A Birth House as a Compromise between Childbirth at Home or at Hospital

This work is devoted to the topic of birth houses. The work consists of a theoretical part and a practical part. The theoretical part describes the choice of the place where to give birth. There is described the hospital birth (and its related birth plan) and home birth. The issue of birth centers is described in detail both in the world and in the Czech Republic. Another topic is birth itself and its possible alternatives. The theoretical part is closed by the definition and position of a midwife. The work intended to map the interest of Czech women in giving birth in the birth center. Four hypotheses were set for the goal. H1: University-educated women are more interested in giving birth in the birth center than women with lower education. H2: Women with a bad experience with giving birth in the hospital are more interested in giving birth in the birth center than women with a good experience. H3: Women who gave birth in hospitals are more interested in giving birth in the birth center than women who gave birth at home. H4: Women who have already given birth are more interested in giving birth in a birth center than women who have not given birth. The set goal was achieved by a quantitative research survey in the form of a questionnaire of own design. The questionnaires were evaluated by using a statistical chi-square test, which determined the significance or insignificance of the results. The research group consisted of 981 women active on the social network Facebook and various web servers related to pregnancy. The research showed, that university educated women show a higher interest in giving birth in the birth center than women with lower education. We also found, that women who have been dissatisfied with hospital care are more interested in birth centers than women who report having a good experience. We also wanted to find out whether the place of the previous birth influences the interest of women in giving birth in the birth center. This hypothesis has not been confirmed. The result is influenced by the small number of respondents who experience a home birth. The last hypothesis was related to whether women who have already given birth are more interested in giving birth in the birth center. It turned out that women´s opinions on childbirth in the birth center do not differ depending on whether the woman has already given birth or not. The research survey showed that 59,2 % (answers yes or rather yes) of women are interested in giving birth in a birth center. We can therefore assume that part of the women would like such a facility to exist in the Czech republic.
The Controversy about Childbirths outside Hospitals

1 ABSTRACT The issue of births outside a medical facility is currently a controversial topic between specialists as well as in the public sector. While the specialists concur in refusing home births, the society is divided. There are voices that refuse home births but there are also plenty of people who support the right of mothers to give birth outside of a medical facility. Very sensitive topic is the legal and ethical aspects of the protection of unborn children and the issue of a safely conducted delivery. In the last decades, the life value of an unborn child has been seen as an ethical problem. The right to life is one of the fundamental human rights, which is enshrined in the constitutional system of the Czech Republic and also in the international human rights conventions. Because there are different opinions regarding this issue, it is necessary to establish a view that is widely accepted so that legal standards can be approved provided that the opinions of minorities are being respected. Childbirth as a natural part of life, where the woman giving birth has prime position and the option of choice. She should feel free and accepted - to be able to give birth physiologically (without unnecessary external interference). Physiological childbirth as the best for a woman, a baby and their relationship. Up to 80 % of women do not need a doctor to give birth, but only a "suitable environment". In my diploma thesis I propose a "maternity house". This concept is well-known abroad, in our country it is a new type of care. For this purpose, I use two existing houses, which are owned by the city of Blansko and stand next to the hospital.
Maternity hospital

A new concept of maternity hospital creates a place, where future mother is not stressed and where she feels free. A place, where birth is perceived as a common part of everyday life. It searches a compromise between birth at classical hospital environment on one side and birth at home on the other. Both aforementioned possibilities bring certain risks that are intended to be minimized using this concept. The idea is based on practice from birth facilities from Germany and creates new concept of so called „birth houses“ that is consistent with law in the Czech Republic. Design of maternity hospital should not be based solely on meeting technical requirements of a bulding, but first of all comfort of mothers should be taken into account to make them feel home.
Choice of a maternity hospital by expectant mother

The birth of a child is one of the most beautiful experiences that is remembered by the woman throughout her life. Therefore it is very important to find the right place in which she wants to give birth and to understand what she expects from such a place. The theoretical part summarizes information about maternity facilities, as well as about various alternative childbirth options. It also deals with factors that influence the choice of the maternity hospital. The practical part investigates how the mothers are informed about maternity facilities and about the factors that influence their choices of where to give birth. In order to examine the topic and to achieve the objectives a quantitative research method using anonymous questionnaires was used; the questionnaires were distributed to women who had just given birth and were hospitalized at maternity wards in the hospitals in Benešov (Nemocnice Rudolfa a Stefanie Benešov, a.s.) and in České Budějovice (Nemocnice České Budějovice, a.s.) Two objectives and related hypotheses were established. The first objective was to find out which factors influence the women when they choose maternity facilities. The other objective was to find out whether mothers preparing for the first birth decide differently than mothers who already have a child or more children. Both objectives have been met. The hypothesis 1 states that ?Clients choose such maternity facilities where they feel that they and their children are safe? and it has been confirmed. Also the hypothesis 2, stating that ?Clients prefer maternity facilities which maintain privacy of mothers giving birth?, has been confirmed. The hypothesis 3 to the effect that mothers preparing for the first childbirth (primiparas) choose maternity facilities differently than mothers who already have a child (multiparas) has not been confirmed. The theory and results of the survey may be beneficial for nurses and for development of information brochures for pregnant women attending pre-birth classes to help them to select the maternity facility, as well as for midwives in practice.
