Indications of Emergency Medical Services Responding

The bachelor's thesis is focused on a closer understanding of the issue of indications of emergency medical service (EMS) departures through the perspective of medical emergency dispatch centers. The issue of indications of emergency medical service departures is one of the still addressed and current topics in the pre-hospital emergency care. The activities of outreach crews as well as the activities of the dispatch centers are governed by Act 374/2011 Coll., however non-emergent, calls to the emergency line are becoming more and more frequent. The thesis maps the knowledge of the indication list of individual dispatchers and the attitude of dispatchers towards the indications itself. At the same time, it deals with the solution of unindicated calls and also the abuse of emergency services. The aim of the work is to compare the knowledge and procedures of dispatchers of both regions. The research took place at the Medical Emergency Dispatch Center of the South Bohemian Region in České Budějovice and at the Medical Emergency Dispatch Center of the Pilsen Region in Pilsen. The aim of the research was, among other things, to compare approach to indication issue between both regions. The research method chosen was a qualitative type of research that allows deeper examination of the topic. Research was executed by semi-structured interviews. The sample of respondents consisted of 8 dispatchers of the medical operations center i.e., people directly responsible for receiving calls and coordinating crews. The interview questions are focused on both the reception of the emergency call and the operational management. Above all, they focus on the urgencies N3 and N4, ones where we most often encounter not entirely unambiguous indications. It also addresses the relationship between the use of formalized and intuitive procedures in operators' decisions. Last but not least operators are confronted with the notion of EMS abuse and the solution of unindicated calls. The interviews provided information on a wide range of approaches to dispatching work but there was often agreement on key points. The results show that dispatchers are familiar with emergency procedures and legislation, however, despite the application of a largely formalized approach to classifications, it is necessary to act partly intuitively, as no call is ever the same as a regulatory case. There is a greater effort to define the terms unindicated calling and abuse of emergency medical services, which, however, are perceived individually by each of the dispatchers, as well as perceived by every rescuer who comes into contact with them. These concepts often evoke great emotions among dispatchers and rescuers. However, as the results show, even this problem does not have only one point of view, but it has several of them and all of them must be taken into account when solving it.
Receiving an emergency call on the line 155

Receiving an emergency call on the line 155 is the topic of this bachelor's thesis. The theoretical part of the thesis deals with the description of the medici operating center, the requirements and workload of dispatchers who are currently working at the medici operating center. It also describes the processing of an emergency call, the method of communication with the celer and, last but not least, telephone-assisted first aid and telephone-assisted cardiopulmonary resuscitation (CPR). In the practical part, two goals were set. The first goal was to map the knowledge of dispatchers in providing first aid for telephone-assisted cardiopulmonary resuscitation of adults. The sekond goal was to map the knowledge of dispatchers in providing first aid for telephone-assisted cardiopulmonary resuscitation of children. Qualitative research was performed using the semi-structured interview technique. These interview were conducted at the Medical Operations Center of the South Bohemian Region. The respondents were the emergency dispatchers .Ten respondents were randomly selected. The results were dividend into twelve categories, for better orientation at bachelor's thesis. Both set goals were achieved in this way. It was found that the dispatchers of the medical operating center responded very carefully and with a clear dose of knowledge to the issue of telephone-assisted CPR of children and adults. Dispatchers have knowledge and can describe the procedure of individual steps efore resuscitation. Knowledge and experience with AED was also at a good level. Respondent's answers regarding the need for resuscitation for more disabled people did not agree, some had no answer. Dispatchers could not imagine this situation and would not know how some would make decisions. When communicating with someone who refuses cardiopolmonary resuscitation, dispatchers disagreed. The results of this work showed the need to include in training programs by the employer methods of communication during urgent resuscitation, preferably using model situations.
A distress call owing to chest pains versus real diagnosis of a flying squad

This bachelor thesis is called: A distress call owing to chest pains versus real diagnosis of a flying squad. Thesis deals with possible causes of inception of chest pains and possibilities of their diagnosis. Theoretical part contains definitions of emergency medical services, differential diagnosis, ischemic heart disease and inflammatory heart diseases which cause chest pains. There are also described respiratory disease, cardiac arrhythmia and other possible causes of inception of chest pains, hypertension, hypoventilation, swoon. There are provided descriptions of cause, symptoms and prehospital emergency care of every mentioned disease. There was conducted a qualitative research and was presented in practical part of this bachelor thesis. Collection of data was based on technique of analysis of secondary data (archived recordings of distress call, departure records) as well as on the method of specific selection. The research sample consisted of case studies of distress calls owing to chest pains and of departure records used at emergency medical service of South Bohemia region. On the account of the quantity ofthe distress calls owing to chest pains wereto this research chosen and compared, after supervisor's advice, only the distress calls owing to chest pains from the months of July and December 2014. The aim of this thesis was to find out the most frequent cause of chest pains. Mapping was done enumeration of specific diagnoses and inserting tables. The research question was: ,,What are the most frequent chest pains?"On the grounds of the quantity of the distress calls owing to chest pains, were to this research chosen, after supervisor's advice, only the distress calls owing to chest pains from the months of July and December 2014. These months were chosen because they both are climaxes of summer/winter periods. The most frequent diagnosis in December 2014 was chest pains NS. There were 145 cases of this diagnosis in December. This diagnosis was the most frequent one in July as well. There were 108 cases of it in July. These facts can be clearly seen in table 1 and 2 which contain diagnosis based on symptom of chest pains. From data, which were collected and analyzed in the practical part, itis obvious, that distress call owing to chest pains can be caused by various conditions. Diagnosis of flying squad showed that chest pains can be caused by cardiovascular diseases, respiratory diseases, gastrointestinal disorders, backache, arm fracture, etc. However, flying squad is not always able to diagnose the cause of chest pains. These cases are labeled as chest pain NS, other pains NS, etc.
The characteristic work of paramedic in the operations centre- stress factors and possibilities of their elimination

In my bachelor thiesis of the Characteristics of the paramedical rescuer´s job at the control centre - stress factors and possibilities of their elimination, I concentrated on the Health control centre and the employees working there. Particularly, on their work and stress factors that affect these employees. The issue of a job at the Health Control Centre of Health Rescue Service is really wide and interesting. In a wide range, operators are able to increase the quality of human rescue due to their knowledge and skills. The theoretical part of my thesis concerns paramedics, health rescue service, the topic of work at health control centres, further stress, reactions to stress, as well as possibilities of stress eliminations. In the research part, I focused on the stress situacions that occur in this profession and that affect every operator. The research was carried out with the qualitative form, where the qualitative research was performed with the technique of a controlled dialogue. The data acquisition was realized with the questioning method followed by registracion and evaluation. The first aim of the thesis was to analyze paramedicals´ work processes at the control centre. As the second aim, I set to find out the stress factors specific for the operator´s profession, to characterize them more closely and suggest their elimination. It resulted from the research thatemployees working at the control centre know their work procedure, their proceed in accordance with them and are further educated in their branch. Moreover, it turned out that operators constantly face stress situacions and tasks which they have to solve. However, these situacions can be partly influenced, as written at the end of this thesis. In conclusion, I would like to mention that this thesis contains my opinions on this topic and suggestions for the improvement of the present situacion. At the same time, the thesis offers space for a discussion and consideration.
Emergency Operators´ Mental Load

This bachelor thesis deals with mental strain in dispatchers of medical emergency services. I focused on covering these questions, the follow-up analysis and description, on the basis of research. The first objective was to survey opinions of operations centre dispatchers of the mental strain in their work. The next objective was their opinion of the question whether the number of dispatchers on duty in one shift influences their mental strain. My research is based on questionnaire surveying, and the research object covered 40 employees in operational centres all over the Region of South Bohemia. (Note: a nationwide research would be technically hardly realisable, as it is improbable that the questionnaires sent in would be returned). The introductory chapters contain a brief description of the operations centre and activities that the dispatchers perform every day, and I try to formulate the principles of the correct communication that should be led in the conversation with the callers on the distress line. I also describe general features of stress, the factors that lead to it, and the states breaking out as a result of its long-term effect on people. Specifically in this thesis, I describe post-traumatic stress disorder and the burn-out syndrome, and I also mention prevention to defend oneself against these risk factors. The second part of the thesis focuses on the results of the research conducted by quantitative technique, by handing out 45 questionnaires, 40 of which were processed after they were sent back. The research and results make up an essential part of the thesis and bring proofs of the fact that employees of operations centres undergo mental strain influenced by a number of factors. Some of these factors can be influenced, and becoming aware of these factors, this strain may be at least partially reduced. The thesis also uses the comparative method with the possibility of comparison of the mental strain between individual dispatchers. Last but not least, it contains inspiration for considering these questions, and personal proposals for a positive contribution to lowering this strain and a further development of this problem in general.
