, the following research questions were formulated: 1. Which are the most commonly used physical methods in the treatment of rheumatic diseases? 2. What effects does the application of physical therapy have on the patient? 3. How does inflammatory rheumatic disease affect the patient's life? 4. Which procedures can the patient apply in the home environment? 5. Are patients satisfied with physical therapy? Qualitative methods were chosen for the research part of this work and a semi-structured interview was used as the technique for data collection. The respondents were asked 28 questions. The research sample consisted of 10 respondents. The respondents were chosen from rheumatologic surgeries in Tábor and Pelhřimov. They consist of 5 respondents with a diagnosis of Bechterew's disease and 5 respondents with a diagnosis of rheumatoid arthritis. The interviews were conducted as previously arranged in the respondents' homes or via email. The results show that in most of the respondents the disease occurred between the ages of twenty and thirty. The first symptom was a painful and swollen joint. They most often use disease modifying drugs, anti-inflammatory drugs, and analgesic drugs for the treatment. Examples of disease modifying drugs include Methotrexate, Sulfasalazine, Aflamil, and Arava. Aulin is among the most commonly used NSA. Enbrel was mentioned by the respondents as a preparation used in biological therapy. Among the regime measures which the respondents take to reduce symptoms, were most often mentioned home workouts, changes of diet, compresses, ointments, and other compensation aids. As regard physical therapy, the respondents were most satisfied with balneotherapy, namely with massages and warm packs. The most used method is magnet. Physical therapy primarily helps to ease pain and relax muscles and stiff joints. Nurses educate patients about drug effects, the changes that are to come in their lives and to which they need to prepare; about the importance of workouts, rehabilitation, and compensatory aids. The quality of the respondents' lives is substantially limited by pain, stiff joints, and severe fatigue. For these reasons, the respondents are unemployed and have to receive a disability pension. In the initial period, the respondents suffered from the feelings of anxiety, helplessness, hopelessness, and depression. The family plays an important role in this context, helping and supporting the respondents. According to the data obtained it can be concluded that physical therapy helps to customers with rheumatoid inflammatory disease to relieve from pain and stiffness in joints. The course of the disease can be influenced by regular workouts that can help ease the symptoms. In some cases, the symptoms disappear for some time. It is very important to appropriately inform customers about their disease, therapy, rehabilitation and the changes they will have to undergo in life. The doctors and nurses deal those problems in the rheumatic clinics and spas. This thesis points to common problems of patients with inflammatory rheumatic diseases.
Physiotherapy for patients with thoracic spine injury (without spinal cord injury) in the early period after injury

This bachelor thesis deals with traumatic spinal cord injury (no lesion of the spinal cord) with a focus on the thoracic area. Fractures of the thoracic vertebrae occurs most frequently in traffic and bicycle accidents or falls from a height. Great influence also has osteoporosis (especially in postmenopausal women). Doctor´s assessment is followed by conservative or operative treatment and rehabilitation. The theoretical part consists of the sum of knowledge concerning this issue. There are explained the basic anatomical structures, the stability of the spine, definitions and divisions of injuries and fractures, fracture diagnosis and types of treatment. Also the effects of fracture healing are discussed. Subsequently it is focused on physiotherapy methods, which can be used in the treatment of fractures of the thoracic vertebrae. Effective physiotherapy is in place and might include practicing good posture, physiological respiration, restoration of movement stereotypes, briefing care scar, autorelaxation or motorization of the thoracic area. For the practical part of the thesis has been used qualitative research method. Physiotherapy in three patients with fractures of the thoracic vertebrae is described there. The first meeting included the history-taking in the form of a structured interview followed by entrance kinesiologic examination. It consisted of static testing of all respondents with aspection front, back and side, breathing wave dynamic tests and walk test. In addition, a measurement of distances on the spine was made. Another examination was testing of postural stabilization and postural reactibility for which they were selected three tests - diaphragmatic test, trunk flexion and extension test by Prof. Pavel Kolář. The therapy lasted eight weeks and individually at each patient's home. The aim was to map the possibilities of physiotherapy techniques used in patients with spinal thoracic spine (without damage to the spinal cord). Furthermore, the design and implementation of short-term treatment plan for individual patients and comparing their status at the beginning and at the end of research in relation to their degree of affinity to the active physical exercise. The therapy was finished with output kinesiological analysis. The probands were also subjected to an unstructured interview, in which each told their subjective evaluation of the progress of the research. This bachelor thesis could serve as study material for students and health professionals and to raise awareness of the general public about the impact of physiotherapy for pain relief and increasing range of motion in patients with fractures of the thoracic vertebrae. Equally essential purpose is to raise awareness of patients about their treatment options.
Physiotherapy as a component of an active stance to a senior´s age

Problems of senior age are currently and in the view of demographic situation an up-to-date social issue. Many branches of science and various organizations deal with senior age, its particularities in order to help to cope with difficulties which are usually associated with aging and old age. Bachelor thesis involve in the issue of senior age in its active experience which might be strengthened by physiotherapy and medical rehabilitation. The theoretical part of thesis focuses on description of senior age, possibilities of physiotherapy and medical rehabilitation in the old age and on the role of a physiotherapist in the health and social services. The practical part of bachelor thesis assesses questionnaires of an anonymous survey which has been carried out in several health and social services of chosen towns in the Plzeň region (Plzeň, Klatovy, Sušice) and South Bohemia region (České Budějovice, Strakonice, Vodňany). The questionnaires assessed whether a standard offer of those health and social services provided kinesiotherapy and various forms of physical therapy and whether the different offer showed the different satisfaction of seniors. The results show that some forms of physical therapy in the chosen health and social services are not represented, nevertheless this fact does not considerably influence the seniors´ satisfaction. In fact the respondents relatively seldom use provided services of physiotherapy and medical rehabilitation. That is why it would be helpful to extend the offer of physiotherapy and medical rehabilitation in the health and social services and at the same time to increase seniors´ information about the beneficial effect on their physical and mental health.
Posibilities of Physiotherapeutic interventions in children with cerebral palsy

Physical therapy plays an important part in the process of treatment of this disorder. Which in this case for life. The objective of my thesis was to sum up theoretical knowledge and information on CCP and on physical therapy of children suffering from CCP. The first task was to set up an overview of options and alternatives of physical therapy for children with CCP. The other task was to work out case reports of two children with the CCP diagnosis. Then a therapy plan, based on the knowledge from the theoretical part and on the patient´s examination, was made up. A combination of therapy techniques was proposed (the Reflection Method of Locomotion {--} a method developed by M.D. Vaclav Vojta, Proprioceptive Neuromuscular Facilitation (PNF), Basal programmes and sub programmes) suitable for a client with regard to his current state, his needs and abilities. The results showed that combination of methods in therapy is beneficial for a positive influence of motoric skills of a child, improvement of motion in particular joints and for a positive mental condition of a patient This thesis might be used as a set of helpful information for physiotherapists in clinical practice, who deal with CCP, and also as a guide for parents of children with the diagnosis of children cerebral palsy.
Effect of static and dynamic load in the treatment of osteoporosis in women in klimakterium

In the theoretical part I summarized theoretical knowledge related to this problematics. I described the anatomy, the function of a bone and the principals of its remodelation. I also summarized the pathogenesis od osteoporosis, its demonstrations, diagnostics possibilities and treatments focused on movement therapy. Besides this I elaborated on the possibilities of substitution therapy and tratment with osteocalcin. I also summarized the effects and possible sources of minerals and vitamin D. Since they are closely related on the metabolism and related ilnesses. At the conclusion of the theoretical part I elaborated on the investigation methods used in physiotherapy which even I was using in my research in order to assess the course of the therapy. In the practical part of the bachelor´s work I used the qualitative research method. I worked up the results thereof by the form of casusistics. I conducted the research at the outpatient rehabilitation facility at Policlinic Jih MEDIPONT s.r.o. The tested set of this research consisted of two patiens. The first one suffered from the form of osteoporosis abut on osteopeny and had no physical problems related to this ilness. In this patinent I chose a dynamic kind of physical burden. The second on had a little higher degree of osteoporosis and she came to the rehabilitation because of her backache, which was likely the consequence of this illness. This patient undergone static exercise. The aim of the work was to assess which of these attitudes is going to have a bigger influence in the progression if the ilness. However, the results of both patients show similar success of both attitudes. Therefore there is no difference in terms for which kind of physical facility we decided and according to my opininon it is even suitable to combine both.
Special relaxation and regeneration techniques for health improvement(cryotherapy).

The theoretical part of my thesis covers amount of erudite information about whole-body cryotherapy method and sums up its transparent overview. In the practical part all results of my practical research are completed and evaluated. This research was focused on detection of whole-body cryotherapy knowing level with regard to age, sex, life style, and place of residence. Thanks to foundations and papers this work also proves, that we can include the whole-body cryotherapy in medical procedures. This fact is supported by experts{\crq} discussions and clients{\crq} experiences of whole-body cryotherapy. At the end there are referred some options how to spread knowing about the method of whole-body cryotherapy among public.
