Adherence to pharmacotherapy by idiopathic inflammatory bowel diseases

Aim of the work The aim of the work was to analyze patient's adherence (compliance) to the pharmacotherapy of inflammatory bowel disease in Czech Republic, as well as to analyze quantity and character of potential drug interactions in medications of the tested kohort. Our aims were : 1. to analyze adherence to pharmacotherapy of IBD: o in the patients cohort followed up in the single IBD Unit (Gastroenterological Centre, the 4th Medical Department, Prague) o in the patients cohort followed up in a few gastroenterological ambulancies in the Czech Republic 2. to analyze potential drug interactions in medications of IBD patients Results 1. Adherence to pharmacotherapy of IBD in the patients cohort followed up in the single IBD Unit (Gastroenterological Centre of the 4th Medical department, Prague) 177 patients were enrolled in the study (84 male, 93 female, 117 CD, 60 UC). The patients evaluated their adherence to the pharmacological treatment in an interview. From 47 patients, treated by mesalazine, the urine sample for HPLC analysis of mesalazine was taken. Aim: Analysis of pharmacy practice during dispensing of drugs (types and frequency of interventions and solving drug-related problems) and the influence of pharmacist`s education on proper dispensing practice and identification of drug-related problems. Method: During two periods in 2006 was monitoring two pharmacists in dispensing activities in basic pharmacy located in municipality with 15,000 inhabitants. Between these two observed periods was inserted education seminar, which was built on the analysis of the cases identified in the first phase of the project. There is a risk we will not have antibiotics effective against pathogenic microorganisms in future. It is therefore very important to make guidelines for rational prescribing and use of antibiotics. Aim: Analyze antibiotic consumption data package collected in given period depending on consumer age or sex. Analyze which group of doctor prescribes antibiotics in most cases. Analyze the most prescribed substances depending on different expression of consumption and compare obtained data with available data. Methods: Source of data is a database of employment insurance. Introduction and objective: This study observes consumption of antibiotics in patients treated with dialysis in The University Hospital of Hradec Králové in years 2006 and 2007. Pilot project investigate, which kind of antibiotics used dialyzed patients (DP) the most of all. Studies further compare information from two sets of DP from Czech Republic (Group I) and Lithuania (Group II) in specific aspects of antibiotic's consumption. Methodology: Retrospective analysis resulting from data evaluation from Nephris database of University Hospital of Hradec Králové by DP (2006&2007). Set of data of the Group II from Clinics of Kaunas Medical University - Lithuania was process by retrospective analysis too. GLMZ mode was used for statistical analysis. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Kralove, Charles University in Prague, Czech Republic Background: Studies on antiepileptic drug utilization are important for evaluation of epilepsy treatment and risk of antiepileptic drugs (AEDs). Aim: To describe group of patients with epilepsy, AEDs utilization patterns and adverse drug reactions (ADRs) of AEDs. Methods: Data were retrieved from medical records in University Thomayer's Hospital in period 2005 - 2006. Epileptic outpatients (n = 97) aged over 16 years were considered for analysis. Results: The age ranged from 17 to 72 years (mean 55), 62 % were women. Partial seizures were diagnosed in 62 % of patients. 31 % of patients were more then 2 years without seizures, 39 % of patients had more then 12 seizures per year. More then half (65 %) of the patients were treated with polytherapy. Lamotrigin was the most commonly prescribed AED (20 %), followed by valproate (18 %) and carbamazepine (17 %). Literature search Introduction: Osteoporosis (OP) is an important non-communicable disease, the incidence of which is increasing. It is a systemic metabolic skeletal disease characterized by reduced bone mass and changes in bone tissue quality. These disorders increase the risk of fracture and consequently have a considerable impact on public health. OP affects primarily postmenopausal women, but has recently become a great concern in men as well. Among the most important preventive (and non-drug therapeutic) interventions are a diverse diet high in calcium, adequate physical activity, avoidance of exposure to toxic substances and fall prevention. Multiple therapeutic strategies are used in attempts to restore balance between the bone resorption and the bone formation. A first-line intervention is calcium and vitamin D supplementation. Specific drug therapies (anti-osteoporosis drugs) are classified according to the predominant mechanism of action into anticatabolic or antiresorptive drugs (bisphosponates, raloxifene, calcitonin) and formation stimulating, anabolic drugs (parathormones), and drugs with a dual mechanism of action (strontium-ranelate). Knowledge of patients with diabetes mellitus about hypoglycaemia.

Nowadays, diabetes mellitus belongs to wide-spread diseases. 4-7% of population suffer from diabetes in developed countries and the number is increasing. Education of patients with diabetes mellitus ensures effective treatment and it contributes to the delay of diabetic complications. Therapy itself without correct and effective patient education can hardly lead to successful treatment. One of the frequent diabetic complications is hypoglycaemia, which can be fatal due to patient ignorance. The aim of this pilot study was to evaluate the degree of patient knowledge about hypoglycaemia, to monitor its distribution among patients and to analyze its possible risk factors. Cross-sectional observational study was used for the analysis. Data were obtained from structured direct interviews carried out among diabetic patients. The study was performed in Faculty hospital in Hradec Králové. Submission of patients into the study was random. Patients with diabetes mellitus types 1. and 2. visiting the diabetologic centre in Faculty hospital in Hradec Králové were included. Information was gained from 100 patients (109 addressed (92 %)).
