Reproductive and Psycho-sexual Aspects of Female Congenital Genital Defects and their Genetic Context.
Chmel, Roman ; Nováčková, Marta (advisor) ; Pastor, Zlatko (referee) ; Smetanová, Dagmar (referee)
The aim of the dissertation was a retrospective-prospective analysis of the anatomical and functional effect of a neovagina creation using Vecchietti vaginoplasty, including the evaluation of quality of life, sexuality and interest in motherhood in women with Mayer- Rokitansky-Küster-Hauser syndrome (MRKHS). The study group included 70 women with MRKHS who underwent this surgery at the Department of Gynecology and Obstetrics, Second Medical Faculty, Charles University and University Hospital Motol in 2003-2022 and agreed with the participation in the study. The control group consisted of age-matched women without congenital anomalies of the genitals. The cause of MRKHS with agenesis of the uterus and the upper two-thirds of the vagina is the incorrect development of the paramesonephric (Müllerian) ducts. The ovaries are normal, oogenesis proceeds as usual and the production of female sex hormones is also normal. MRKHS is detected during adolescence due to primary amenorrhea at the age of 16 or impossibility of sexual intercourse. In the atypical form of MRKHS, simultaneous malformations of the kidneys, skeleton and cardiovascular organ system may occur. The dissertation is divided into theoretical and empirical parts. The theoretical part presents current knowledge about the history, etiology and...
The effect of manual lymphatic drainage on the upper limb lymphedema after axillary lymphadenectomy for breast cancer
Chmelová, Kristína ; Nováčková, Marta (referee)
The aim of my thesis is to investigate the effect of manual lymphatic drainage on upper limb swelling after axillary lymphadenectomy in patients with breast cancer. The theoretical part will include information about secondary lymphedema including anatomy, physiology and pathophysiology of the lymphatic system, an introduction to breast cancer and its treatment. I will focus on complex decongestive therapy as part of the treatment. The aim of the practical part will be to demonstrate the importance of lymphatic therapy in women after axillary lymphadenectomy for breast cancer who develop a complication of anticancer treatment - secondary lymphedema of the upper limb. The topic will be presented through a prospective study.
Evaluation of postoperative lymphoedema after differently radical surgery for vulvar and cervical carcinoma
Nováčková, Marta ; Halaška, Michael (advisor) ; Špaček, Jiří (referee) ; Mlynček, Miloš (referee) ; Kučera, Eduard (referee)
The aim of this study was a prospective detection of postoperative lymphedema of the lower limbs in patients after the surgery for cervical and vulvar cancer using different methods of examination and their comparison and monitoring of postoperative complications and quality of life. Totally 78 women were followed after the surgery for cervical cancer and 36 for carcinoma of the vulva. Due to the radicality of the surgery the patients were divided into the conservative and radical groups. Lower limbs lymphedema were evaluated preoperatively and 3, 6 and 12 months after the surgery by the measurement of the lower limbs circumference, multifrequency bioelectrical impedance analysis (MFBIA) and subjective feeling. Quality of life using the European Organisation for Research and Treatment of Cancer (EORTC) questionnaires was evaluated before and 6 and 12 month after the surgery. 12 months after the cervical cancer surgery 35.9 % of patients reported subjective lymphedema, 37.18 % lymphedema were objectively diagnosed by the measurement of lower limb circuits and in 52.56 % of cases the increase of amount of extracellular fluid was detected by the MFBIA Ri/R0 method. The prevalence of lymphedema after the surgery for vulvar cancer reached 19.44% by the subjective assessment, 38.89 % by the measurement of...
