Operation of emergency medical service in selected regions of the Czech Republic following a mass disaster

The current system and working status of medical rescue service related to mass accidents has undergone changes in recent years, including gradual development, progress, modification and improvement. Together with that development, the conditions and opportunities needed for provision of timely and high-quality urgent pre-hospital care have transformed and improved automatically. The provision of professional pre-hospital urgent care for conditions endangering human life has been becoming the basic contents of activity of the rescue service. The care can be provided through operation centres that should receive and evaluate distress calls on line 155 and through field rescue vehicles pulling out of a network of trip points all over the country. The unification of the procedures and organization when settling mass accidents constitutes an indispensable measure leading to improvement of such interventions. The adequate procedure may lead to life rescue and to mitigation of the consequences of injuries for a number of persons equal to the number of the whole year of standard activity of the medical rescue service. Of course, the opposite may apply too. Therefore each physician or rescue worker should know the basic rules of behaviour at the point of the accident. The goal of the submitted dissertation is to map the general suggestions for the work of the medical rescue service of the Czech Republic, to analyze three medical rescue services related to mass accidents - that of the South Bohemian Region, the South Moravian Region and the Vysočina Region - and to compare weak and strong points of the medical rescue service of the Czech Republic based on the analysis of the three organizations in connection to mass accidents - of the Medical rescue service of the South Bohemian Region, the South Moravian Region and the Vysočina Region. Based on the SWOT analysis of selected medical rescue services, it was possible to identify their strengths and weaknesses, opportunities and threats. Each of the analyzed medical rescue services can see the opportunities of their respective organizations in other points. The medical rescue service of the South Bohemian Region sees its opportunities in the improvement of the education and training centre; the medical rescue service of the South Moravian Region sees its opportunities in the development of solution of mass accidents and the medical rescue service of the Vysočina Region sees its opportunities in finishing the urgent admission and first aid for schools; but all the analyzed medical rescue services see their threats in the expected lack of funds, caused by funding of new ambulances or new trip points or by increasing wages in the public sector. The lack of physicians in permanent employment is not only a matter of the above stated rescue services. The problem has countrywide character at present. The importance of the lack of physicians is not proportionally dependent only on cases of mass accidents. At present, it can be stated in general that there are multiple causes of lack of interest for internal work in rescue service. Low attractiveness of work in ambulance crews of rescue services becomes one of the main causes. Also the working conditions are usually incomparable. The crew of the rescue service must perform actions of pre-hospital urgent care in field under all and any circumstances. Another difference, as compared to the work of a physician in the hospital, consists in the need of right and quick decision-making without the opportunity to consult a specialist and without support of laboratory examinations. So the rescue service physician must constantly master different situations, from childbirth to post-mortem examination.
Integration of operations centres in the South Regional Police Headquarters Region

As the topic of my diploma work I chose the issue of integration of operation centers in the jurisdiction of the Directory of Czech Police of the South Bohemian Region. This topic was chosen with regard to my job in the police of the Czech Republic and also with the assumption to assess the current state of the operation management in the jurisdiction of the Directory of Czech Police of the South Bohemian Region and compare it with the newly proposed integrated centre which should be created within the framework of integration of existing regional operation centres, evaluation of communication of operations centres within the framework of the integrated rescue system and within the framework of the police of the Czech Republic itself at different levels of operation management. The aim of my thesis is to describe the current state of workplaces of operation centers of the police of the Czech Republic and to assess their integration into a single operation center in the teritory of the South Bohemian region. I analyze the situation from the viewpoint of conduct, management and personal security and I assess material-technical security. Last but not least it's about confirming or refuting the hypothesis that was formulated before i started the thesis. This thesis stems particularly from internal act of conduct of the police of the Czech Republic and from the current legislation. Aims of my work are summarized in the capter Discussion which was preceded by description of the current state of operation centers and their function in the framework of operation conduct. Comparing the current and the suggested solution from the viewpoint of conduct, management, personal security, economic and material-technical security and physical integration. The formulation given by the hypothesis was confirmed based on the comparison carried out in the study of the current state and the prepared state, and seems to be effective. Because of the introduced measures that are realized by integration of the current operation centers into one, the effectivity will be increased.
The characteristic work of paramedic in the operations centre- stress factors and possibilities of their elimination

In my bachelor thiesis of the Characteristics of the paramedical rescuer´s job at the control centre - stress factors and possibilities of their elimination, I concentrated on the Health control centre and the employees working there. Particularly, on their work and stress factors that affect these employees. The issue of a job at the Health Control Centre of Health Rescue Service is really wide and interesting. In a wide range, operators are able to increase the quality of human rescue due to their knowledge and skills. The theoretical part of my thesis concerns paramedics, health rescue service, the topic of work at health control centres, further stress, reactions to stress, as well as possibilities of stress eliminations. In the research part, I focused on the stress situacions that occur in this profession and that affect every operator. The research was carried out with the qualitative form, where the qualitative research was performed with the technique of a controlled dialogue. The data acquisition was realized with the questioning method followed by registracion and evaluation. The first aim of the thesis was to analyze paramedicals´ work processes at the control centre. As the second aim, I set to find out the stress factors specific for the operator´s profession, to characterize them more closely and suggest their elimination. It resulted from the research thatemployees working at the control centre know their work procedure, their proceed in accordance with them and are further educated in their branch. Moreover, it turned out that operators constantly face stress situacions and tasks which they have to solve. However, these situacions can be partly influenced, as written at the end of this thesis. In conclusion, I would like to mention that this thesis contains my opinions on this topic and suggestions for the improvement of the present situacion. At the same time, the thesis offers space for a discussion and consideration.
Blackout and Its Impact on the Emergency Medical Service

At present, due to the use of different technologies, it is hard to think of any human activity that does not demand electric power. It is necessary to be prepared for of large-scale long-term blackouts. Possible scenarios of the occurrence of such a crisis situation must be analyzed and the crisis management must be prepared; primarily to minimize the impact on the lives and health of the population, their property and also natural environment. The objective of the thesis was to identify the operational readiness of regional centers of Emergency Medical Services in the Czech Republic for the possibility of blackouts and to create a survey of power supply possibilities to the operating center of the Emergency Medical Services. The questionnaire was focused on the analysis of operational readiness of regional centers for total blackout, on the framework of their technical support in alternative energy production and its time sustainability. The documents analysis included a pattern plan of the Ministry of Trade and Industry of the Czech Republic and an operational plan for the occurrence of a crisis situation - large-scale electric power supply disturbances, and other internal regional documents concerning the situation during a blackout. The outcomes of the thesis can be used as a background material for the comparison of emergency blackout preparedness in international environment. They can be used to achieve certain standards in the field of sustainable alternative energy supply of Emergency Medical Services centers.
Operational and Crisis Management within the Police of the Czech Republic

This thesis deals with the issue of operational and crisis management within the Police of the Czech Republic. The organizational structure and the staffing of operational centers and emergency management centers is described as closely related to police activities, with emphasis on the responsibility for executing tasks resulting in particular from internal acts of the Police management as the essential component of the Integrated Rescue System (IRS). The issue of emergency calls handling and the related communications between the basic components of the integrated rescue system, particularly between the Police and the Fire and Rescue Service, considering the technological and material and technical equipment of operational centers of the police of the CR is dealt with. The empirical section is focused directly on the "modern" Integrated Operational Centre of Regional Police Headquarters of the Vysočina region where questionnaires are distributed to workers (policemen) of this organizational element of the Police, the object of study is divided into three parts. The first part is focused on the IRS issues, the level of knowledge, orientation in this area and views on cooperation of the IRS components are identified by the questionnaire. The second part is focused on crisis management, the knowledge level of respondents in this area is determined. The third part of the questionnaire is directed entirely to obtain information and knowledge from practice of the staff members involved in special activities. The discussion and conclusion gives information about the progressive development and integration of operational centers, including the possible and most convenient way (system) of operational and crisis management within the Police of the Czech Republic.
Emergency Operators´ Mental Load

This bachelor thesis deals with mental strain in dispatchers of medical emergency services. I focused on covering these questions, the follow-up analysis and description, on the basis of research. The first objective was to survey opinions of operations centre dispatchers of the mental strain in their work. The next objective was their opinion of the question whether the number of dispatchers on duty in one shift influences their mental strain. My research is based on questionnaire surveying, and the research object covered 40 employees in operational centres all over the Region of South Bohemia. (Note: a nationwide research would be technically hardly realisable, as it is improbable that the questionnaires sent in would be returned). The introductory chapters contain a brief description of the operations centre and activities that the dispatchers perform every day, and I try to formulate the principles of the correct communication that should be led in the conversation with the callers on the distress line. I also describe general features of stress, the factors that lead to it, and the states breaking out as a result of its long-term effect on people. Specifically in this thesis, I describe post-traumatic stress disorder and the burn-out syndrome, and I also mention prevention to defend oneself against these risk factors. The second part of the thesis focuses on the results of the research conducted by quantitative technique, by handing out 45 questionnaires, 40 of which were processed after they were sent back. The research and results make up an essential part of the thesis and bring proofs of the fact that employees of operations centres undergo mental strain influenced by a number of factors. Some of these factors can be influenced, and becoming aware of these factors, this strain may be at least partially reduced. The thesis also uses the comparative method with the possibility of comparison of the mental strain between individual dispatchers. Last but not least, it contains inspiration for considering these questions, and personal proposals for a positive contribution to lowering this strain and a further development of this problem in general.
