Quality control management of extracorporeal circulation with Data management system
Fichtl, Jaromír ; Rulíšek, Jan (advisor) ; Mašín, Jaroslav (referee)
During extracorporeal circulation are typical fluctuation of parameters of basic life functions. These variations of values can have a direct impact on postoperative recovery. May even lead to temporary or permanent damage to organ function. Mean arterial pressure is one of the basic unit of tissue perfusion. If there is a drop in mean arterial pressure below 50 mmHg, there may be organs hypoperfusion associated with desaturation of venous blood. In our study focused on gathering data in the management of cardiopulmonary bypass during cardiac procedures by using the data collection would Stocker De. We focus on course of the values of mean arterial pressure and actual venous blood oxygen saturation. The group included 45 patients meet the criteria: valve surgery, surgery is an operating room No.3, surgery in normothermia. In 22 patients were recorded during cardiopulmonary bypass hypotension for more than four minutes ( Group A). In the remaining 23 patients the episode of hypotension has not been reported (Group B). Both groups were compared. We compared the average time of hospitalization, the average time of intubation and the incidence of neurological complications. The average time of hospitalization was in Group A - 180 hours, Group B - 210 hours. The average time of intubation was in Group A - 114...
THE AUDIO COMPONENT IN TEACHING CZECH AS A FOREIGN LANGUAGE
Smrčková, Tereza ; Šebesta, Karel (advisor) ; Mašín, Jaroslav (referee)
(in English): This thesis deals with the issues of teaching listening in Czech as a foreign language. It is based on studying theoretic sources, structured observing and the comparison of didactic materials, on questionnaires and informal interviews. In its theoretical part, it brings a brief overview of present knowledge in this field, using predominantly foreign sources that deal with teaching English as a foreign language. The second part enables the comparison of various didactic materials from the points of view acquired in the previous chapter, and it also provides an overview of the typology of listening tasks, including their evaluation. The last chapter designs some new sample materials based on what has been found out about their imperfections in this field so far, and it also mentions some suitable resources for designing the new listening tasks. The merit of this thesis on the theoretical level consists of making the main results of contemporary Anglo-American science accessible for the Czech auditory, on the practical level it lies in presenting an overall list of listening tasks and showing (in the Czech materials) some so far unknown possibilities of teaching the listening comprehension, these alternatives being accompanied with practical examples.
Didactic usage of video in Teaching Czech as a foreign language
Šturmová, Kateřina ; Mašín, Jaroslav (advisor) ; Šormová, Kateřina (referee)
Usage of a video at real-time Czech lessons (not distantly through YouTube) presents to students a very attractive way of learning a language and the socio- cultural competence quickly and entertainingly. The aim of this paper is to sum up findings of the specialized literature and to create special videos useable in foreign language classes of A0/A1 level based on those findings. The videos will be focused on development of sociocultural competence, the themes shall be following: 1.Lesson of the good manners in the Czech environment part 1. (formal X informal language) 2. Lesson of the good manners in the Czech environment part 2. ( Traveling with the Public transport, how to behave) 3. Holidays ( Christmas, Easter ) The empiric part of this thesis will be based on the hypothesis that using video in the classroom will make the acquisition of the sociocultural competence easier. The video materials will be shown in a couple of groups of student at the A0/A1 level while couple of group of other student will study without them. Those students shall get the competence through printed materials (for example books or materials prepared ad hoc by the teacher) and explanation of the teacher. There will also be prepared some supporting activities related to the videomaterials in order to confirm the...

Linguistic pragmatics in Teaching of Czech as a foreign language
Mašín, Jaroslav ; Šebesta, Karel (advisor) ; Hrdlička, Milan (referee) ; Škodová, Svatava (referee)
The focus of dissertation belongs to the field of learning Czech as a foreign language and deals with the acquisition of pragmatic competence of non-native speakers. Whereas this is a broad area, the work concentrates on speech act request. The aim is to describe how two students of Czech acquired the speech act during a one-year intensive course. The theoretical background is given in Chapter I. The term Interlanguage Pragmatics is introduced to Czech readers. The Chapter further maps what the pragmatic research of non- native speakers generally deals with and what questions it asks. Attention is paid to possible methods for collecting data for research of pragmatic competence, what kinds of errors non- native speakers usually make, what environment is suitable for the acquisition of pragmatic competence or how individual differences among students affects pragmatic competence. In the second chapter attention is paid to speech act request. We present an overview of major longitudinal studies. On the bass of this longitudinal research it was found that students of English go through five stages of development when acquiring request. The stages are presented here in detail. In the research part of the dissertation the investigation of how two non-native speakers (a student from Taiwan and a student...
Assessment of suitability of minimal extracorporeal circulation during cardiac surgery
Nikitinský, David ; Rohn, Vilém (advisor) ; Mašín, Jaroslav (referee)
Nikitinský, David - Assessment of suitability of minimal extracorporeal circulation during cardiac surgery First Faculty of Medicine Charles University in Prague Thesis supervisor: doc. MUDr. Vilém Rohn, CSc. The aim of the present Thesis is to evaluate the suitability of use of minimal extracorporeal circulation (MECC) by means of comparison with conventional extracorporeal circulation. In this work, I retrospectively compared two groups of patients who underwent heart surgery, one treated with MECC, and the other with conventional extracorporeal circulation - ECC. The patients were divided into two groups of 29 (MECC) and 51 (ECC) probands. Male patients prevailed in both groups (69% MECC; 80.4% ECC). The mean age was 70.5 years (MECC), 65.6 years old (ECC). In this work, I evaluated the amount of postoperative blood loss, number of units of blood derivatives used, the total duration of intubation, length of stay in postoperative intensive care unit, the execution time of surgery until discharge. I also evaluated the frequency of neurological complications, delirium, non-infectious pulmonary complications, gastrointestinal complications and other complications. The frequency of neurological complications in the MECC group (4 patients, 13.8%) was increased in comparison with the ECC group (1...
Comparison of noradrenalin medication during cardio surgeries using extracorporeal circulation
Samková, Markéta ; Špunda, Miloslav (advisor) ; Mašín, Jaroslav (referee)
The theme of this thesis is a comparison of two methods of medication a noradrenalin hormone during surgeries using extracorporeal circulation. Both of these methods - continuous and bolus medication are common used in praxis to regulate blood pressure, but it is not uniquely determined which one is more convenient. The thesis has two parts - theoretical and practical research. The theoretical part is concerned with the brief description of extracorporeal circulation technique, including its connection, conduct, anatomical and physiological description of affection of blood pressure and influence of extracorporeal circulation over natural regulation mechanisms. The practical part is concerned with methodical description of the research, description of used tools and statistic evaluation of two groups of patients (each group was medicated via different method - continuous and bolus). The goal of this thesis is to discover, if any of these two methods of controlling the blood pressure is not more convenient regarding the after surgery keeping vasomotoric tonus and the length of convalescence.
