Evaluation of cost effectiveness of NOAC (New Oral AntiCoagulants) in the indication of atrial fibrillation
Vothová, Petra ; Lešetický, Ondřej (advisor) ; Pažitný, Peter (referee)
In my thesis, in the theoretical part I deal with atrial fibrillation. I also deal with management of atrial fibrillation treatment. In the paper I explain the basic principles of pharmaco-economic evaluation in the Czech Republic, on the basis of which laws these pharmacological interventions are evaluated and who are in the Czech Republic. In the work I present cost breakdown and what are the most commonly used pharmaco-economic analyzes. In the practical part I will apply the theoretical knowledge. I have developed cost utility analysis based on available clinical evidence - randomized clinical trials ARISTOTLE (Granger, 2011), RE-LY (Connolly, 2009), ROCKET AF (Patel, 2011). I made an indirect comparison of the total benefits of NOACs. In the baseline scenario, I have calculated QALY to evaluate the benefits of effectiveness, safety and reduction of mortality. I have also dealt with a bleeding-related scenario. The ICER's greatest contribution to the public healthcare system has shown the active substance apixaban in both scenarios. I added the results and confirmed the clinical results of a recent, robust, retrospective study by Mayo Clinic (Yao, 2016).
Program of continual health care quality improvement in a private clinic in Ukraine
Shvets, Anna ; Lešetický, Ondřej (advisor) ; Pažitný, Peter (referee)
The aim of this master s thesis is to establish continual program of health care quality improvement in a private multi-specialty medical facility Nebolejka based in Ukrainian city Kharkov. Thesis is divided into several parts. Theoretical part is focused on quality examination from the healthcare system point of view, deals with the analyses of Ukrainian healthcare system actual state. Practical part is focused on health care quali-ty evaluation in a private clinic Nebolejka. With a use of patient satisfaction survey a program of health care quality improvement was established. Its effectiveness was also verified via repeated patient satisfaction survey in short- term time period. This thesis is primarily useful for medical facility Nebolejka, because improvement of health care quality not surprisingly constitute to better competitive market position.
Cost-effectiveness of vaccination against the bacteria Neisseria meningitidis B in children aged 0-4 years
Kříž, David ; Pažitný, Peter (advisor) ; Kandilaki, Daniela (referee)
After many years of research have been developed, and then in January 2013 approved by the European Medicines Agency new multicomponent vaccine against meningococcal disease Group B Bexsero from Novartis. Meningococcal disease, because of its rapid progression and tragic consequences, a very dangerous disease. The aim of my thesis was to try to quantify the impact on the costs and benefits of the introduction of vaccination for the most vulnerable groups, namely children after birth to 4 years of age, by comparing the hypothetical vaccinated and unvaccinated cohorts. The work itself has not demonstrated at today's prices and epidemiological data Bexsero vaccines, cost efficiency. However, at a reduced cost or increased incidence of cases appear to vaccination Bexsero as well utilized resources.
Effectiveness of price regulation of drugs from a social point of view in the Czech Republic
Půžová, Kateřina ; Pažitný, Peter (advisor) ; Lešetický, Ondřej (referee)
The aim of this thesis is to determine the effectiveness of price regulation of drugs from a social point of view in the Czech Republic. The first part focuses on the theoretical basis for subsequent processing practical part. Theoretical solutions include a description of the objectives and instruments of health and drug policies, and a more detailed description of price regulation in the Czech Republic as a tool of drug policy. The practical part is composed of two parts. The first of them covers three models that show how you can set a maximum price of producer. This first part is the starting point for the second part. Second part deals with the analysis of administrative proceedings in which it exercised its right to express their views to the documents MAH medicine. The analysis shows that price controls are not efficient in these cases, and it burdens the society's interests. The thesis also recommendations for increasing efficiency.
Cost analysis of remote and in office pacemaker check ups after device replacement from the perspective of health care payer
Čurila, Karol ; Lešetický, Ondřej (advisor) ; Pažitný, Peter (referee)
Current pacemakers allow check up of their function using telemonitoring. Even so, the majority of the checks are carried out on an outpatient basis. It is not known, whether remote follow ups of the patients after pacemaker replacement would be cost-effective compared to in-office controls. A retrospective analysis of patients files of patients after pacemaker replacement in 2002-2005 in Faculty hospital of Kralovske Vinohrady was done. A total of 1407 records from the years 2002-2015 was analyzed. It was demonstrated that the vast majority (96%) of planned controls of these patients was not associated with a change of program of the pacemaker, so it could be performed remotely. Based on the obtained data a model, which compared the cost-effectiveness of outpatient and theoretically made remote controls of pacemakers was created. Using an analysis of cost minimalization, it was found that the remote controls of pacemakers, was not associated with a significant reduction of the costs compared to the ambulatory controls.
