The Application of the Virginia Henderson{\crq}s Model in Retirement Home Nursing Care.

The Application of the Virginia Henderson{\crq}s Model in Retirement Home Nursing Care. Basic unit of the V. Henderson{\crq}s model is the clients{\crq} independence when satisfying their needs. The client is seen as an individual with the needs of assistance when trying to reach health, independence, and peaceful death. The lack of strength, will and knowledge are seen as the source of troubles. The needs the client is not able to satisfy and is in need of assistance are by V. Henderson called as the focus point intervention. The nurse assists to substitute, complete, support or increase the patient{\crq}s strength, will and knowledge. The outcome is the unaided performance in satisfying personal needs or peaceful death. The information about the needs is not documented in the uniform way and that is why every single ward and institution creates its own documentation system, according to its own specifics. The nursing documentation is always inseparable part of the nursing process. This model is determined by its specifics to be used appropriately in elderly people. The goal of this work was to find out all possibilities in using the Virginia Henderson{\crq}s model when caring of clients in the retirement home. There were placed three main questions of the research: 1.To what extents are satisfied the needs of client in the house for elderly people when using nursing documentation compiled according to V. Henderson? 2. Is there any influence of using V. Henderson{\crq}s model of nursing documentation on nursing practice in the retirement home? To what extent are nurses comfortable with the V. Henderson{\crq}s nursing documentation system? The collecting of data was done by the qualitative research investigation, the technique of the direct observation of nurses{\crq} caretaking of the clients, and by detailed interview with the nurses. The research had been done at Retirement Home at Strakonice from April to June 2009. There was created nursing documentation according to V. Henderson{\crq}s system. This documentation was filled out by a nurse and client and followed by searching for the needs of the client and their satisfaction. The nursing planned out specific care of a particular client using nursing process after filling out particular documentation. There has been led direct observation of caretaking provided under new nursing documentation. To assess the care, there was developed the point scale for evaluation of the client needs satisfaction (1-Satisfied, 2-Rather Satisfied, 3-Rather Unsatisfied,4-Unsatisfied). As a next we have been evaluating client{\crq}s level of strength, will and knowledge to be able to satisfy his/her needs (he/she is independent, partly independent, and dependent). The observation outcomes demonstrated that nurses were able to satisfy all of the 14 clients{\crq} needs in most. There were done case reports out of the observations and the results were processed into comprehensible charts. The nurses had been working with the nursery documentation for months followed by detailed interview aimed to inspect the usage of this documentation in nursing of elderly people. All of the five nurses agreed on the applicability of the documentation in nursing practice and that was why the documentation was modified under nurses{\crq} notes and has been applied in practice since. The research goal was accomplished. The nursing documentation has always been integral part of nurse{\crq}s work.It provides all needed information concerning the patient. It can be considered as the prove of ``lege artis{\crq}q practice of the work done. The documentation also serves as the record which is being used for health insurance billing.
Communication problems between nurses and hospitalized handicapped patients

Social contact is a necessary part of health care profession, and the ability to communicate well belongs to the most important skills of every medical staff. Successful treatment and overall cooperation is a result of mutual relationship and communication between the involved parties. However, professional communication of a nurse is a very demanding subject. There are numerous barriers and issues that prevent efficient communication. This bachelor{\crq}s thesis is aimed at the topic of communication of nurses and patients disadvantaged due to some handicap. 3 objectives were established for the paper. The first was to find out whether nurses have communication skills in relation to the handicapped patients. Further we were interested in whether the nurses were ready for communication with the handicapped patients through educational program in school, and the third objective was to find the most frequent problems in the identification of psycho-social needs of those patients. Hypotheses were formulated for the objectives H1: Nurses have communication skills in relation to the handicapped patients, H2: Nurses are sufficiently prepared for the communication with the handicapped patients through educational program in school, H3: Nurses emphasize time constraints as the problem of identification of psycho-social needs of the handicapped patients, and H4: The handicapped patients mention lack of privacy as the problem when talking with the nurse. The hypotheses H1 and H3 were confirmed, while H2 and H4 were not. Quantitative research was chosen for the preparation of research part of this bachelor{\crq}s thesis, and query method was used. As interpersonal contact and relation play major role in the communication, it can{\crq}t be evaluated only in one direction and two groups of respondents represented the sample file. The first group consisted of nurses in ward blocks of five hospitals, and the second of hospitalized handicapped patients. This thesis could serve as an educational material for medical staff. It is necessary to continuously learn and deal with this issue with the goal to improve the quality of the provided medical care for the hospitalized handicapped patients so that it corresponds to their actual needs. The results of the research could be an inspiration in the field of education of medical staff.
Special aspects of nursing care of clients in the Nursing home for disabled people in Zběšičky

In the Nursing home for disabled people in Zběšičky nursing care for persons with combined disabilities who need specific nursing care is provided. Nursing activities are focused on meeting the clients´ needs. The mission of the Nursing home is to create another home for the disabled and make the disabled clients´ lives similar to those of healthy people. The main effort of the nursing staff in the Nursing home is to make clients self-sufficient in daily routine - with respect to their individual abilities. The aim of the research is to determine the level of nursing care in the Nursing home and to analyse the care provided to the clients. Using the results of the research we can point out the possible inadequacies in nursing care so that the quality of nursing could be improved. The research was conducted by the quantitative method. The techniques used in the research were observation, interview and analysis of documents. During the observation the main attention was focused on meeting the clients´ needs. Relevant medical records were analysed. The research database 1 was composed of four Nursing home clients with combined disabilities, who were chosen randomly. Semi-standardized interviews were conducted with all the nurses of the Nursing home (seven nurses) who formed the research database 2. The results of the research and the medical records analysis show that clients´ needs are met to the extent limited by the clients´ condition. If the needs are met only partially it is caused by the client´s serious disability the nurses try to compensate. During the observation special aspects of nursing care were determined: these are especially meeting the clients´ needs concerning activity and rest. Nurses take part in physiotherapeutic and activating exercises. Another special aspect of nursing is basal stimulation which significantly improves meeting needs in communication, perception and cognition. In meeting the needs concerning hygiene, excretion and nutrition the care is similar to that in immobile clients in other health care facilities. Nursing staff members were interviewed to find the level of adherence to the health and social standards set for nursing homes for disabled clients. The care provided in the Nursing home is community care and the standards correspond to the principles of thit type of care. These principles are: complexity, respecting human rights, health recovery in society, continuous education and standardization of the process. On the basis of the research a following hypothesis was stated: the nursing care provided to clients with combined disabilities corresponds to health and social standards applied in the Nursing home for disabled people in Zběšičky. Some inadequacies were found in abiding by the principle of complexity. There is a lack of medical diagnostics in the nursing process. The objectives of the thesis were accomplished. The results obtained by the research will be provided to the Nursing home for disabled people in Zběšičky and can be used to improve the nursing care. It is advisable to organize a seminar on the nursing process for the nursing staff in the Nursing home.
The offer of health and social services for seniors in relation to their needs

Abstract The society has become increasingly interested in the issue of seniors at present. Considering the increasing age of seniors, a situation when they are not able to take care of themselves and need help of another person may occur. The assistance secured by health and social services may be provided to seniors in their homes or in housing facilities. Human needs are changing and developing with age and form an integral part of the personality of an individual. The theoretical part of the thesis addressed the terms of aging, old age, needs and health and social services. The data were collected using the qualitative research strategy, the method of questioning and the technique of interviews, involved observation and technical literature analysing. While interviewing the respondents, their anonymity was respected. The objective of the thesis was to ascertain whether needs of seniors are satisfied in relation with the offered health and social services. In the research, the main research question was set out. In order to answer the question, five research sub-questions associated with the interview structure were used. The research was conducted in retirement home Hvízdal České Budějovice, p.o. (organization co-funded by the state budget). The main research question led to confirmation of the thesis objective. The obtained information may be used for improvement of quality of the services provided in the facility where the research was conducted.
The use of the nursing model by V. Henderson in care of a child client

Contemporary nursing care of children is based on the holistic approach aimed at the satisfaction of needs. Nurses in the majority of health facilities use documentation developed by M. Gordon. The nursing model by Virginia Henderson, however, is actually much better suited to the identification and subsequent satisfaction of children's needs. This model belongs to the category of models of needs and of humanistic models. A child's needs are divided into 14 basic domains in this model. The present bachelor's thesis describes a study of the use of nursing documentation based on Henderson's nursing model at the Department of Babies, University Hospital in Plzeň. In Part 1, the quasi-experimental method {--} observation method was applied to data processing. The research group included in-patient children at the Department of Babies. Part 2 of the research was quantitative, based on answers to questions asked by means of a questionnaire. Here the research group included nurses working at the Department of Babies, University Hospital in Plzeň.
The satisfaction of child´s needs in the establishment: nursery

The bachelor thesis is aimed at the determination of the level of satisfying biological and psychical needs of children in facilities of the nursery-like type. Nurseries belong to special health-care facilities ensuring the all-round development of children in accordance with the provisions of Act 20/1996 of the Law Code, section 38. In the 1990s these facilities were mostly liquidated, now the attendance is about 1% of the children up to 1 year of age. However, nowadays the concept of nursery-like care seems to gain in importance. The objective of the thesis was to find if the nursery-like health care facilities can satisfy the child´s biological and psychical needs. The thesis elaboration was based on qualitative research, on the analysis of documents, methods of observation and interviews with the nurses in the facility as well as the parents of the observed children. The survey took place in Emil Pitter Nursery in České Budějovice. The facility belongs to the allowance organization network Nurseries and Shelters, České Budějovice. The sample consisted of eight children who regularly attended the facility. The case reports involved in the thesis comprise the information obtained at the stage of survey.
Needs of a patient after vitrectomy in ophthalmologist unit

The principle of a quality nursing care is a detection and satisfaction of biological, mental, social and spiritual needs which are altered by the illness. The mission of nurses is to help individuals, families and groups meet their current needs and achieve their maximum physical, mental and social satisfaction in accordance with the environment they live and work in. The objective of the thesis was to find out what kind of needs the patients after vitrectomy in ophtalmologist unit have and how are these needs satisfied. Another objective was to find out in which ways are the patients educated by the nurses. Before the research itself the questions were selected. The first question was to identify needs of patients after surgery in the opthalmologist unit in hospital in Kyjov and the second question was enquiry about meeting these needs. Third question raised was about education of patients after vitrectomy. The fourth question was to find out if the nurse can, by suitable education, prevent the patient from fear of returning home. The last fifth question was to find out if the nurses recommend cooperation with multidisciplinary team. The thesis consists of a theoretical and a practical part. The objective of the theoretical part was to give sufficient information on the patient´s needs. I also summarize problems which the patients may encounter when they are discharged from hospital and during subsequent socialization. In the practical part I used qualitative research using the method of case studies and in depth interviews with the use of dictaphone. The research sample consisted of patients in the ophtalmologic unit in hospital in Kyjov. Altogether, 10 patients were interviewed. The second part of research sample consisted of 5 nurses from the opthalmologist unit who were being observed while educating patients. In the discussion individual findings were discussed and reseach questions were evalutated. With regard to qualitative research the resulting hypotheses are stated in the last part of the thesis. The results show that the most frequent needs indicated by the patients were: absence of pain, security and safety, sleep and rest, food and drink and defecation. Furthermore, the needs of patients are being satisfied by suitable interventions. The nurse does help patients by suitable education to reduce the fear of returning home and the nurses do recommend cooperation with multidisciplinary team. The objective of the thesis was accomplished. The findings of the thesis can be used for training of nurses and medical staff who may encounter people with visual impairment also in other hospital units. They can contribute to improved care for these patients.
Application of the basal stimulation in nurcisg care

Basal stimulation is a pedagogically-nursing concept, based on the knowledge of pedagogy, development psychology, anatomy, physiology and nursing. The basic elements of the basal stimulation concept are movement, communication and perceiving. The concept enables to clients with changes in the field of perceiving, movement and communication the integration into the present life with the maximum usage of their former life habits and experience, it respects the preserved abilities and moreover accepts their future life needs. The correct integration of basal stimulation elements into the nursing care enables to intermediate to the man the perceptions from his body and the stimulation of perceiving organism enables him to perceive better the surrounding world and to establish the communication with it. Hereby the total development of the individual is supported.
The support of tourism development in Vodňansko

All of objetcives of this bachelor were accomplished with the use of given methods Primary and secondary offers of tourism in the region of Vodňany were analysed. The further analyses comprised of the needs and requests of visitors. The work also evaluated the contributions and loses of tourism to the destination. The products that are developing most are biking and walking; angling is also very popular. During the analysis of the current condition of tourism in the region of Vodňany, it became clear that the biggest advantages of the area are high quality environment and rich history. The questionnaire survey shows that favourite sights of the region are the ruin of the castle Helfenburg, chateau Kratochvíle, pilgrimage site Lomec, and the chateau of Protivín. The survey also indicates that it is necessary to increase the number and improve the quality of sports and culture facilities, make the boarding and accommodating facilities better.
Holistic approach in providing nursing care for disabled children in the health and social facilities.

Holistic nursing care requires body and psyche orientation. That means it stresses both, body and psychosocial human needs. It is necessary for nurses to understand the principle of psychosocial interaction and care of the whole person including his/her dearest in health and illness. The aim of the thesis ?Holistic approach in providing nursing care to handicapped children in health & social facilities? was to find out how the needs of handicapped children are satisfied in health & social facilities from the point of view of holistic approach. The work involved both, qualitative and quantitative research. 104 nurses filled in a questionnaire and 10 respondents were observed within the quantitative research. Two hypotheses were set. The first hypothesis was focused on handicapped children in health & social facilities and on satisfaction of their needs from the point of view of holistic theory. Most of the nurses are convinced that they are providing holistic care. However on the other hand a lot of nurses answered that they preferred provision of physiological needs to psychosocial needs. This finding does not fully confirm the holistic approach in providing nursing care. The second hypothesis says that nurses provide more holistic care in non-profit making facilities than in those operated by regional authorities. Non-profit making facilities could not be included in the research as nurses are not employed there and those who work there work at the positions of social workers and caregivers, not at the positions of nurses. Comparison was only performed at facilities operated by private limited companies and the regional authority. The research also compared the approach to holistic nursing care between head nurses and ward sisters and nurses working on shifts. The nurse managers answered more positively than the nurses working on shifts. A nurse providing holistic care sees a human as a unique complex being, not as its single parts. Such a nurse is ready to help people achieve and maintain optimal health, helps in prevention and reduction of diseases and provides support to those incurably ill. It is important to realize that body and psyche are closely linked, not separated. This thesis may be used by nurses working with handicapped children in health & social facilities. Elaboration of a standard for holistic nursing care for handicapped children in health & social facilities is the practical output of the thesis.
