Microsporidial infections of cause by \kur{Encephalitozoon cuniculi} genotypes II and III in immunocompetent and immunodeficient hosts
KOTKOVÁ, Michaela
Microsporidia are obligate, unicellular intracellular parasites that have a unique way of entering the host cells. Encephalitozoon cuniculi was the first microsporidia found in mammals. This parasite has a broad host spectrum, infects a variety of host?s cells and is found in most tissues of an infected individual. The course of acute and chronic infections caused by various genotypes of Encephalitozoon cuniculi was investigated by using different strains of mice. The aim of this study was to clarify the origin and extent of neglected latent microsporidiosis in immunocompetent hosts. Furthermore, the role of individual lymphocyte subpopulations (CD4+ and CD8+) in protective immunity against microsporidia was examined using CD4 and CD8 knockout mice experiments, and also the efficacy of treatment with albendazole was studied. The obtained data help to elucidate the potential danger of latent microsporidiosis, which may pose a risk to recipients of organs from infected donors in human medicine. A considerable part of this thesis was the study of transplacental transmission of microsporidia from mother to a fetus, which answered the question about the risks associated with prenatal infections of fetuses in chronically infected mothers.
The course of microsporidiosis caused by \kur{Encephalitozoon cuniculi} in immunocompetent and immunodeficient mice
KOTKOVÁ, Michaela
The course of microsporidiosis caused by Encephalitozoon cuniculi in immunocompetent BALB/c mice and immunodeficient SCID mice was screened using molecular methods. The site of infection in organs was located using molecular and histology methods. The effectiveness of albendazole treatement and possibility of infection relapse after immunosuppresion (cyclosporine A, tacrolimus, mycofenolate mofetil) was also studied. Moreover, the course of excretion of microsporidial spores in feces was monitored during the whole time of experiment.
Autism as a brand and nurse's apart in collaboration with family.
KOTKOVÁ, Michaela
Autism spectrum disorders, as some of psychiatric disorders, which also include childhood autism, atypical autism, Asperegs´syndrome, childhood disintegrative disorder, and Rett syndrome, are considered to be significant anomalies in children´s development, which are characterized by problems in communication, social interaction and behavior. These are the typical symptoms creating barriers between children and their families, medical personnel, or environment. Despite all efforts to improve the care for children with autism spectrum disorders, the situation is generally unsatisfying. Counseling services are not available in all regions and specialized preschool and school facilities are missing in a lot of places. Also, specialists´ awareness is often insufficient and even in health care facilities unfamiliarity with the issue of children with autism spectrum disorders may be seen. On the part of the public a negative reaction, misunderstanding and prejudice against these children and their families are still being expressed, making autism spectrum disorders stigmatizing. A nurse as a professional provides the care through the nursing process, in which her role consists not only in provision of the direct nursing care, but she also takes on the role of the child´s rights advocate and a mediator for information exchange between the child, the parent, the doctor and herself. In the care of children with autistic spectrum disorders, a nurse provides not only nursing care, but also educational care with the aim to support the child's development. In order to succeed in her efforts, the nurse needs a large portion of empathy, patience and communication skills and all the support and cooperation on the part of the child's parents. The above conclusions are related to objectives of the research carried out using in-depth interviews, during which we wanted to determine if this disease is stigmatizing for families caring for a child with autism and also the way a nurse participates in cooperation with the family. The third objective was to define the specifics of nursing care for children with autism spectrum disorders, from which standards of nursing care for these children were established. Based on the objectives and the study of available resources, four research questions were stated. First: Is the disease stigmatizing for families caring for a child with autism? Second: What is the impact of autism on families caring for a child with this disease? Third: What is the way a nurse participates in cooperation with families caring for a child with autism? And fourth: What are the specifics of nursing care for a child with autism? All the objectives were met. We hope that this thesis will provide nurses with so much information on the issue of nursing care for children with autism spectrum disorders, so that nurses could provide quality care based on the child's needs, and the cooperation with the family could become beneficial and joyful.
Influence of tick saliva on cytokine IL-17 production by murine splenocytes.
KOTKOVÁ, Michaela
Aim of this study was to quantify production of IL-17 cytokine. IL-17 was induced in murine spleen cells after stimulation of the T cell receptor through CD3 crosslinking. Futher, the effect of salivary gland extract (SGE) and tick saliva on production of IL-17 was investigated. The mechanism of the interaction was also studied.
The Attitude of nurses to introduction of nursing process into the practice
KOTKOVÁ, Michaela
Abstract The attitude of nurses to implementing new nursing process methods into practice The contemporary modern health care philosophy stemming from the recommendations of the WHO ( World Health Organization), the ICN ( International Council of Nurses) and the EU (European Union) aims to specify the roles of a nurse and show the direction for a quality education of the medical staff and an individualized nursing care focused on patients´ needs. The nursing process has been changing constantly and it is necessary to introduce new methods into practice. The nursing process is a series of mutually connected activities done for the benefit of the patients and in collaboration with them if it is possible. The results of individual steps are recorded in the patient´s medical history which is substantial for this methodology. Nowadays, in majority of health care facilities in the Czech Republic the nursing process is an integral part of their care. The objective of this thesis was to find out nurses´ approach to the method and also whether they have enough knowledge to implement the method into practice. The results of the quantitative research by the questionnaire method I have conducted show that nurses´ attitudes to the modern nursing method have been changing favourably. They do not perceive the method to be annoying but they see it as a method that can help them better cope with their duties. I set several hypotheses. The hypothesis IV - nurses agree that the method enables to discover the patient´s real troubles better {--} was confirmed. The hypothesis V {--} nurses agree that the method enables to discover the patient´s potential troubles better {--} was also confirmed. On the other hand nurses still insist that the new nursing process brings about more paperwork {--} the hypothesis II proved to be correct. Nurses working in standard units in the hospital in Frýdek-Místek consider the new method more encumbering due to more paper handling. According to the data obtained by using the questionnaire method, nurses have enough theoretical knowledge on the nursing process. Only two questions asking about phases of the method were given false answers. Thus it is possible to conclude that nurses are well informed about the principles of the nursing process. Thereby the hypothesis VI was rejected - it supposed that nurses in standard units in the hospital in Frýdek -Místek did not have enough information on the new nursing process. The results of the research show that nurses agree with implementation of the methods into the nursing process. 57% nurses agree that the new nursing process will bring about a better care for patients, 54% nurses agree with implementation of the nursing process into practice. The hypothesis I - nurses working in the hospital in Frýdek Místek agree with implementation of the new nursing process methods into practice - proved to be correct. Also the hypothesis III - nurses are sure that new methods implementation will make for a better nursing care - was correct. A nurse is the key-person in the nursing process. To fulfill her crucial role it is necessary for her to be concerned with the new nursing process methods.
