A summary on self injury

Self injury is a common problem in our society today. A large percentage of adolescents engage in this behavior and health care workers will most probably meet some of these as patients at some point in their career. There has been a lot of research on self inflicted injury over the last decade or so, but it is rarely a part of the curriculum for most health care workers. This article aims to give a brief summery on today's' knowledge about self injury. It contains a description of the behavior, it discusses possible causes, treatments and consequences and suggests what can be done in order to prevent this behavior. The article is based on on a literature search from PubMed and articles from The Journal of the Norwegian Medical Association. The article concentrates on self injury as an act of deliberately doing damage to one self, and this damage is the primary cause of the behavior. It does not include self injuries done in the name of religion or damage to tissue as an adverse effect, as would be the case in for example drug abuse, eating disorders and smoking. Body modifications that are intended for ornamentation are not considered a form of selfmutilation, as they are done for spiritual or social purposes. Prevention of patient harm caused by wrong drug administration

The diploma thesis surveys the most important problematic fields of the patient safety issue; particularly in the context of drug administration. In the general part there are described the risks associated with LASA medications, using of concentrated electrolyte solutions, incorrect patient identification and wrong communication. Reported clinical cases of medication errors that occurred in connection with the triggers mentioned above are demonstrated and there are listed recommendations aimed at preventing the adverse events and assuring safe care. The closing part of the thesis describes and compares how the Antwerp University Hospital and three teaching hospitals in Prague ensure the safety of drug administration. The thesis manifests a noticeably more elaborate system of education and promotion of the patient safety issues both among the staff and among the patients and their families in the Antwerp University Hospital. Also the technical assuring of the right medication (barcode scanners, automated dispensing devices in certain wards etc.) was in the Belgian hospital on higher level comparing to the average of the followed Czech hospitals.
Nursing care of the child with Hyperkinetic syndrome

Cílem závěrečné práce je zpracování případové studie ošetřovatelské péče o třináctiletém dětském pacientovi s diagnózou hyperkinetický syndrom. V klinické části své práce v samotném úvodu krátce nastíňuji historii dětské psychiatrie. V hlavní části popisuji hyperkinetický syndrom, jeho příčiny, projevy, základní terapeutické postupy a léčbu. Popisuji i důležitá specifika práce sestry na pedopsychiatrickém oddělení. Dále se zabývám prognózou i prevencí toho hyperkinetického syndromu. Pracuji se základními údaji o nemocném dítěti a popisuji průběh jeho hospitalizace. V ošetřovatelské části své práce pracuji s metodou ošetřovatelského procesu. Ke zhodnocení zdravotního stavu nemocného dítěte jsem zvolila model Marjory Gordonové. Po analýze všech základních informací o dítěti stanovuji ošetřovatelské diagnózy. Pro podrobné zpracování krátkodobého plánu ošetřovatelské péče jsem vybrala 10. den hospitalizace. Plán realizuji a následně hodnotím. Dále stanovuji dlouhodobý ošetřovatelský plán. Zabývám se edukací pacienta.
Self-harm prevention in inpatients

The aim of this diploma work is to explore tactics which would help serve as a preventive measure from inpatients inflicting self-harm. This work was conducted in selected psychiatric departments throughout the Czech Republic by using an anonymous questionnaire investigation. Only fifty percent of the departments have internal guidelines for detection of at risk patients. Eighty-eight percent of departments have an internal system for reporting extraordinary events. Many of these departments have advanced technical equipment, but there are still some departments with technical equipment at lower levels, probably due to the high costs associated. Only two departments are using scales for evaluation of admitting patients. Fifty percent of the departments evaluate risks of self-harm for admitting patients. Self-harm was present in all departments in this study; seventeen percent of departments did not report any number of suicide attempts and forty two percent of the departments did not report any number of the self-harm attempts. The results are showing a positive trend in the role of prevention, as well as increasing the safety level within individual departments. Continous Quality Improvement in a Healthcare Organization

This graduation thesis describes some changes, which had taken place in the Central Military Hospital in Prague before this organization obtained the prestigious international accreditation - Joint Commission International. An example of one of these changes is introducing a new type of medical documentation, which meets the demanding requirements of accreditation standards. The thesis is divided into two parts - the theoretical part and the practical one. The theoretical part is divided into two chapters. The first chapter deals with describing the quality and it contains a description of some models of continuous quality improvement as well as their comparison. The following part describes the main principles of the management of changes in a healthcare organization. The practical part of the thesis presents the Central Military Hospital in Prague as well as some changes that accompanied this organization's effort to obtain the international certificate JCI. The research is focused on the nursing staff and their opinions of the introduction of new medical documentation. The thesis could serve as an inspiration to those healthcare organizations that are preparing (for future continuous quality improvement) improvement of the quality of their services.
