ANALYSIS OF SELECTED MODELS OF FINANCING HEALTH CARE

The aim of this bachelor thesis is to analyze models of financing health care and compare the systems of health care. Rising health care costs, increasing labour costs for higher payments to health care system have been discussed whether the funding of ongoing health care system is sustainable. The theoretical part focuses on the existing models of health care and mechanisms, under which systems of health care are structured. The analytical work will discuss models used in Great Britain, Sweden, Germany and Switzerland. These systems are among a few basic models, which can be found principles of behaviour, which may be recommendation and advice to other systems of health care. Firstly thesis shows the mechanisms and principles of individual models. Further, it shows financing of these models and associated advantages and shortages of the systems. Then these models will be compared with the system of health care in the Czech Republic. Finally, the possible alternatives will be presented, which can be inspiration for Czech health care system for its further development and possible reform health care financing in the Czech Republic.
Analysis of corruption in health

This BA thesis deals with influence of corruption on number of competitors in relation to detection of final price of public order in medical service. The academic part defines concept of the word "corruption", its legal framework, kinds of corruption, possibilities of corruption measurement, control system of public competition etc. The second part of the thesis is an analysis of public order efficiency with the aid of regressive model. For the intention of the BA thesis, 223 public orders executed in the Czech republic during 2006-2009 period were choosen in random way with use of variable quantities - entering price, final price, number of offerors etc. The thesis attemps to confirm presumption of relation between explicite competition ( number of submited offers) and final price. The anticipated outcome lies in the fact that the number of submitted offers fundamentally influences the final price of the public order in the Czech republic. Presumably it's the consequence of uneconomical and non-transparent legal regulation of public medical service which supports mismanagement dealing.
Incorporation options of nurse educational activities in public health service

The present thesis named ``How feasible it is to integrate educational activities provided by nurses into public health care system{\crq}q is based on the fact that education comprises a huge part of nurses{\crq} professional lives which translates into an essential aspect found in nursing practice. Educational process or education is a type of activity which involves a man learning and a man teaching. It takes place in an educational environment defined by physical conditions (waiting room, special room for schooling, classroom, etc.), people present and their psychosocial relations. Education helps to meet preventive goals in modern nursing. To achieve this in targeted primary care, nurses must focus on identifying health risks which shall be consequently eliminated with active client{\crq}s participation in the nursing process alongside with the help provided by the nurse and doctor. Therefore it is essential for a nurse-educator to be well educated so that she can educate adequately and at high-quality level. The survey included in the thesis is based on a quantitative survey employing a method of asking questions listed in a questionnaire. The questionnaire was anonymous. The target group for such survey included general nurses working at clinics in Jindřichův Hradec and České Budějovice. The thesis tries to find out how feasible it is to integrate nurses-educators into the public health care system. Firstly, four hypotheses were defined. Hypothesis 1: The role of a nurse-educator is feasible to be incorporated into the public health care system. Hypothesis 2: Nurses are interested in incorporating the role of a nurse-educator into the public health care system. Hypothesis 3: Nurses see the role of a nurse-educator as an essential part of the public health care system. Hypothesis 4: Institutions involved in the public health care system are interested in nurses-educators. All four hypotheses proved correct. The target of the thesis was accomplished. The survey showed that education is in the common interest of nurses, their superiors, and clients as well. Education has become fundamental for public health care system which is required to meet flexibly demands of its clients. The thesis may be used as a guideline for students and working nurses so that they can perform efficient education using proper tools and materials which meet the required standards properly.
Private sector in public health care systems

This master thesis is trying to describe the situation of private sector in public health care systems. As a private sector we understand patients, private health insurance companies and private health care providers. The focus is placed on private health care providers, especially in ambulatory treatment. At first there is a definition of health as a main determinant of a health care systems, definition of public and private sectors in health care systems and the difficulties at the market of health care, including externalities, inperfect information and elasticity of supply and demand at this market. There is also the problem of financing health care as a mix of public and private payments, which is different in every health care system. The main focus is laid on the situation in the Czech republic. As a comparison there are described situations in Denmark, Germany, Great Britain and United States. The size of private sector in every health care system is different and it depends on different circumstances in particular state, including social-economic development, regulation of health care market from the state and the attitude of patients to private health care.
Public finances reform in the light of pre-election proposal by the Civil Democratic Party

In this work I would like to evaluate government's reform about public finances, everything what was agreed upon and voted for by the end of 2007. At the same time I would like to compare that to the original pre-election proposal by the Civic Democratic Party and government's declaration by Mirek Topolanek coalition's party. In my work I will concentrate mainly on revenue of state budget, where the most impact manifest of the fiscal reforms is seen. The main differences are consolidation of tax brackets for individuals and also changes in rates of VAT, lowering corporate taxes and introduction of ecology taxes. Even though this reform appear like the straight taxes from the pre-election program from ODS, this form is only similar in tax brackets, but not by other structural tax provisions. On the side of current expenses was government less bold. On this issue, they enforced only small differences in the transfer expenditure, where they discontinue few pre-election populist statues, even though almost no changes were done in this section so far. This proposal in pre-election for negative tax was much more bold and interesting, because here together with the tax system the state could safe money and also be a motivation for people to work. Purpose of work is outlining other necessary steps in the reform process for public finances and also in area of financing of public medical system, retirement system and changes in budgetary allocation of taxes.
