Birth oriented to woman

Nowadays, respecting individual wishes of the woman in labor and her partner, the effort that pregnant women bring the child to the world in a quiet and friendly environment is accentuated. The target is that women see the birth as an important event in the life and not only as unpleasant and painful moments. From the professional point of view the alternative obstetrics consist in the effort to fulfill the wishes of women in labor without endangering the safety of the woman or of the child. In the theoretical part, the possibilities of the alternative ways of birth are summarized with focus on the birth outside the hospital facility, ambulatory births, influence of the environment of the birth room on women during the birth, accompanying persons present at birth and first of all the non-pharmacological way of softening the labor pains. From the pharmacological methods, only the epidural analgesia and spinal analgesia are mentioned in the thesis. The practical part focuses on perceiving the environment by women in labor, her satisfaction with the staff and wishes of women in labor. To treat the given issue and to achieve the postulated goal, the qualitative research examination was chosen. The investigation was performed in the form of the non-standardized talk with women after the birth which have given birth in any hospital in the Vysočina region. The target of the thesis was to find out the needs of women leading to the higher quality of the nursing care during the birth. The target was achieved. Based on the elected target, 4 research questions were formulated. The first research question was: Whose presence as member of the staff is preferred during the birth and what is expected from him/her? The research question number two: How is the environment perceived by women during the delivery? The research question number three: Would the women elect another kind of birth leading based on preceding experience? The last research question number four was: What is seen by women giving birth for the first time as negative factor during the birth? All the research questions were answered. This bachelor treatise may be a source of information for following future midwives or pregnant women preparing for the birth. It may be utilized also by midwives who can acquire in this way the survey of what is expected by women in labor from them, how these women perceive the environment and what alternatives they appreciate.
Educations concerning problems of gravidity and labour at women with aural handicap in the regions South Bohemia and Passau.

A midwife working in the delivery room can get into a situation where communicating with an aurally impaired pregnant woman and effectively educating her becomes essential. Pregnancy and birth represent considerable stress, whether physical or mental, for the organism of every woman. Owing to more complicated communication, aurally handicapped women in particular are subject to increased fear and distress. The theoretical part of the thesis focuses on the question of physiological pregnancy and birth as well as the specification of aural disability and educational process. The differences in care for pregnant and birthing women in the Czech Republic and Germany are mentioned at the end of the theoretical part. Studying aural disability more consistently as well as possible contradictions and consequences of insufficient communication during pregnancy, birth and subsequent care for the newborn between the staff and the aurally impaired woman enables a more professional view, thus contributing to a better level of prevention of the woman?s physical and mental problems. The empirical part of the thesis aims at finding out the skills and knowledge of the ways of communication and education of midwives within their work responsibilities as far as aurally handicapped women are concerned. The research also consists of deeper understanding of the area where the necessary educational process should be aimed from the point of view of aurally impaired women?s needs. Quantitative and qualitative research methods were used to get and process data for the empiric part. The quantitative research survey was carried out through the method of data collection by a questionnaire for midwives working in the delivery room. The qualitative part of the research survey was carried out via framework analysis of the output gained from half-structured interviews conducted with aurally handicapped women. Identifying the communication skills of midwives with aurally impaired women in the obstetrics can result in the optimization of the mutual cooperation between a client and the nursing staff. Upon the learnt facts, the thesis points put potential problems in the area of education of aurally handicapped women. At the same time, suitable communication and education strategies are recommended within the professional powers of a midwife in the area of informing aurally handicapped women about pregnancy and birth.

Effect of antenatal preparation for childbirth first female nascent and cooperation with the midwife. Birth has always been perceived as a difficult life situation, which requires necessary preparation, even if it was mere passing on the wisdom from one generation of women to another. An expectant mother has a possibility of attending antenatal courses alongside with her partner. There, the parents-to-be gather information on the course of pregnancy, good nutrition, and possible complications during pregnancy. They become familiar with a might-be management of the birth, the issues relating to postpartum period, and the basics of how to take care of newborns. Since 2002, the author has organized antenatal courses for expectant women and their partners in Strakonice. This drew author?s attention to the question whether antenatal preparation of mostly primiparas has an impact on the management of the birth and postpartum period, and the question of collaboration between a prepared woman and a midwife. The first objective of the thesis was to confirm whether antenatal education of mothers-primiparas has an impact on the management of the birth and postpartum period. The second objective of the thesis was to observe the impact of the antenatal preparation on the collaboration between a woman-primipara and a midwife during the delivery and postpartum period. The bachelor?s thesis is based on a qualitative research that introduced 2 objectives and 4 research questions. Data were collected by interview. For the purposes of the research, interviews were held with primiparas who had completed antenatal preparation, primiparas who had delivered their babies at the maternity hospital in Strakonice, and midwives currently working at the Department of Obstetrics and Gynecology of the Hospital Strakonice PLC. The results of the qualitative research were processed in the form of case reports and assessment tables. The established objectives were met. The research questions provided necessary data to accomplish the research. Based on the responses to the research questions, 4 hypotheses were established. The outcomes of the research point out the quality of care provided by midwives during the antenatal preparation. In practice, they might be utilized in negotiations with insurance companies that are welcome to contribute to women so as they do not have to pay the full price of the courses in question. The outcomes also raise questions whether they help prevent complications during the birth and postpartum period or not.
Satisfaction of Women in Labour with Nursing care Provided in the district Hospital Jindřichův Hradec.

This thesis is focused on satisfaction of women in labour with nursing. The theoretical part of the thesis is divided into eleven chapters and is focused on theoretical grounds of obstetrics and nursing, midwifery and satisfaction of the clients. The research part is focused on the satisfaction of the women in labour with nursing provided in delivery rooms at the district hospital in Jindřichův Hradec. The elaboration of the thesis at hand was based on three objectives. Objective No. 1: was to find out whether the women in labour are satisfied with nursing provided in the delivery rooms. Objective No. 2: was to observe whether the women in labour are satisfied with the equipment in the delivery rooms. Objective No. 3: was to identify what element the women in labour consider most important in the process of giving birth. A quantitative method was used in the research. Data were collected by the technique of anonymous questionnare. The research corpus comprised of 82 women after spontaneous delivery. Three working hypothese were tested. H 1: women in labour are satisfied with nursing provided in the delivery rooms. H 2: women in labour are satisfied with the equipment in the delivery rooms. H 3: empathetic attitude of midwives is found to be the most important element in the process of giving birth by women in labour. Based on the results of the research, it is possible to state that all three hypotheses have been confirmed and all three objectives have been met. The satisfaction of the patients is one of the most important indicators of the quality of health services. The outcomes of patients satisfaction surveys are irreplaceable source of stimuli for improving the quality of nursing. The information contained in this thesis may be interesting and motivating to all healthcare professionals, doctors and/or other medical staff, that participate in providing health care and nursing to women giving birth. The outcome of this research is an information brochure for expectant mothers distributed by the department of obsterics of the hospital in Jindřichův Hradec.
Women´s interest in care provided by a community midwife during pregnancy, childbirth and the confinement period

The period of pregnancy, childbirth and confinement is a big milestone in a woman's life. Since ancient times up until recently it had been a midwife who took care of a woman in childbed in that period. The midwife was not just a provider of professional care, but also a mentor. Till the nineties of the last century this kind of care was provided also in our country, but, unfortunately, in connection with changes in the health care system the visiting midwife service has almost disappeared. If we want to improve the care for women during this period, it is necessary to offer this service to women again. The aim of this thesis was to determine whether women take interest in physiological pregnancy care by a community care midwife, whether they would make use of her services during childbirth in hospital and whether women miss services of a community care midwife during the confinement period. Hypotheses assumed that pregnant women prefer a doctor's care, are interested in services provided by a community care midwife during childbirth and they miss care in the confinement period. In the thesis the quantitative research method using the questionnaire data collection technique was applied. The questionnaire was anonymous and was composed of 25 questions. The survey was carried out from the end of February till the beginning of April 2011 in consultant gynecologists´ surgeries in Vimperk. The total number of distributed questionnaires was 120, 112 questionnaires were returned, 7 questionnaires were excluded. Therefore, the total number of respondents was 105 (100%). The stated objectives of the thesis were met. The hypothesis assuming that pregnant women prefer a doctor's care was not confirmed. The other two hypotheses were confirmed. At present, there is no community care midwife providing the visiting service in Vimperk and its surroundings. Therefore, the results of this investigation could be a good argument for community care midwives who want to make a contract with health care insurance companies. The outcomes may also influence decisions of midwives who consider the visiting services provision, but are afraid of disinterest on the women´s part.
Nursing care of women in childbirth completed VEX (vakuum extraction)

My thesis is focused on nursing care for a woman during the childbirth ended with VEX (vacuum extractor). The first part of my thesis is the theoretical part where the method of vacuum extraction in obstetrics, its indications and conditions and the procedure technique is introduced, as well as nursing care of a midwife for a woman in the childbirth ended with VEX in the delivery room and the confinement department. The aim of the thesis was to determine if midwives have experience with care for a woman in the childbirth ended with VEX. Whether they know the conditions and indications for using VEX during childbirth on the part of a mother and fetus and in what maternity hospitals in the South Bohemian region the VEX method is used. To meet the objectives, hypotheses were stated. The first hypothesis: Midwives have experience and know how to take care of a woman during the childbirth ended with a VEX. The second hypothesis: Midwives know the conditions for ending the childbirth with a VEX. Third hypothesis: Midwives known indication on the part of a mother and fetus for childbirth ended with a VEX. Research Question 1 In what hospitals in the South Bohemian region is the VEX method for childbirth ending used? The quantitative research method was used to find the answers. The data were collected by the interview method, the questionnaire technique. The questionnaire was anonymous, it was composed of pre-formulated questions, which served as identification data. The questionnaire contained 23 questions. The research respondents for the quantitative research were midwives working in the delivery room and confinement department in selected hospitals of the South Bohemian region. The questionnaires were distributed to seven selected hospitals in the South Bohemian region (České Budějovice, Český Krumlov, Jindřichův Hradec, Strakonice, Písek, Tábor, Prachatice). The total number of distributed questionnaires was 100, 20 of them were not returned, namely from the hospital in České Budějovice. Therefore, the total number of responses was 80 (100%). The results are processed into graphs. The goals of the thesis were met. The research survey has shown that midwives are experienced and able to take care of a woman in the childbirth ended with a VEX, the first hypothesis was confirmed. The second hypothesis was also confirmed because midwives know conditions for the childbirth ended with a VEX. The hypothesis on indications on the part of a mother and a fetus was also confirmed. The second and supplementary technique was an inquiry intended to determine in what hospital in the South Bohemian region the method of childbirth ending with a VEX is used. The inquiry was addressed to head nurses of gynecological and obstetrical departments in hospitals in the South Bohemian region (České Budějovice, Český Krumlov. Jindřichův Hradec, Strakonice, Písek, Tábor, Prachatice). The total number of distributed inquiry questionnaires was 7, 1 of which was not returned, it was from the hospital in České Budějovice. The inquiry contained seven questions. It was found out that the vacuum extraction method is used in hospitals in Strakonice, Český Krumlov and Prachatice. In other maternity hospitals this method is considered to be used in the future. In the final part of the thesis a standard for midwives concerning the care for women in the childbirth ended with a VEX was created supposing that the standard will be used in nursing care.
The Importance of Physiotherapy in the Puerperal Period of the Woman

The theme of this thesis is The importance of physiotherapy in the puerperal period in women. As far as pregnancy is concerned, not only is available excellent health care important for women, but there are a large number of organizations that provide advisory services, exercises for pregnant women, and antenatal education for mothers-to-be. Unfortunately, this care usually ends soon after childbirth and women in puerperium, who go through many physical and mental changes, are not given such attention. It is precisely in this period that women deserve some interest in their health status. In the field of physiotherapy, they should be given sufficient information about the changes undergoing in their organisms, about the effects and principles of exercises after childbirth, about the suitability of positioning and about other issues with which women who have just given birth meet. The objective of the thesis was to study the physical changes in women after childbirth, to develop a plan for rehabilitation exercises, and to explore whether individual therapy has a positive impact on the status of the mother. The theoretical part deals with the composition of the birth canal, physiological changes during pregnancy, childbirth problems, childbirth injuries, involutionary changes in the organism during puerperium, and appropriate physiotherapeutic methods. The experimental part is devoted to qualitative research where the following methods were used: history, status praesens, direct monitoring, exit examinations, case studies and a secondary data analysis. The research was conducted in two patients after physiological childbirth and in one patient after Caesarean section. These patients were selected from a basic sample of patients at a gynaecology and obstetrics department. The results of the thesis identified some positive effects of after-childbirth physiotherapy on the carriage of body, breathing stereotype, deep stabilization system and the adjustment of the most prominent muscle imbalances. The objectives of the thesis were met. The new findings, which this work has brought, undoubtedly include pointing out the importance of physiotherapy during this period, impossibility to standardize such care due to personhood of each woman, and the importance attention given to the woman?s body as a whole after childbirth because to care for only its most visibly affected parts is not enough.
The Pain and Delivery

The topic of my bachelor thesis is pain and confinement. The reason why I selected the topic is that it holds my interest. The theoretical part contains general information explaining what pain is and stating what kinds of pain there are. I aimed to describe in detail in particular parturition that is closely associated with confinement. It is a natural state accompanying almost every confinement. I focused on the mechanism of pain origin and the methods of parturition alleviating. Nowadays parturition can be prevented by various methods. The methods are divided into pharmacological and non-pharmacological. In my bachelor thesis I set three objectives and hypotheses associated with them.The research set were women in the puerperal ward of Nemocnice a. s. České Budějovice. The method of quantitative research was used and the data were collected by means of an anonymous questionnaire.100 questionnaires were distributed, of which 90 were returned.The research part has revealed that hypotheses 1 and 2 have been confirmed and hypothesis 3 potvrena not. The bachelor thesis could be used as an information material for the women in childbed who are not aware of the opportunity to utilize pain relieving methods.Furthermore, as information material for students of medical science.
Obstetric situations in prehospital emergency care and duties of a medical rescuer

The paper treats the subject of obstetric situations in pre-hospital emergency care and the tasks tackled by paramedics. Vital in such situations are the early recognition of symptoms; correct diagnostics; and a speedy transfer of the patient to a hospital. The paper concentrates primarily on delivery during the pre-hospital emergency care; on premature leak of the amniotic fluid; and on bleeding encountered in pregnant women in the second half of gestation and the related complications. The effort aimed particularly at mapping the frequency of emergency obstetric situations met during the pre-hospital emergency care in the Region of South Bohemia; at exploring the experience of paramedics; and at finding whether they were afraid of such situations and whether they were able to apply their theoretical background in practice. The defined objectives yielded four hypotheses: H 1: Emergency obstetric situations emerging during the transit are rare in the South Bohemian Region; H 2: Paramedics possess sufficient theoretical erudition to manage the emergency obstetric situations; H 3: Paramedics do not possess sufficient theoretical erudition to manage the emergency obstetric situations; and H 4: In providing emergency pre-hospital care paramedics try mainly to take the patient to hospital in good time. The research section of the paper relied on a quantitative survey conducted through a questionnaire distributed among paramedics in the Region of South Bohemia. Altogether 80 questionnaires were distributed, out of which 66 constituted the surveyed set. The results were processed and assembled into cake diagrams best depicting the percentage shares. All objectives have been accomplished. The first and fourth hypotheses proved false, while the second and third proved true.
Delivery plann, its formulation and testing at The Gynaecological and Obstetrical Clinic in Plzen (GPK FN Plzen).

The subject of this bachelor thesis is delivery plan, its formulation and testing at the Gynaecological and Obstetrical Clinic in Plzen (GPK FN Plzen). Most women presently show active interest in pregnancy and delivery. They often imagine in advance the progress of childbirth, the behaviour of the maternity hospital staff towards them and the options offered by the hospital to delivering mothers. The delivery plan will provide to this type of women the progress of delivery most convenient for them. They may note their requests and desires in it concerning the progress of childbirth, its individual parts and potential alternatives. Thus the pregnant woman gets control over its coming delivery and may think the individual delivery parts over and prepare for them. The GPK FN clinic in Plzen does not dispose with its own delivery plan to offer to mothers-to-be within its offer of services to the pregnant. That is why this bachelor thesis will represent an asset both for the Plzen clinic of gynaecology and obstetrics GPK FN and for the mothers who decide to give birth to their child here. The theoretical part of this thesis includes chapters explaining the notion of the delivery plan and its individual parts. Further chapters briefly describe the progress of childbirth and the options of care of the delivering mothers in the course of the individual sages of delivery. The purpose of the present thesis was to find out which areas of the delivery plan are considered relevant by the pregnant women who have decided to give birth to their baby at the GPK FN clinic in Plzen and which parts of the plan are considered most important by the healthcare staff working at the GPK FN clinic in Plzen. Another objective of the thesis was to formulate the optimum delivery plan meeting the demands of both the delivering mothers and the staff of the GPK FN in Plzen and to find out the response of the healthcare staff of the GPK FN Plzen and the pregnant women to the use of the formulated delivery plan in practice. The theme was processed on the basis of a combination of qualitative and quantitative research. The needed information was obtained by means of post-structured interviews with open questions and a questionnaire with open questions, yes-no questions and semi-open questions. The subjects of the qualitative research included male and female obstetricians working at the delivery ward of the GPK FN Plzen, midwives assisting deliveries at the GPK FN Plzen, pregnant women hospitalised at the Risk Pregnancy Department of the GPK FN Plzen and mothers after delivery (in their puerperium) hospitalised at the Puerperium Department of the GPK FN Plzen. The research further included private gynaecologists of the Plzen {--} city district and gynaecologists of the IVF centre, and midwives working in the advisory centres of the private gynaecological surgeries in Plzen {--} city and in the IVF centre in Plzen. On the basis of the first part of the research the following research questions were formulated: ``What areas of the delivery plan are considered relevant by the pregnant women who have decided to give birth to their baby at the GPK FN clinic in Plzen{\crqq}, ``What areas of the delivery plan are considered relevant by the staff of the GPK FN in Plzen``, ``What should the optimum delivery plan look like to be usable for deliveries at the GPK FN in Plzen``. The second part of the research included evaluation of the following hypotheses: ``Pregnant women consider important for the delivery plan to include criteria assuring progress of their childbirth according to their desires``, ``The healthcare staff of the GPK FN Plzen takes a positive approach to the formulated delivery plan``, ``Pregnant women are interested in application of the delivery plan to their childbirth at the GPK FN in Plzen``.
