Degree of difficulty in performing activities of daily living in patients with rheumatoid arthritis

Theoretical foundation: Rheumatoid arthritis is a chronic system disease manifesting itself particularly by inflammatory joint impairment. The main problems confronting daily the patient include pains of the motor system and fatigue. The disease has typically long, progressive development, and although it does not cause directly the patients' death, it reduces their life by some ten years on average. The serious character of the disease consists in the fact that the motor abilities of the individual are gradually impaired. At the beginning the patients must avoid major physical activity, often give up their hobbies and sometimes even their jobs. In the most serious cases, the disease may even prevent the patients from performing basic self-service activities. Goal of the thesis: The goal of the thesis consists in mapping the level of problems of rheumatoid arthritis patients at performing daily activities. Hypotheses - H1: Rheumatoid arthritis patients feel discomfort when performing common daily activities, H2: Rheumatoid arthritis patients make use of supporting and compensation aids when performing common daily activities, H3: Rheumatoid arthritis patients make use of another person's assistance when performing common daily activities. Methodology: The research part of the thesis was implemented based on quantitative inquiry within the grant Project No. 120/2012/S ?Reflection of life quality in nursing?. The actual research made use of HAQ standardized questionnaire, distributed among the rheumatoid arthritis patients. The size of the research set was set at 200 rheumatoid arthritis patients from all over the Czech Republic. The respondents were chosen based on quota selection, striving to observe the proportion of men to women at 3:1, i.e. 150 women and 50 men. When searching the respondents, organizations associating patients with the respective diagnosis were first approached. All results obtained were statistically processed in the SASD (Statistical Analysis of Social Data) program. Descriptive statistics were used. Results: The results can be divided into three areas, by the related hypotheses. The first part of the results gave information on the degree of discomfort felt by the individuals when performing specific common daily activities. It was found out that the patients see the most difficult activity in taking down an item from a place above their head and bathing in the bathtub. The second area brought information on the aids used to perform common daily activities. The respondents named dressing aids or handgrips at the bathtub most frequently. The third and last area brought the answer to the question what activities require another person's help. The patients named reaching or opening of things most frequently. But an essential part of the patients stated not to need any help of another person in common daily activities. Based on the results, the hypotheses were evaluated as follows: H1 - Rheumatoid arthritis patients feel discomfort when performing common daily activities - confirmed, H2 - Rheumatoid patients make use of supporting and compensation aids when performing common daily activities - confirmed and H3 - Rheumatoid arthritis patients make use of another's person when performing common daily activities - not confirmed. Conclusion: The thesis provides a detailed and comprehensive view on the issues of performing common daily activities in rheumatoid arthritis patients. The results can be used both by practical nurses and by students of nursing and related disciplines. The thesis can also constitute foundation for further research. The thesis results will be also presented and published.
Nursing care for patient with polytrauma

Multiple traumas mean a present injury of at least two body systems. The patient with the multiple traumas is, because of the threatening failure of one or more organs, alternatively systems, placed on the intensive care unit. The nursing of the patient is therefore systematic, continual and its part is the monitoring of the conditions and saturation of basic needs. The nursing care on the intensive care unit is dependent on their health conditions and it permanently changes. A nurse usually takes over the whole care of the patient and they are dependent on her. Therefore the nurse must know all the patient´s needs and provide them regard to their self-sufficiency. Self-sufficiency is the level of independence during the daily routine. To its evaluation we use Barthel test of basic daily activities. In literature the word self-care is used as an identical term. The empiric part of the bachelor thesis is focused on self-sufficiency, self-care of a patient and activation of the nurses. The goal of the research is to find out the difference in the amount of care for a patient with multiple traumas on the Intensive care unit and a standard ward in connection with the level of self-care. To fulfil the goals three research questions were set. The research questions are: How do the nurses judge the self-sufficiency of a patient with multiple traumas on the intensive care unit and a standard ward? How do they provide the nursing care at a patient with multiple traumas in a standard ward? How do they provide the nursing care at a patient with multiple traumas on the intensive care unit? The qualitative research with the help of semi-structured interview was used to answer the questions and fulfil the goals. The research was taken on the traumatologic intensive care unit and in the standard ward. 5 respondents and one patient were from the intensive care unit and other 5 respondents and two patients were from the standard ward. The answers for the first research question were identical at all respondents. The evaluation of the patient´s self-sufficiency is according to Barthel test. The answers of most respondents point at the fact that the test in not practical and suitable. The results of the second and third research question can be comprehended in one conclusion. Regarding the fact, that the nurses from the intensive care unit and the standard ward made comments in the same way. The nursing care is always provided regarding the self-sufficiency of a patient. They support a patient mentally to make him more self-independent. They use different accessible tools for practice. With the help of the research questions we reached the goals. We found out that the difference in the extent of provided care is in the amount of activities which are done by nurses for the patient on the intensive care unit and in the standard ward. The nurses provide a patient with more activities logically on the intensive care unit. It is due to the conditions of a patient which are acute at the beginning. Even though, the nurses try to activate a patient as much as possible. By contrast, the nurses in a standard ward have the chance to increase patient´s self-sufficiency in more activities. They endeavour patients to be self-sufficient in hygiene, catering, walking and excretion. In these activities patients should be helped by physiotherapists. As it is agreed by the nurses from the intensive care unit as well as the nurses from the standard ward, this approach does not really work and so the only ones who activate a patient are the nurses and hospital attendants. The work should serve the nurses to clarify the importance of multiple traumas and deepen their knowledge of discussed problems.
The level of self-service skills by the clients of sheltered housing in Domov PETRA Mackov

The bachelor thesis deals with the level of self-service skills by mentally disabled people who exploit the residence social service called sheltered housing. It is devided into two main parts. The theoretical part characterizes mentally disabled people and their self-service skills. It deals also with social work and the system of social services for mentally disabled people and it particularly describes the social service called sheltered housing. The practical part is focused on the research of the given problem in a particular organization focused on providing residence social services for mentally disabled people, in Domov PETRA Mačkov. The aim of this research is to find out the level of self-service skills by the clients of sheltered housing.
Self-sufficiency of NGOs in culture

The master's thesis focuses on self-sufficiency of NGOs in culture, especially on NGOs in performing arts. The thesis aims to analyze the current status of self-sufficiency of NGOs doing performing arts in the Czech Republic. The research was conducted by means of electronic questionnaires sent to NGOs managers or financial employees. The research has shown that NGOs doing performing arts generate a relatively high level of own funds and they diversify their sources successfully. NGOs do not draw much money from the private sector but they use foreign funds a lot. There is still room for improvement in strategic planning of NGOs fundraising and marketing activities.
Financial self-sufficiency of cultural non-profit organisations

The diploma thesis deals with financial self-sufficiency of non-profit and non-governmental organisations operating in the cultural sector. The theoretical part describes all methods of financing. Emphasis is placed on the cooperation with donors and on fundraising through social networks. To make the comparison of financing easier, different legal forms of non-profit organisations with examples are mentioned. The practical part of the thesis presents five different Czech cultural non-profit organizations. The structures of their financial resources are described, financial analyses are performed, and the percentages of self-sufficiency are determined. The analyses show that none of the organizations is self-sufficient. The thesis presents recommendations on how to increase the percentage of self-sufficiency. The thesis explores importance of the crowdfunding in the Czech republic and describes the cooperation with donors in observed organisations.
Expectations and Reality of Living in Nursing home in Planá nad Lužnicí

This bachelor thesis deals with an issue of seniors´ living in a nursing home, which is one of residential social services offered in the Czech Republic. The aim of the thesis is to identify in which aspects expectations of seniors before moving into the Nursing home in Planá nad Lužnicí differed from the reality of the life in this facility. Therefore the research aim is to find out what the expectations of local residents related to and to what extent they were met. The partial aim is to gain an overview about the fact whether the seniors before their arrival in the nursing home are sufficiently informed about the housing and services in this facility. In the theoretical part, first there are defined concepts of ageing and old age and there are described changes accompanying this period of life. Further there are presented social services for seniors provided in the Czech Republic. There is described an adaptation process and in the end of the theoretical part the Nursing home in Planá nad Lužnicí is presented. To get necessary information for the practical part of my thesis I used a qualitative research. I chose a method of interviewing, a technique of a narrative semi-controlled dialogue. From results of my investigation it is apparent that the seniors´ expectations before their arrival in the nursing home were associated not only with the reason of their removal, but also with the fact, what information about their future living was available. Through the research I found out that the vast majority of respondents had enough information about the nursing home. The respondents´ expectations were in most cases associated with a barrier-free environment and a possibility to use chosen nursing care operations. Then the research showed that respondents in the nursing home appreciate a comfortable barrier-free living in the pleasant surroundings. Respondents evaluate positively the nursing care as well as the staff, although some of them point out to financial demands of the housing and services provided. It is important for the respondents that they can continue to be engaged in their favourite activities and meet their friends here. The research shows that in some cases the reality of the housing in the Nursing home in Planá nad Lužnicí even exceeded the expectations associated with the moving into this facility. The findings gained through this thesis could serve as a feedback for the staff of the Nursing home in Planá nad Lužnicí. They also could contribute to increasing of a general awareness of the fact what seniors expect from their living in the nursing home and what the housing in such a facility can offer them.
Nursing care of patients with cataract

This Bachelor´s thesis focuses on caring for cataract patients. Cataract is one of the leading causes of blindness in the world and the surgery for its correction is the most common surgery in the developed countries. 80 % of all that people perceive comes to them through their eyes; that´s why deteorating vision may mean limited self-sufficiency or even a complete loss of thereof; patients may then find it difficult to look after themselves. This Bachelor´s thesis consists of both theoretical and practical components. The theoretical part is concerned with cataract as a disease; it establishes the definition of this illness, its kinds, diagnostics and treatment. A substantial part focuses on nursing care for cataracts, defining concepts such as self-sufficiency and its evaluation and assessing the functional degree of self-care. The practical part consists of a choice method of a quantitative and qualitative research. The quantitative research uses the method of a questionnaire for patients; a partly-structured interview with nurses was used as a means to achieve the aim of the qualitative research. The interviews, which took place in the Ophthalmology department in the hospital in České Budějovice and the Lexum European Eye Clinic are included in this thesis. This work had established two aims. The first one was to find out the specifics of nursing care for cataract patients. This aim was aided by 2 research questions. The first of them was: what are the specifics of nursing care for cataract patients? The second one was: do the nurses know what the specifics of nursing care for cataract patients are? The second main aim was to find out the differences in the patients? care for themselves before and after getting surgery for cataract. This aim was supplemented by Hypothesis 1: after having surgery to correct cataract, patients show higher level of self-sufficiency than before the operation. Hypothesis 1 was confirmed on the basis of the acquired results. In the case of a successful defense of this Bachelor´s thesis, the results will be published in a medical journal.
Influence of biological therapy on the level of self-sufficiency in patients with rheumatoid arthritis

Rheumatoid arthritis is a chronic autoimmune disease manifested mainly by stiffness and pain in joints. The main trouble of the disease lies in deterioration of patients´ quality of life resulting form painful syndromes and impaired function of joints. This progressive disease often leads to disability and invalidity, therefore rheumatic arthritis is an important therapeutic and social problem due to demanding nursing and social care and financial requirements. Significant changes in rheumatic arthritis treatment have occurred recently. Biological treatment has become a new therapeutic method. Data suggesting that this kind of therapy may slow or even halt the x-ray progression of the disease have been obtained.
The role of a nurse during a postoperative period supporting patient's independence after an amputation lower limb.

Having studied medical literature and having gained practical experience in taking care of patients with amputation in the Surgery Ward of the Hospital České Budějovice a.s., I decided to focus my bachelor thesis on: The role of nurses in promoting self-sufficiency in patients with amputation during postoperative period. Lower-extremity amputation can be defined as the removal of a peripheral body part resulting from an injury or surgical operation. Currently, road transport in particular accounts for a high accident frequency. Many young people experience the loss of a body part due to injuries. As a planned surgical procedure, amputation of the lower extremity is performed in order to relieve or eliminate pain or in order to prevent infection or metastases from spreading. Lower-extremity amputation changes the life quality of most people. Health professionals provide the care for amputations according to the standards and lead the patients towards gaining self-sufficiency. The qualitative research was aimed at: 1. mapping the role of nurses in promoting self-sufficiency in the patients with amputation during postoperative period, as viewed by both the nurses and the patients, and 2. determining the degree of successful practising self-sufficiency in the patients with amputation during postoperative period. The relevant research questions were set as follows: 1. What is the role of nurses in promoting self-sufficiency in the patients with lower-extremity amputation, as viewed by nurses? 2. What is the influence of nursing on the self-sufficiency of the patients with lower-extremity amputation? 3. How do nurses practise self-sufficiency in the patients with lower-extremity amputation during postoperative period? The qualitative research was based on the use of in-depth interviews with the nurses and patients. The research sample included patients with lower-extremity amputation and nurses of the Surgery and Traumatology Wards in the Hospital České Budějovice a.s.. As regards the promotion of self-sufficiency in the patients with lower-extremity amputation, the results indicate that the nurses from the Surgery and Traumatology Wards performed mainly activities aimed at satisfying patients´ physiological needs (hygiene, food and fluid intake, excretion, assistance in moving and in the need to be pain-free). Nurses of the Surgery Ward consider practising self-sufficiency in these patients more demanding in view of the patient´s age and the type of amputation related to a chronic disease. Patients believe that nursing care during the hospitalization has a significant influence on their self-sufficiency. The nurses of both the Surgery and Traumatology Wards consider practising self-sufficiency to be of great importance and can observe different effects as related to the patient´s age and the cause of amputation. Both the nurses and the patients agree that practising self-sufficiency includes hygiene, assistance with food intake and excretion, getting up from bed to assume vertical position, practising movability and managing pain. The investigation objectives were fulfilled and the obtained results may help nurses improve the quality of nursing, which will contribute to the improvement of the quality of life in the patients with amputation.
Promoting of self-sufficiency by hemiplegic patients after a brain stroke

Nowadays, cerebral apoplexy is no longer a disease that only attacks elderly people, but increasingly, persons at a productive age. The consequences of this didease are diverse and vary according to the affected person, both in extent and severity. This is why this disease fundamentally changes the way of life of both the diseased and their families.The teoretical part of my bachelor thesis is focussed on the disease itself. The reader of this thesis will learn more about the nature and the substance of this disease. This thesis describes the types of cerebral apoplexy, the risk factors, the diagnosis and the methods of treatment. If further describes the princilpes of methods for nursing people suffering cerebral apoplexy,emphasising their self-suffuciency, rehabilitation a rehabilitation targets.The socond part of my bachelor thesis was research work. The applied method was based on a survey in which two types of questionnaries were deployed, one for respondents working in inpatient wards in medical facilities and one for respomdents working as home carers. This part focussed on acquiring information and discovering differences in providing support to hemiplegic patients after cerebral apoplexy to help them to becouse self-suffucient in personal hygiene and to collaborate wiht a physiotherapist, both in an inpatient ward and during home care. The result should contribute to better nursing care provided to patients after cerebral apoplexy.
