Pharmacotherapy at the ortopedic department from nurse´s perspektive

Attention should be paid to pharmacotherapy in the orthopedic department mainly because patients at this department make up a risky group, they are older and often tend to use more types of drugs at the same time. Nursing at the orthopedic department is both mentally and physically demanding. Nurses administer medication to patients throughout the whole day, that is why lack of certain medication may occur in this department, the same way they would with all routine procedures. Be it conscious or unconscious. The thesis sets its objective to map out the issue of pharmacotherapy in the orthopedic department. Further, to map out factors that may influence the origin of complications when administering medication to patients in the orthopedic deartment, and suggest improvements in the process of pharmacotherapy in the orthopedic department. A qualitative research via participant observation and semi-structured interviews carried out with the nurses and patients of the orthopedic department, was used. The results of this survey prove that nurses do have basic theoretical knowledge in the area of pharmacotherapy. At the same time certain shortcomings were discovered, i.e. this theoretical knowledge is not being applied in practice. Most surprising were the results in the area of patient identification before administering medication, this step not always being abode by the nurses as a part of medical error prevention. Quite on the contrary, it can be said, based on the results, that nurses do have excellent knowledge and skills in pain reduction. This is proven by both participant observation and the happiness of patients themselves. Partial results have been presented at a seminar in the orthopedic department, and they have been passed on to the management of this department. The resulting shortcomings have been used as aid material. The diploma thesis was written so as to serve as material of information to beginner or existing nurses at the orthopedic department as well.
Nursing care for a client with percutaneous endoscopic gastrostomy at the Department of Subsequent Care.

The bachelor thesis has been divided into a theoretical and a practical part. The theoretical part dealt with enteral nutrition, percutaneous endoscopic gastronomy. The theoretical part is concluded by a chapter about nursing clients with PEG probe. The practical part contains two aims. The first aim was to map out specifications of nursing clients with percutaneous endoscopic gastronomic probe. The second aim was to find out what problems there are in nursing clients with PEG probe from the nursing staff perspective. Two reseach questions were posed: Question number 1 was focused on the specifications of nursing clients with PEG probe. Question number 2 aims at problems in nursing perceived by nursing staff in clients with PEG probe. The empirical part of the bachelor thesis was processed via a qualitative research survey, using semi-structured interviews and involved observation. Interviews were carried out with eight probants in the aftercare department. Results have proven that interviewed probants manage PEG client nursing using a PEG probe. They manage to nurse the injection spot, serve nutrition and medication through the PEG probe. Problems have been spotted in sucking stomach content away, and client positioning before nutrition serving. Further, the research has proven that probants stated PEG probe blocking as one of the most frequent complications, together with an inflammation of the injection spot, everything was consulted with a doctor. The results of this bachelor thesis could be used for purposes of educational seminars in aftercare department, thus increasing nurses´ awareness as well as imporvement in PEG probe client nursing
The level of self-care patients after leg amputation

This thesis deals with the issue of patients after the amputation of the lower limb. Loss of the lower limb means a great change for the patient, not only in understanding the integrity of one's own body, one's own health, but also from a functional point of view. The theoretical part of the thesis presents the latest available knowledge about lower limb amputation and postoperative care for patients with this diagnosis. It also deals with self-care, based on Dorotheus Orem's theory of self-care deficit. The aim of the work was to determine the level of self-care of patients after amputation of the lower limb using some aspects of D. E. Orem's model. To find out in what systems D.E. Orem's nurses support patients after the amputation of the lower limb in self-care and how to help nurses care for these patients. The research part of the thesis was realized through a qualitative research survey. The technique of data collection was a semi-structured interview with patients with amputation of the lower limb and nurses who provide nursing care for such ill persons. The theoretical basis for the interview was the DE Oremové model, which was complemented by the ADL (Activity Daily Living), IADL (Instrumental Activities of Daily Living Scale) and the analogue range of pain in the group of respondents - patients. The data obtained were processed in the form of transcripts of the interviews, which are supplemented by evaluation of individual scales. The output of the diploma thesis is a set of recommended measures and a proposal of a comprehensive information material on the amputation of the lower limb for patients, which the nurses should be available to their departments.
Cystic fibrosis and pitfalls of life of patients with this diagnosis.

Cystic fibrosis is a disease that affects the digestive and respiratory systems. It is an autosomal recessive genetic disorder. The hallmark of cystic fibrosis is a thick mucus in the lungs, which must get out of the lungs. People with this diagnosis have no favorable prospects for quality life. The individual and his family have a lot to adapt to with this insidious disease. This disease is not curable, but thanks to the progress of science it is treatable. The treatment includes a proper acceptance of medications, pancreatic enzymes, antibiotics, vitamins, respect proper diet, intense inhalations, rehabilitation, periodic check-ups at the doctor, etc. The individual afflicted with the disease can live to adulthood, which was not possible before. Since its very childhood the child has to be led to a proper attitude to treatment and be aware of what can worsen his illness. Over the time, when the child becomes an adult it takes those life habits to its partnership where there should exist a support from the partner. Afterwards, they should both consider whether to have a family and everything that goes with it. Most men with cystic fibrosis have a blocked transport of sperm with thick mucus so sperm cannot pass through the egg and fertilize it. Only a small percentage of men can have a baby the natural way. Most men are recommended during puberty sperm collection and storage in the vault sperm. The name of the thesis is Mucoviscidosis and Pitfalls of Life of Patients with this Diagnosis and is processed purely on a theoretical basis. The main source for the work is the primary sources, i.e. professional publications of Czech and foreign authors. The aim of this work is to obtain comprehensive information about the disease, including the difficulties of life of patients with this diagnosis. The second part outlined the problems which cystic fibrosis has on an individual's life, from childhood to adulthood. The third part mentions the social issues. Financial assistance is emphasised, as the treatment of this disease is expensive.The partial goal was to focus on the latest advances in the diagnosis of cystic fibrosis, which is a new method of sweat test. It is important to receive digestives enzymes before each meal along with inhalations.The information about the disease was obtained mostly from specialized publications, articles from professional journals, as well as the official website of organization of the affected individual. The main goal of bachelor thesis was to get a comprehensive information about the disease including the difficulties of life of patients with this diagnosis. As a help to achieve the main objective significantly contribute sub-goals. Partial aim is to introduce the disease itself from its beginnings to its development, especially in terms of diagnosis. Another objective in writing this work was to explore the issue of this disease and provide informatik to the public and the family with a sick child. In terms of methodology, methods of explanation, synthesis and induction are used in the work. These methods are based on collection and combination of primary information and are aimed at the logical reconstruction of explanation and understanding of the topic.
