Palliative care in neonatology from a nursing perspective

The aim of the research diploma thesis was to find out about the system of children's palliative care with a focus on the neonatal period and to compare the opinions of health professionals on the system of palliative care in neonatology. The aim of the research part was also to evaluate the readiness of nursing professions to cooperate with the public in the field of care for premature newborns at the end of life. The theoretical part presents current knowledge in the field of perinatal palliative care. Furthermore, the theoretical part maps the current knowledge about the possibilities of support and further education of health professionals and defines the possibilities of saying goodbye to the child after perinatal loss. The empirical part presents the results of a qualitative research survey using standardized in-depth individual interviews with neonatal nurses in five perinatology centers in the Czech Republic.
Supportive Care for Premature Newborns with psychological and ethical Aspects

The thesis, or more precisely its theoretical part, introduces the field of Neonatology, namely the specification of premature newborns concerning supportive care for them. In the thesis, the basic concepts of Neonatology are presented as well as the system of neonatological care, including the needs of premature newborns, which are part of everyday nursing care. Following the above chapters, the thesis describes not only the nursing care as such but also the possibilities of psychosocial care in Neonatology. For a more comprehensive presentation of the issue of premature newborns, the thesis also includes findings concerning ethical issues in Neonatology and it introduces some organizations in the Czech Republic, which are an integral part of the supportive care. The character of the thesis should correspond with the program of nursing care, namely nursing in selected clinical disciplines - the module of paediatrics. In the empirical part of the thesis, qualitative research by interviewing has been used. Despite this scientific approach is more time-consuming, the author considered this method more meaningful and natural concerning the chosen topic because it concerns the support of the lives of these children and their families. Premature newborns are unique, affecting and enriching human being as such. The target of the thesis was firstly to find out how supportive care in premature newborns is perceived by its providers, secondly to disclose the considerations of the care providers regarding premature newborns in České Budějovice. The research was effectuated in connection with the above objectives, which have importance and a significant value for the author. Conclusions of the Thesis: The informants assumed that society didn't have enough information about the provided services. Almost all the informants collaborated on some research or project. As service providers, the respondents acknowledged the comments from the clients. The informants were interested in improving or enhancing the services. The addressed people have specified many factors and difficulties that make their work difficult. The informants concretized foreign experience. The respondents felt considerate enough. The informants expressed sincerely that there was a lot of space for improvement in their profession. The respondents mostly took a partner and gentle stand to the premature newborns and their families. The service providers gave due reasons for a specific approach in their statements. The informants expressed their respect for premature newborns and their families, giving a specific justification. They mostly expressed respect for the needs of the families, for the biorhythms of the baby, for the naturalness, the intuition and the personal pace of the baby. The thesis can be seen as a source of information for both the general and professional public, which could improve current practice. Based on the presentation of the issue through the thesis, the author has published a guide on premature newborns.
Psychological and ethical aspects of the last things of man

The thesis deals with the very last issues of a person from the psychologically-ethical view, however it doesn't neglect the social view. With the topic is worked also in a context of selected current ethical systems, mainly deontologism and utilitarianism, also regarding the topic of the thesis, the topic of bioethics is discussed as well. Regarding the field of psychology, the thesis elaborates more in detail on the existential psychotherapy, actually also on the logotherapy, which was shaped by an Austrian psychologist Viktor E. Frankl. There is also included the problematics of burial, the possibilities of giving a last farewell, also in a religious and an ethnical point of view. The key part of the thesis deals mainly with a needed psychosocial care for the terminally ill, the dying and also their close ones, in the times of pre finem, in finem and post finem by the prospective death of adults, it deals also with the influence of this care on the experiencing of the final period of life of the dying, and the following grieving of the survivors. In the thesis is also shortly mentioned the topic of palliative and hospice care. The thesis also follows the evaluation of the influence of the last farewell on the process of the survivors' grieving. The inspiration for writing the thesis were apart from professional literature and other professional sources also to a bigger extent the many years of work experience of the author, thanks to which is the work supplemented with many own cases.
Effect of acute stress on paramedic and subsequent crisis intervention.

This thesis deals with problems of psychological stress sustained by paramedics working in rescue service, as well as mapping the possibilities of the crisis intervention in case of excess stress events in the practice of these workers. The thesis is divided into two parts, theoretical and practical. The theoretical part of the thesis defines the basic terms relating to rescue services and paramedics, the stress, stress levels and subsequent psychosocial care, crisis intervention and its history. The practical part is focused on the level of mental stress found in paramedics in service, which was examined by the Meister questionnaire. In addition, the respondents were asked about the possible symptoms of acute stress. Another part of the questionnaire examined the awareness and possible experience of psychosocial assistance and crisis intervention among the paramedics of rescue services. The survey was conducted by questionnaire method in two regions of the CR, the Hradec Králové Region and the Moravian-Silesian Region.
Transformation of father´s role after the birth of a premature child from the psychosocial viewpoint

The bachelor thesis is based on the experience of problems with premature babies from the fathers? point of view. The main target of the bachelor thesis was to find out how the father sees his role after the birth of the premature child, to point out his feelings and needs and simultaneously to map the psychosocial impact on the family from his point of view. In the bachelor thesis the following research questions were asked: How the father felt in the period before and straight after the birth of the premature child? What helped tothe father to manage the unexpected situation? Did the situation reflect in any way in his normal life? The thesis was elaborated based on the qualitative research. Data was acquired based on talks with fathers and by the analysis of contents of documents. The research sample was fathers of children who were hospitalized in the neonatology ward of the Hospital in České Budějovice. The results of examination show that the premature birth of a child is, for the father a very difficult life experience, influencing considerably his everyday life. For the father, the fact that he has the opportunity to share with somebody his feelings and needs is very important.
Effects of surgery treatment of epilepsy on psychosocial condition of the patients.

The thesis "Effects of surgery treatment of epilepsy on psychosocial condition of the patients" deals with the disease which causes many social effects on the patients. The most frequent ones are limitations in the working life and mental problems. I am interested in the epilepsy issue and therefore I have selected this topic which continues the bachelor thesis written in 2008. The objective of my work was to determine effects of epileptosurgery on the work life and on the mental area. Two hypotheses were established which assumed that the surgical treatment improved the patient´s life both in the working and in the mental spheres. The hypotheses were tested by means of quantitative research; questionnaires were used for the collection of data and the respondents were adult patients, who underwent surgical treatment of epilepsy and have been in the care of the Center of Epilepsy Na Homolce in Prague, and patients in the care of neurology departments in České Budějovice and in Strakonice. The results have shown that the epileptosurgery failed to improve the lives of the patients in the working sphere and thus the first hypothesis was not confirmed. Many of the patients are still unemployed and they still find it difficult to find jobs. The offer of vacancies has not increased much after the surgery. Those who work often feel stress and anxiety as a result. Some patients even lost jobs after the surgery or their working positions do not correspond to the achieved qualification level, experience and potential. The improvement in the mental area has been confirmed and many of the patients no more suffer from mental problems they had before the surgery. In some of the respondents mental disorders diminished or disappeared entirely. They are more self-confident, more even-tempered and they need less professional help. In this area I see the effects of surgical treatment as very beneficial. The results of my thesis might be used in publications for the general public, as well as for professionals specializing in the field.
Social worker as a member of a team handling the comprehensive care of a family with a child born prematurely

The thesis deals with psychosocial aspect of premature birth, to which our society does not still pay the attention they would deserve. The aim of the thesis is to emphasise the need to provide specialized psychosocial care to families with prematurely born children within a neonatology ward and to evaluate the possibilities to apply the role of a social worker to provision of such care. The thesis is conceived as a qualitative research. The techniques of semi-structured interview and data content analysis were applied on data collection. The research sample consisted of 11 specialists of various professions involved in the care about prematurely born children and their families. The work partly continues in the research of 2007 where I dealt with the psychosocial issues of premature births from the point of view of parents of prematurely born children. The research results definitely confirmed that striving for creating a conception of specialist psychology care forming a link between health care staff and parents is necessary. Such care should be based on the following forms of assistance so as it supports family function and child{\crq}s healthy development: provision of crisis intervention, support of early relation between the mother (parents) and the child, support and strengthening parent competences, advisory on care about a prematurely born child or perhaps a handicapped child, social law counselling, mediation of related services, support of sharing experience among parents. A social worker is fully competent to provide such care. As the research shows no unified legislation dealing with his/her activities within a multidisciplinary team of a neonatology ward exists. The Social Service Act defines the conditions for the profession; separate activity would probably be subject to special regulations dealing with the activity of a social worker in hospitals. The work may be useful to all professionals pushing for rendering complex care about families of prematurely born children, i.e. care naturally involving specialist psychological care.
CHOICES OF THE PSYCHO-SOCIAL CARE OF FAMILY WITH THE CHILD BORN PRE-MATURELY

This paper deals with the problems of prematurely born children. Premature birth means an extremely stressing situation not only for the child itself, who is because of its premature birth in danger of many health complications, but also for the child´s parents. The objective of this thesis was to map the possible ways of psycho-social care of a family with a prematurely born child. The work was concieved as a quality research. The data gathering techniques used were semi-standardized interview, participant observation and secondary data analysis. The researched croup were mothers of prematurely born children who gave birth earlier than in the 32nd week of pregnancy and who spent more than three weeks with their children in the infant unit of the hospital in České Budějovice.
