Case study of physical therapy care of patient after ischemic stroke

Title: Case study of physiotherapy treatment of a patient after the ischemic stroke Objectives: The main aim of this study was to verify my ability to work with the patient, his examination, making conclusions, proposed treatment and its subsequent implementation. Other aim was mapping of the literature on the subject. Methods: The first part of my bachelor thesis is theoretical and it is aimed on stroke. There are described anatomy of cerebral artery, knowledge about stroke, its clinical symptoms and treatment options in this part. The description of spasticity with its influence on therapy and its treatment and the techniques used in physiotherapy are also there. The Brocca aphasia is described in the last section. The second part is the case report of a patient P. V. with diagnosis: ischemic stroke with dextral symptomatology. This special part is based on two weeks' work with the patient in the hospital (Ústřední vojenská nemocnice - Vojenská fakultní nemocnice Praha). This work lasted from 9th January 2012 to 3rd February 2012. Title: Case study of physiotherapy treatment of a patient after ischemic stroke with right-sided hemiparesis Objective: The main aim of this study is to analyze the theoretical knowledge about etiology, clinical symptoms, stages of disease and the following therapeutic methods used during treatment given to patients who suffered a brain stroke. Secondary, prooveing and veryfing my own capabilities in terms of communication with the pacient and his examination, lastly conducting conclusions and creating concepts of therapy treatments which were practically and subsequently applied. The aim was to centre a text that summarizes the theoretical and practical information relating to the treatment of patients with VBE (stroke), in which one of the clinical symptoms of spasticity. The first part - theoretical - focuses on the anatomical and physiological information relating on the motor control, spasticity and vascular brain event. Control of movements is divided into subsections according to the level of the nervous system, from the lowest level - the spinal cord to the highest - cortex. Chapter VBE is divided by the emergence of haemorrhagic VBE and ischemic. The chapter is presented spasticity pathophysiology, clinical manifestations and its treatment. Therapy of spasticity is most devoted to theoretical work. The second part - practical - it provides examples of patients after VBE (stroke), in which one of the complications of spasticity. It is written in the form of case reports.
Medical Aids and Equipment for Children with Cerebral Palsy

Title of the thesis: Medical Aids and Equipment for Children with Cerebral Palsy. The thesis' aims: The main goal of the thesis is to convey a well-structured description of the use, benefits and types of currently available medical aids employed in the treatment of children with cerebral palsy. Another cardinal aim is to complexly abridge the issue of how the condition originates, manifests itself, develops, and explore the alternatives for its treatment. Method used: Besides technical bibliography, in-depth surveys that included first- person interviews and personal encounters with a number of professionals as well as patients have been used. Results and conclusions: Various medical aids play a crucial role in the affected child's life, lessen his dependence, improve his motor skills, raise his life standard and help him in his quotidian activities and communication. Effect of hippotherapy at clients with sclerosis multiplex

Hipotherapy is a complex therapy, broaden and recently very sought after rehabilitation method, which is being more and more recognized by experts for its broaden scene. It is a complex therapy thanks to involving medicine, psychology, pedagogy and also a social sphere. That's why is this method indicated at many different levels of disablement. It's being used with pacients with children's cerebral palsy, diverse level of cerebral dysfunction and multiple sclerosis. And it's the therapy's effect on multiple sclerosis that this thesis paper discusses. It also applies to brain and spinal traumas, scoliosis, back pain, orthopedic defects, cardiovascular disease, degenerative muscle disease, obesity, as well as it provides help at emotional and mental aspect, e.g. children with behavioral disorders and many others. At hipotherapy is being used a three dimensional (multidimensional) horse motion. Horse's spine moves up - down, forward - backward, right - left. Imitating a mechanisms of human walking, which leads to relaxation of spasticity. Impulses are transmitted onto back which brings interaction of impeller and back muscles, stand up pose, and posture interference. Passive rehabilitation affecting on spasticity of multiplex sclerosis

Title: The influence of a passive rehabilitation on spasticity in multiple sclerosis Objectives: The main objective is to summarize the current knowledge about multiple sclerosis and the possibilities of influencing the spasticity as a serious symptom. Another objective is to verify the effectiveness of the antispastic passive rehabilitation of the lower limbs of selected patient in practice. Methods: Verification of the effectiveness of the system of antispastic passive rehabilitation of the lower limbs was through a qualitative research. For the work design is used the type of qualitative experiment. The research sample is a person with spasticity in the lower limbs. In the first two parts of the work was used an analysis of scientific literature and a content analysis. The strategy of data obtaining is also in the form of observation, interviewing, measuring, scale and variety. A medical documentation, anamnesis, casuistry and a secondary analysis were used. Results: Individual use of each method resulted in the influence of spasticity only for a short time. Mobility was affected in the sense of passive movement the most after the soft techniques and massage therapy. The modification of spasticity and functional abilities of patients with infantile cerebral palsy by selective dorsal rhisotomy

Selective dorsal rhizotomy is a neurosurgical procedure for treating carefully selected patients with cerebral palsy. The aims of this thesis are to assess the influence of this treatment on changes in spasticity, functional abilities, locomotion and activities of daily living of these patients and also to assess the changes in these parameters in patients who were treated using this method at the neurosurgical department in University Hospital Motol in previous years. Six patients with CP diagnosis were tested - five 6 to 21 years old boys and one 35 years old woman. Ashworth and modified Ashworth scales were used to assess spasticity, goniometry was used to measure the range of passive movements. The Barthel index and Global care impairment scale by Dressler tested activities of daily living. Penn Spasm Frequency Score and Peacock's scale of locomotion were also used. The results showed a reduction in spasticity in lower as well as upper extremities, an increased range of passive movement of lower extremities, reduction of clonus the tendon of Achilles and an improvement of speech. The influence of botulotoxin application in lower extremities on physiotherapy of children with a spastic form of infantile cerebral palsy

Cerebral Palsy is one of the most frequent neurodevelopmental illnesses occuring in the child age. Main manifestations of the illness include a disorder of muscle tone leading to the development of spasticity. One of contemporary methods of Cerebral Palsy's therapy has been local application of Botulinum Toxin A. This toxin effects a blockade of neuromuscular transmission on neuromuscular junction and in short term limits excessive contractions of spastic muscles. This thesis deals with the influence of physical therapy of children suffering from Cerebral Palsy following the application of Botulotoxin A and compares physical therapic methods applied in the Czech Republic and abroad.
