Financing of social care services for the elderly in the Czech Republic - current problems and challenges

The thesis "Financing of Social Care services for the Elderly in the Czech Republic - Current Problems and Challenges" deals with analysis of system of social care services for the elderly in view of the fact that the current financing form is subject to criticism for its inefficiency. The aim of this thesis is to critically reflect changes in the provision and financing of social care services for the elderly in the Czech Republic after 2007 in the context of current challenges related to long-term social care and health care. The thesis in its introduction first deals with the formulation of the research problem, deals with particular targets, research questions and the methods of data collection, which are the methods of analysis and public policy making and qualitative research methods. It also discusses the theoretical background of social services and their financing. The next chapter of the thesis deals with the system development of social services and changes in the provision and financing of social care services, which occurred in connection with the adoption of Act No. 108/2006 Coll., On social services. Legal aspects of care for dependent persons More than half a million people in the Czech Republic is directly affected by the issue of care for dependent persons. It concerns not only those who are relying on care due to a physical or mental disability, but also those who provide the necessary care. Regarding the providers, so-called informal carers constitute majority of them. They are often family members who provide care without any official formalization, contract, or registration. However, as a group, informal carers are repeatedly overlooked. While emphasising the rights, duties and needs of both dependent persons and carers, this paper aims to comprehensively evaluate the current legal framework regulating the care for dependent persons. Firstly, the system of care in the Czech Republic is generally described followed by the presentation of specific forms of care highlighting the position of informal carers. Secondly, the paper brings up an analysis of basic activities of daily living that are crucial for the assessment of dependence. Subsequently, the administrative procedure in which the assessment is undergone is closely depicted. Functionality of care allowance

My work offers a view of the care allowance as one of the sources of funding for social services, which over time has become a support for people with disabilities, which greatly helps sustain the limited number of social services - residential or field. Allowance realy gives recipients purchasing power pay part of the service / private assistance at their decision. I perceive the care allowance as an important state support for people dependent on the help of another person and I consider it a tool that should help to live a free life for people with disability. The allowance became very non-individualized (inflexible) - it is unable to respond to changes or adapt to the needs of its beneficiaries. Act on Social Services is strict in the individual approach, setting, way of working with people in the framework of social services, but in the care allowance practice does not apply this. The granting and payment of contributions is not individualized, each of them must be assigned / be classifiable in four levels. Ruthless whether it needs it and it uses the level of support it need. The care allowance system does not have one problem, but several that they do a lot. This is nothing without which the system should not function, it's something that could be better.
Care allowance as a pillar of care for person with disability

The thesis is focused on the issue of care allowance. The theoretical part contains basic information about this social benefit. There are described various forms of administrative proceedings. It is imperative that you know the social services that you can turn to if necessary, so they are also mentioned in the work. Attention is paid to the characteristics of the social worker. The practical part aims to give the reader an insight into this issue. There are actual stories of people here to see where difficult people can be. The questionnaire survey found that the respondents had a negative experience with the care allowance, as the proceedings took several months. Furthermore, it was found that the primary information about this social benefit is learned from the doctor. Research has shown that they most often need help with shopping, household cleaning, or hygiene. These are tasks that are physically demanding for the respondents and often helped by family members.

Long-term care and non-formal persons caring the clients

My diploma thesis "Long term care and non-formal caretakers caring for clients" deals with persons who provide long-term and non-formal care for seniors in the home environment. This diploma thesis is divided into two parts, one for theory and one for the research. The theoretical part deals with the long-term care and with a number, type, economic status of non-formal caretakers. There is also a brief passage about the care benefit in the context of long-term care. One chapter deals with services that might be useful for the non-formal caretakers. The aim of the research part is to identify the most severe difficulties that the non-formal caretakers face. There are two main questions in the research part, " What barriers let to the termination of the care of the non-formal caretakers?" and "What are the dilemmas of the non-formal caretakers when deciding the transfer to a social institution?" There is also a description of the method and techniques of a structured interview and its analysis. The research showed that the most severe difficulties are the lack of finances for the home-care, limited or unsuitable spaces for the long-term care, restrictions of the free time, health problems of the care-takers and high physical and psychological demands. The main reasons responsible for the ending of the care are high demands, health problems of the caretakers, and fears of the future. Another dilemma that has been proven is the necessity of choosing between career and providing of non-formal care.
Perception of the Medical Assessment Service by Applicants of the Care Allowance.

In bachelor thesis I focused on the perception of the medical assessment service, whose aim is, besides other tasks, also to assess the health condition for the purpose of the care allowance. The aim of the theoretical part is to map the process of assessing health condition of the applicants of the care allowance and in this part, I describe the classification and tasks of the medical assessment service, medical assessors, newly established position as the specialist assistant of the medical assessor, assessment criteria, principles of health assessment and principles of assessing the ability to handle basic living needs. In the third chapter I outlined perceptions, prejudices, stereotypes and mistakes in social perception. The aim of the empirical part is to find out what are the stereotypes and prejudices by applicants of the care allowance. Three hypotheses have been established, which were investigated in the quantitative research by the questioning method, through the semantic differential technique. The selection sample was chosen randomly from the records of received applications for assess the health condition for the purpose of care allowance and was made up of 400 respondents. The return rate of questionnaires was 62 %, so the research sample was composed of 212 respondents. To evaluate the data and verify the hypotheses, statistical methods were used to sort the data for first and second degree. The research confirmed only the hypothesis that applicants for care allowance are not satisfied with the length of assess the health condition. Other 2 hypotheses have been disproved and it has been found out that applicants of the care allowance perceive the medical assessment service rather neutral, as well as they perceive neutral the objectivity of the assessment. The results of the bachelor thesis can be used by the state administrative bodies as an indicator of current perception of the medical assessment service by applicants of the care allowance and can be used in the process of assessing the dependence levels in non-legislative aspects.
Long-term Care And Non-formal Persons Caring For Clients

The aim of the diploma thesis themed Long-term care and the informal persons caring for clients is to identify both positive and negative aspects of the long-term care and its influence on physical and mental condition of persons taking care and their closest relatives. At first, the thesis deals with the issue of long-term care in the Czech republic and its division into two categories, formal and informal. Furthermore, the thesis presents the situation of the informal caregivers who have to face various obstacles arising from the nature of long-term care, such as their complicated relation with the formal institutions, lack of financial support, worsening of their lives' qualities and also lack of social recognition of their status. In order to retrieve the relevant information within the scope of the research question, the method using half-structured interviews was used. The research sample consisted of 7 female respondents from České Budějovice and its surroundings who were taking care of persons between the age of 5 and 71 years. The pesons suffered either physical or mental disorders. Some of them suffered both physical and mental disorder. At the beginning of the research the caregivers received two main questions. The first question was focusing on their perception of the current system of rules for informal caregivers, whether they support it or not. The second question aimed on describing the biggest positives and also the setbacks related to the care of person with some kind of disorder. The outcome of the interviews was that if a person decides to care of a close person with disorder there is a wide network of social services provided for him, even though it could be definitely improved. The issues which could be improved are primarily financial income of caregivers, accessibility and afffordability of relief services, providing more information and education for the informal caregivers, and last but not least the opportunity for caregivers' self-realization and their position at the labour market.
