The Possibilities of Artificial Lung Ventilation in Pre-hospital Emergency Care

The bachelor thesis deals with the possibilities of artificial pulmonary ventilation in pre-hospital emergency care. The thesis consists of theoretical and practical parts. The theoretical part first informs the reader about the anatomy and physiology of the respiratory system. Then it deals with the topic of artificial pulmonary ventilation, focusing, for example, on its history, goals, indication or complications. The thesis also introduces the topics of non-invasive ventilation or oxygen therapy. We must not forget to introduce the possibilities of securing the airway and the competences of paramedics. The practical parts contains the results of a research survey obtained from paramedics by means of semi-structured interviews. The collected data were analyzed and divided into 11 categories based on open coding. The results of the research investigation showed that the most commonly used method of artificial pulmonary ventilation in prehospital emergency care is an invasive method. Most paramedics have experience with this method only as a witness to it. Paramedics are aware of this issue mainly through annual training.
Interpretation of electrocardiograms in the resuscitation department and in emergency pre hospital care

(v AJ) 1) introduction - introduction to the problem The diploma thesis on the interpretation of electrocardiograms by nurses and paramedics in the resuscitation department and in urgent pre-hospital care includes a theoretical summary of ECG pathologies, as well as addresses the issue of correct interpretation of electrocardiograms in relation to gender, age, workplace experience, and suggestions for improvement. 2) goal formulation, methodology The goal of the diploma thesis was to map the level of knowledge in interpreting electrocardiograms by nurses and paramedics in the resuscitation department and in urgent pre hospital care, depending on age, gender, length of experience, and job placement, with a focus on ECG interpretation. For the empirical part of the thesis, which deals with answering predetermined goals, a quantitative research method was chosen, conducted in the resuscitation department and emergency medical services. Data collection was carried out through a questionnaire survey using a self-constructed questionnaire. Motivation and perception of the paramedic profession

In my bachelor thesis I examine the motivations of paramedics and their perception of the profession itself. The introductory theoretical part introduces the basic principles of pre-hospital emergency care and the personality traits often present in these professionals. Furthermore, the historical development of the ambulance service is explored, which influences shape of the proffesion. The subsequent section analyzes the qualification education required to work in this field and the legislation regarding the competencies of paramedics. The second part of the thesis is devoted to motivation, where the characteristics, type and division of motivation, well as the factors of motivation and demotivativation in general are discussed in more detail. Special attention is given to the motivation of emergency medical technicians, including an analysis of motivating and demotivating factors specific to this profession. The research section is based on qualitative research using semi-structured interviews,The research population is paramedics. The first objective of this thesis is to map out the motivation for working as an paramedics. The second objective is to find out the perception of the profession among paramedics. This thesis can be perceived by the management of the Medical Rescue Service as a tool to improve the motivation of its employees. The information obtained from the research can be an important factor in deciding whether prospective applicants want to study in the field of paramedics.

The theoretical part deals with the basic concepts and definitions of digital health care, especially electronic health documentation and digitization of emergency medical services. The practical part examines the scope, use and benefit of digitization in pre-hospital care. It also deals with the satisfaction of paramedics with digitization within the ZZS.
Invasive meningococcal disease in prehospital care

The topic of this bachelor's thesis is Invasive meningococcal disease in pre-hospital care. The thesis is divided into two parts- the theoretical part and the practical part. The theoretical part focuses on the anatomy of CNS, invasive meningococcal disease itself, i.e. information about the pathogen, and clinical forms, procedure in pre-hospital care and material and technological equipment of the ambulance car. Two objectives were established for the practical part of this bachelor's thesis. The first objective was to map the provision of pre-hospital care for patients with suspected invasive meningococcal disease. The second objective was to map the material and technological equipment available for treatment of the patient with suspected invasive meningococcal disease. Qualitative data collection was used to achieve the objectives, specifically the form of semi-structured interviews. These interviews had pre-prepared sixteen open questions and in case of an inadequate response follow-up questions were asked. The interviews were conducted with paramedics of the Vysočina Region from the regional centres of Pacov, Pelhřimov and Kamenice and Lipou. Seven interviews were conducted in total. The research proved that paramedics are oriented in the given issue even tough the call-outs to those patients are not routine and they have sufficient knowledge to provide urgent pre-hospital care for a patient with suspected invasive meningococcal disease. The summarization of the outcomes, the evaluation of achieving established objectives and also my recommendations for paramedics can be found in the conclusion of the bachelor's thesis.
Psychohygiene of a paramedic

This study focuses on the psychohygiene of emergency medical responders, aiming to understand the professional demands on their mental health and well-being. The work analyzed stress factors, burnout syndrome, and mental health, examining the influence of stress on individuals and the importance of mental hygiene and self-care. The main objective was to ascertain the perception of psychological demands among emergency medical responders in prehospital care. The theoretical part of the study addresses issues of mental disorders, the influence of stress, and its effects on emergency medical responders. The practical section sought answers to predetermined hypotheses through research. The research was based on a questionnaire survey distributed among emergency medical responders. Based on the findings, it was observed that many responders experience stressful situations related to their work, which can have a negative impact on their mental health and well-being. We found no significant difference in the perception of psychological demands between genders. There was no difference in the perception of psychological demands based on length of practice. We also identified various activities that emergency medical responders engage in for their mental hygiene. The results indicate that providing support for stress management and mental health care is crucial for emergency medical responders in their professional lives. The conclusions of this study may contribute to a better understanding of responders' needs and the support of their mental health and well-being in the workplace. In the conclusion of the bachelor's thesis, in addition to summarizing the research results and evaluating the fulfillment of objectives, recommendations for current practice among emergency medical responders are provided. These recommendations are derived from the findings and can serve as a practical guide for improving the care of mental health among emergency medical responders in their professional environment.
Spine and spinal cord trauma in prehospital care

The bachelor thesis is focused on Spine and Spinal Cord Trauma in prehospital care. Spine and spinal cord injuries should always be thought of if the injury was preceded by a fall from a height, a car accident or a jump into unknown water. This thesis consists of two parts. The first part, theoretical, deals with anatomy of the spinal cord and spine, etiology, pathophysiology of trauma, prehospital care, transport and immobilization devices, diagnosis of trauma, treatment and procedure of prehospital care in children. For the practical part, two objectives were set. The first aim was to map the care of the patient with suspected spinal cord injury in pre-hospital emergency care. The second objective was to map the material and technical resources of the medical rescue service in the Vysočina region available to the paramedic when treating a patient with suspected spinal cord and spinal cord trauma. The research part was conducted using a qualitative method, using a semi-structured interview. The interviews consisted of a total of twelve pre-prepared questions and were conducted with paramedics of the Vysočina Region Ambulance Service. These were from the Třebíč and Moravské Budějovice bases. The interviews were recorded on a dictaphone, then transcribed and analysed using the pencil and paper method. Four categories were then created: diagnostic, procedure, specifics and aids. The categories were then divided into sub-categories and converted into diagrams according to the responses. Eleven paramedics were randomly selected for the research and agreed to be interviewed in advance. The research investigation has shown that paramedics have gaps in some aspects. This is particularly the case for the management of suspected spinal cord injuries. Other gaps were in the transfer of the patient to the spineboard, where two informants did not know what the aid was. But the overall results were satisfactory.
Self-defense for medical staff of the Emergency Medical Service of the South Bohemian Region

The subject of the bachelor's thesis is "Self-Defense for Medical Staff of the Medical Rescue Service of the South Bohemian Region." This work explores the various possibilities of self-defense - both physical and verbal - applicable to the disabling the aggressor. The theoretical part is focused on the current state of attacking exit crews. It also includes training of medical rescueworkers, including cooperation with the Police of the Czech Republic, as well as some factors that can greatly endanger healthcare professionals in their work. In the theoretical part, both the basics of self-defense - verbal and physical are mentioned, including the procedure of simple defensive techniques, for example against kicks, grips or strangulation. If an attack cannot be nullified by self-defense, we have a number of defenses, usable against the aggressor. Such a solution to the conflict also carries many risks in the area of criminal law, which is why the last chapter of the theoretical section is devoted to legal legislation. The data to process the practical part was obtained in the form of qualitative research using a semi-structured interview method with 10 employees of the Medical Rescue Service of the South Bohemian Region. We investigated whether they had experience of verbal or physical assault, how they prepared for possible assault, and whether they had any self-defense means by which to avert imminent attack in the course of their profession. The results of the investigation indicate that cooperation with the Police of the Czech Republic and early detection of the dangerous situation of the Medical Operations Centre tend to be crucial. All 10 respondents have experienced verbal aggression, half of them have experienced physical aggression. A few respondents expressed interest in self-defense courses, and those who keep fit by moving are interested in contributing to sports activities. This work can be used as an educational material for health care professionals or as a source of information in the area of self-defense and security in the South Bohemia Region.
Monitoring in neurointensive care

Monitoring in neurointensive care is one of the important parts of caring for a patient in critical condition. Monitoring enables tracking of values and parameters continuously or intermittently over time. It evaluates the patient's current condition and draws attention to possible changes that may occur as a result of the development of a primary disability, and warns in time of the development of a secondary disability. Neuromonitoring is divided into invasive and non-invasive monitoring. The main goals of the presented work were to detect the knowledge of nurses and paramedics from the environment of the anesthesiology-resuscitation department in the field of neuromonitoring. To discover whether nurses and paramedics in the anesthesia-resuscitation department are familiar with neuromonitoring procedures. And the final goal was to find out whether the recommended procedures are followed in the framework of neuromonitoring in intensive care. The empirical part of the analysis was carried out through quantitative research using a non-standardized questionnaire. The research was conducted among nurses and paramedics who have experience with neuromonitoring in the anesthesia-resuscitation department. To the analysis were subjected 158 complete questionnaires. Based on the statistical analysis of the results, a connection was established between the respondents' highest level of education and the level of knowledge. A connection was also found between the respondents' length of experience and level of knowledge. On the contrary, the connection was not shown in the relationship between the age of the respondents and the level of knowledge.
Evaluation of ECG in prehospital care

This bachelor thesis deals with the issue of ECG evaluations in pre-hospital emergency care by a paramedic. The thesis is divided into two parts, theoretical and practical. The theoretical part focuses on the definition of a paramedic and his/her competencies, instrumentation, and anatomy of the cardiac conduction system. Furthermore, it describes the methods of ECG evaluations, types of arrhythmias and their treatment in pre-hospital emergency care. The major objective of the thesis was to map the ability of paramedics to interpret the waveforms of ECG recordings. The practical part of the bachelor thesis was based on a quantitative research investigation carried out in the form of non-standardized questionnaires. The research sample consisted of 51 paramedics from the Region of South Bohemia. The research took place from March to April 2023, with the participation in it being entirely voluntary. The collected data were subsequently analysed and compiled into graphs, which were completed with a commentary. The knowledge in interpretation of ECG waveforms was mapped by a series of 24 questions in which the respondents could obtain a total of 35 points. The high knowledge of the respondents is demonstrated by the fact that three quarters of the respondents achieved 80 percent or more correct answers. Two hypotheses were determined in the research part. The first hypothesis suggested that the knowledge in interpreting ECG waveforms is affected by the length of experience. The respondents working in emergency medical services for 0-3 years had the highest score (median 93%). However, the result of the Kruskal-Waliss test revealed no statistically significant difference between the four groups compared (p = 0.51). The second hypothesis assumed that the knowledge in interpreting ECG waveforms is influenced by the type of previous workplace. The respondents who had worked in the accident and emergency department before joining the emergency medical service were the most proficient in ECG assessment and interpretation (median 96%). Here again, the Kruskal-Wallis test revealed no statistically significant difference between the four groups compared (p = 0.31). The obtained results can provide useful information for the possible implementation of an educational seminar for paramedics.
