Subjective evaluation by users of community-based services in the field of mental health care

The present dissertation focuses on the evaluation of the effectiveness of community services provided in Mental Health Centres. Its theoretical framework provides a description of domestic and international studies and concepts that serve as the main theoretical underpinnings for the thesis. The thesis explores the conceptualization of serious mental illness, including its description, epidemiological overview, diagnosis, treatment, and the concept of recovery as a central approach in the care of persons with serious mental illness. It also describes the system of psychiatric care, psychiatric and psychosocial rehabilitation, the functioning of the Mental Health Centres and the methods of evaluating the provision of psychiatric care in the Czech Republic. A qualitative longitudinal study involving two data collections was used to answer the research questions. Reflective thematic analysis was used to process the data. The research was based on interviews with clients of Mental Health Centers III and their family members, whose statements mapped their expectations and the fulfillment of these expectations. Family psychoeducation and relapse prevention in psychotic disorders

Relapse prevention is a primary long-term clinical goal in treatment of psychotic disorders. Pharmacotherapy by itself is not sufficient, because compliance of patients in remission is usually low and there are more factors contributing to relapse, especially stress of patients resulting from emotionally demanding situations in the family. PREDUKA (PREventive EDUcational programme for relapse prevention) is a six-hour professionally-led group programme for patients with psychotic disorders in ambulant therapy and for their relatives. In a field questionnaire survey we mapped 1) theoretical knowledge of participants, 2) usage of practical advice obtained from PREDUKA and 3) general benefit of programme. Participants from years 2007 - 2009 were included. The sample consisted of 14 patients and 22 relatives, 27 women and 11 men. The average age of patients was 28,6 years, that of relatives 44,4 years. High return of questionnaires from patients confirms their goodwill to cooperate. Knowledge of participants about psychosis and their therapy is very good, compliance of patients from our sample is high. Participants usually follow advice obtained from PREDUKA. Common is the ignorance of important phone numbers (crisis centre, attending psychiatrist), so we suggest to include a page in a workbook for these dates....
The impact of metabolic parameters on cognitive functions in bipolar patients treated with mood stabilizers

TITLE: The impact of metabolic parameters on cognitive functions in bipolar patients treated with mood stabilizers. OBJECTIVE: Cognitive impairment in euthymic phase of bipolar disorder has been documented in many studies. There are several contributing factors exerting detrimental effect to such an impairment, e.g. sedative medication, thyreopathy. Cardiometabolic rick factors constitute another frequent condition found in bipolar disorder and exerting probably adverse impact on cognition. Since this condition is treatable and current literature suggests possible connection with cognitive dysfunction, this study aimed to explore such associations to identify promising targets of complex treatment. METHODS: Forty euthymic bipolar patients were enrolled into a prospective one-year study. Patients underwent the examination in the beginning of the study and one year after. Their body and metabolic parameters were measured. Medical history data were collected. Cognition was evaluated using battery of tests. Neuropsychological performance was transformed into neurocognitive composite score. Day treatment programme is clinically-based 6-week structured integrated programme for out patiens with schizophrenia, best after first attack, when patients need psychological support as soon as possible to cope with their illness and with reality,that they have to use drugs. The programme is led by psychiatrist and nurse-therapist. The group day treatment programme consists of individual and family psychoeducation,social skill training, life style improvement intervention, cognitive remediation and information aided relapse prevention programme ITAREPS. The principal aim of day treatment programme i afford place for psychiatric rehabilitation and surmount stage after release from hospital. Combination of all those interventions is more effective. The thesis is devoted to the issue of involuntary hospitalization in the field of psychiatry. Involuntary psychiatric hospitalization is accompanied by a wide range of ethical issues and practical problems. In the theoretical part of the thesis I describe the involuntary hospitalization from several different points of view. I pay attention to international conventions and documents relating to the issue, reveal the historical roots of involuntary hospitalization, compare the situation in different European countries and mention some important judicial decisions concerning the involuntary admission of a person. Using a comprehensive approach I map all the important aspects of this sensitive area in health care provision. In the research part, using both - the quantitative and the qualitative approach, I offer a more detailed look at two selected aspects surrounding the involuntary hospitalization. The first part of the research is focused on the subjective experiences of involuntarily admitted patients. 88 patients participated in the research. These patients described their perception of coercion, the possibility or impossibility of influencing decisions about hospitalization and subsequent treatment, the level of autonomy and emotional experience. Association of education with mental illness in Czech population Abstract This master thesis aimed to study the association of education with mental disorders in the Czech population. Data were used from a nationally representative cross-sectional study - the CZEch Mental health Study (CZEMS). Mental disorders were assessed with Mini-International Neuropsychiatric Interview and divided into four groups - affective, anxiety, substance use and alcohol use. Information about the highest completed education was self-reported by participants during an interview. Binary logistic regresion examined the association of education with mental disorders, group-wise adjusting for sociodemographic, social and health-related characteristics. The analysis was conducted on 3 175 participants (54 % women, median age 49 years). Lower education (primary and vocational) was associated with higher occurrence of mental disorders, this association remained after adjustment for all characteristics. The association was strongest for alcohol use disorders and weakest for anxiety disorders and did not differ by sex.
