Title: Telerehabilitation Intervention in Pediatric Occupational Therapy Abstract: The goal of this thesis is to evaluate advantages and disadvantages of telerehabilitation in pediatric occupational therapy with pre-school children. At the beginning terminology and history of telerehabilitation is discussed. The thesis continues with evaluation of advantages and disadvantages of telerehabilitation in occupational therapy and selection of suitable guidelines for practical part of thesis. In practical part, the occupational therapy intervention with two pre-school children is designed and carried out. The occupational therapy plan was designed, and in accordance with the plan the aids were made for the use of family as ergo box. The telerehabilitation intervention took place in the form of weekly consultations with parent. During the consultations, parents were given the tasks for the child for the next week and the progress of the previous week was evaluated. Cultural diversity of early childcare

This bechalor thesis is refers to diverse ways of early care for children across the cultures. This is also the main topic of this thesis. In the first part, the thesis clarifies the roots of the cultural diversity and its reasons. Further, the thesis clarifies the concept of early care for children and also describes the child´s development, it´s needs and also deals with the aspects that influence the early care and upbringing. In the next chapter, the thesis deals with its main topic which is the early care for children across the cultures. The thesis follows up the childbirth, breast-feeding, sleep, crying and the child´s contact with the mother. In the last part, the thesis deals with the interaction between an early age child and games and also with the fact what the early care for children in Czechia looks like.
The role of a nurse in preventing CAN syndrome

Abstract Current State: CAN (Child Abuse and Neglect) Syndrome is defined as physical and mental abuse, sexual abuse and neglect of a child. CAN syndrome is the result of an intentional activity of parents or other adults towards a child, the most extreme form of which is killing the child. The aim of the thesis was to evaluate the trends of CAN syndrome occurences in the Czech Republic within the observed period of 2008-2018, to describe nursing care for children suffering from the CAN syndrome, and to evaluate the role of nurses within the area of preventing this syndrome. Methodology: A combination of a qualitative and quantitative research strategy was chosen for the empiric part of the thesis. To collect data for the quantitative research the method of secondary analysis of MLSA documentation from the period of 2008-2018 was chosen. 79 340 cases of maltreated children were analysed. To collect data for the qualitative research, the method of semi-structured interviews was used. The research file consisted of nurses working at a paediatric clinic at a chosen hospital in Prague (10 nurses) and nurses working at practitioners´ offices for children and adolescents (5 nurses). The size of the research file was given by theoretical data saturation. Results: In the period of 2008-2018, 79 340 abused and neglected children were reported to the MLSA. The most frequent form of CAN Syndrome was neglect (65.8 %). 12.6 % children suffering from CAN Syndrome were mentally abused, 12% were sexually abused, and 9.6 % children were physically abused. Proportion of boys and girls was relatively balanced (48 % boys, and 52 % girls). The most numerous group of abused children according to their age were children aged 6-15 (56%). Statistical testing has proved the hypothesis that in the period of 2008-2018, there was a statistically significant growth in the number of children with the CAN Syndrome diagnosis (p<0,001). Addressed nurses came into contact with this syndrome within their practice, they were well educated within the area of symptoms, differentiation between individual forms of CAN, diagnosis, and risk factors leading to the CAN Syndrome. Nurses in both, primary as well as inpatient care play a significant role in preventing the CAN Syndrome. Nevertheless, as the interviews have proven, they do not educate all of the parents about this syndrome, they only do so in risk groups of parents and children. Conclusion and Recommendations for Praxis: The quantitative study that evaluated the statistical data coming from MLSA, has confirmed a significant growth the number of children suffering from the CAN Syndrome for the last ten years. Due to this fact, it is necessary to put greater emphasis on the CAN Syndrome prevention. Based on the results of the study and general recommendations, further recommendations for the management of nursing care have been designed, dealing specifically with the issues of prevention and nursing care after children suffering from the CAN Syndrome.
Children febrile convulsions in pre-hospital care

The bachelor's thesis deals with the children febrile convulsions in pre-hospital emergency care. The theoretical part of the thesis describes the matters of childhood, its specifics and division. It also focuses on the clinical picture, complications, prevention and treatment of febrile convulsions in children in prehospital emergency care. There were two goals set in the practical part. The first goal was to map and compare the procedure of paramedics in febrile convulsions in children in the emergency ambulance crew of the South Bohemian Region and the Vysocina Region. The second goal was to map the knowledge of paramedics in febrile convulsions in children in the emergency ambulance crew of the South Bohemian Region and the Vysocina Region. Two research questions were also identified: How are paramedics in the emergency ambulance crew of the South Bohemian Region and Vysocina providing pre-hospital care to children with febrile convulsions and what is the knowledge of paramedics about febrile convulsions in children? To obtain the necessary data, the method of qualitative research was used through the semi-structured interviews with 10 paramedics of the Medical Rescue Service of the South Bohemian Region at the regional center in Ceske Budejovice and Medical Rescue Services of the Vysocina Region in Trebic. Respondents were answering sets of 16 questions and the outcome was categorized into 13 categories, which succeeded in meeting both goals and at the same time answered both research questions.The research found that the interviewed paramedics do have sufficient knowledge about the causes of febrile convulsions and what their basic therapy is. Unfortunately, they don't know a division of children age well enough. All respondents agreed that they were afraid of the diagnosis of Febrile convulsions precisely because of the pediatric patients and that the knowledge in the field of pediatrics is not their strongside. There was no significant difference between the two regions in the answers concerning the procedures of febrile convulsions in children in pre-hospital emergency care.

of the Thesis: The aim of the bachelor's thesis was to create a manual on iPad use for parents of children with special needs. The manual will be used in home-based therapy, which will be parents able to guide without continuous supervision of a professional worker. The theoretical part deals with the general procedures and methodologies of occupational therapy in the field of paediatrics, especially for children with special needs. In addition, the thesis discusses the use of technology, games and other apps in therapy, as well as the safety risks of overuse and misuse. The role of the family in the child's rehabilitative care and in home-based therapy is an integral part of the work. The practical part is based on a practical verification of the effectiveness of the tablet as a rehabilitation tool and the subsequent creation of a manual for working with the iPad. Biochemical predictors of the treatment failure in pediatric IBD patients

Up to 20 % of patients with IBD (inflammatory bowel disease) is diagnosed in childhood, the course of the disease tends to often be more severe and the treatment more difficult than in case of adults. Therefore, it is necessary to commence or eventually change the treatment in order to prevent development of complications. Early detection of relapse plays significant role for further course of the disease and general prognosis of the patient. Changes to laboratory parameters (e.g. to the level of fecal calprotectin) can significantly prevent the development of clinical symptoms and may indicate the necessity for control endoscopy, further escalation or change in therapy. The primary focus of this thesis are patients who underwent surgical bowel resection, who constitute one of the risk groups. Studies show that as many as 70 % of patients suffering from Crohn's disease reach the point of bowel surgery within the first 10 years of the diagnosis. According to the results, a promising predictor of relapse of the disease seems to be serum albumin, which may relate to the overall nutritional status of the organism. The target of this work is to compile individual physiotherapeutic program for patients with Spinal Muscular Atrophy in childhood. The theoretical part deals with a general introduction to the issue of alpha motoneuron and its involvement with hereditary degenerative diseases. Section of the theoretical part is the anatomy and physiology related to this issue. The Impact of a dietitian guidance on a weight management of obese children

The bachelor thesis deals with the topic of childhood obesity and the impact of dietitian education on its prevention and treatment. The theoretical part defines owerweight and obesity in children and adolescents, its prevalence, causes, diagnosis, prevention and treatment. The practical part is based on data drawn retrospectively from the medical documentation of the dietitian's and obesitologist's outpatient's department of the Department of Pediatrics and Hereditady Metabolic Disorders of the General Hospital in Prague. Data for the period from 1st January 2018 to 1st February 2022 were examined in 58 patients aged 3 months to 18 years 5 months with overweight or obesity. Only patients whose overweight or obesity was caused by a positive energy balance were included in the study. The data included the pacient's anthropometric parameters at birth and during the treatment, blood pressure, uric acid levels and fasting blood glucose. The patient's parents BMI was found out at the beginning of the child's treatment. Metabolism of immunosuppressants in children with inflammatory bowel disease

Metabolism of immunosuppressants in children with inflammatory bowel disease Abstract Thiopurines are still used as the main immunosuppressants in the maintenance therapy of paediatric Crohn's disease. According to official guidelines, in case of failure, biological therapy (mainly infliximab or adalimumab) is generally commenced. The present thesis focuses on the possibilities of therapy optimization in children in whom the conventional Crohn's disease therapy has failed based on the metabolism of administered immunosuppressive medications. We confirmed that the knowledge of thiopurine metabolites concentration in red blood cells may help to monitor patients' adherence to therapy. However, according to our observations in children on combination therapy of infliximab and thiopurine lower concentrations of 6- thioguanine (the active metabolite of thiopurines) seems to be effective compared to what has been expected. We presented a web application designed to partially substitute for the thiopurine metabolite measurements based on easily available laboratory data. Last, but not least, we confirmed that the clinical effect and safety profile of both adalimumab and infliximab are similar in the treatment of paediatric Crohn's disease. Only in case of family history of atopic dermatitis adalimumab may be preferred.
Clinical and elektrophysiology longitudinal study of children with hereditary neurophathy Charlot-Marie-Tooth type 1A

Hereditary peripheral neuropathy, known asCharcot Marie Tooth disease (CMT) and with an incidence of 1:2500 -1:10 000, is the most common hereditary neuromuscular disorder. Type CMT 1A is the most common form of CMT refering to the group of primary demyelinitateing motor and sensory peripheral neuropathies. CMT phenotype is clinically characterized by chronic slowly progressive distal muscle weakness and atrophy with hypo or areflexia and mild to moderate acral sensory loss. The lower limbs are predominantly affected. The aims of this study were to describe the first and most common signs of CMT1A during the first decade of life, to characterize their progression, and evaluate the sensitivity of CMTNS (Charcot-Marie- Tooth neuropath scale) for CMT1A young children. Sixteen children aged 3 to 10 years with genetically proven CMT 1A were examined. All patients were clinically examined, underwent electrophysiological examination, and were scored by CMTNS. Eight were followed for up to two years. Our data shows that CMT 1A in children under the age of 10 years causes only a mild disability. Initial signs of CMT 1A were difficulty in heel walking (15/16, 93%) and lower limb hypo or areflexia ( 13/16, 81%).
