The possibility of simplifying the diagnostics and therapy of rare diseases in dentistry with the help of a database-based expert system in 2D and 3D imaging Abstract The present dissertation describes the methodology of creating a database of patients with rare diseases in the orofacial area. Clear and structured data are the basis for working with knowledge-based systems also in clinical practice. The use of database data is described for two specific diseases, namely Ectodermal Dysplasia and Zimmermann-Laband Syndrome. The methodology for working with the database consists of the steps of identifying a patient, adding a new card to the database, adding patient records and creating layouts. In the case of ectodermal dysplasia, a set of 13 Czech paediatric patients with ectodermal dysplasia presenting in the oral cavity was processed. The patients underwent genetic testing of candidate genes EDA, EDAR, EDARADD, TP63 and WNT10A. Three-dimensional facial scan images were taken of the patients and compared with facial scans of a healthy control group. Contemporary methods of treatment of injuries and their complications in immature permanent teeth in children

This study deals with the evaluation of endodontic treatment using calcium hydroxide materials in immature permanent teeth that have sustained traumas, especially after severe periodontal tissue injuries. - The aim of this work was: a. to analyze the factors that contribute to occurrence of post-traumatic complications, especially after avulsion injuries in immature permanent teeth , b. to investigate the effectiveness of endodontic therapy of these complications with the use of calcium hydroxide materials in immature permanent teeth, c. to evaluate various calcium hydroxide materials, d. to compare our results with other studies in literature concerning calcium hydroxide properties in cases that it is used as an intra-canal medicament. - The pre-clinical and clinical parts of this thesis are divided in 3 parts: 1. Evaluation of primary schools teachers` knowledge on providing first aid in cases of tooth avulsion injuries: immediate and proper first aid and fast transfer of children to dental practitioner are determining factors for prognosis of the injured tooth and for future occurrence of post-traumatic complications. 300 teachers were asked to fill questionnaires about avulsion of permanent teeth. 69 % of the respondents had never been informed about providing first aid in dental trauma situations, 34 %...

Children's dental fear as a minority factor in caries prevalence

Dental caries is a preventable multifactorial infectious disease which results from the overall impact of biological, behavioural and psychosocial factors. The environment of every individual plays a role in its aetiology, too. The content and form of the diet, regularly performed oral hygiene, fluoride intake and continuous dental care act as an unquestionable part in the prevention of dental caries. "Quality of life" or "well-being" is nowadays an emphasized aspect in most of the studies. There is one factor linked to the well-being of a child discussed on a field of paediatric dentistry - the dental fear. The four following studies presented in this dissertation describe the above mentioned topic. The first surveys the dentist's relationship with the child patients and looks for the reasoning why the children were referred to the faculties and specialists. The second pursues the environmental factors which influence the perception of dental environments in pre-schoolers and the fear of even the most common dental procedures. The third performs an evaluation of the dental fear level in a sample of preschool children in the Czech Republic, with correlation to their dental status and geographic location. Oral health of preschool children and its relationship to demographic-social family background and parental attitudes and behaviour regarding caries prevention in children

1 SUMMARY The thesis is based on three cross-sectional studies. The first study analyzed long-term trends in caries experience of Czech 5yr olds. The data originated from 6 studies on caries experience of 5yr olds performed over the years 1994-2009. During this period, a significant increasing trend in dmft=0 was observed in Czech 5yr olds, the trends of mean dmft per child and mean d per child were not significant. The aim of the second study was to characterize family attitudinal and behavioural factors related to dental decay prevention. It was a questionnaire survey in parents of preschool children. The data was analyzed by factor analysis enabling to find latent associations among large number of variables with similar variability and group them into a smaller set of newly formed factors. The results showed that the parents have sufficient knowledge on the main risk factors for dental caries, however, in the caries prevention they mainly rely on dentists, they consider it normal to give sweets to children daily and they underestimate the seriousness of premature loss of primary tooth. Diagnosis and treatment of the injuries to the permanent teeth

Injuries of the permanent teeth represent extensive and serious problems of dentistry. With the exception of the hard dental tissues, gingiva, periodoncium, dental pulp and alveolar bone are offen affected. The healing of injured teeth depends upon different factors, such as the stage of root development, the presence of bacterial infection in oral cavity and the possible penetration of bacteria or their toxins through the dentinal wall. These factors as well as incorrect treatment can cause posttraumatic complication, which appear immediately after dental trauma or up to several years lato. In this study the evaluation of 384 patients with injuries of the permanent teeth was performed. All these patients were treated in the Dentistry Department of the Faculty Hospital in Pilsen. The aim of the clinical part of the study was the analysis of the epidemiologic data and of the occurrence of posttraumatic consequences in every group of dental injuries. The investigated dates were compared with the dates in the literature. The relation between patient' age, stage of root development, treatment of dental trauma and occurrences of posttraumatic complications has been investigated.
