Nursing care for patients surgery with lymphedema in the limbs
PÁNKOVÁ, Kateřina
This bachelor´s deals with Nursing care for patients surgery with lymphedema in the limbs. We set ourself dwo essential targets. The first target is: to map out the specifics of nursing care after a surgical solution of the secondary lymphedema from the perspective of nurses. The second target is: to map out the specifics of nursing care after a surgical solution of the secondary lymphedema from the perspective of patients. The theoretical part is focused on disease lymphedema. Lymphedema reflects a serious disorder caused by malfunctioning lymphatic passages. This disease is characterized by unpainful, pale swelling, cold to touch, which is soft at first and later grows stiff and fibrous. This disease is divided into primary and secondary lymphedemas. We focused which surgical operations are used for solution of the lymphedema. The next we focused which of nursing care provided from nurses. The next we focused hwo patients reacted to surgical treatment. We evaluate methods of qualitative exploratory research were chosen in the practical part of the bachelor ´s thesis. Quantitative research with the method of in-depth interviews was selected to achieve the objectives of the thesis. The first research set comprised two nurses, who work for the University Hospital Motol in Prague. The other research set was made up of 8 female patients after surgical operations. Based on the analysis were created main categories - Lymphedema, Preoperative care, Postoperative care and Follow-up care. The research revealed that liposuction is an operation most commonly for treating lymphedema. With its satisfactory results, liposuction proved successful for all of our respondents.

Adipose-derived stem cells (ASCs) are of a great importance for bone tissue engineering. We compared ASCs obtained by liposuction under two pressures (-200 mmHg and -700 mmHg). The number and proliferation activity of cells isolated under -700 mmHg (ASC-H) were higher than in cells isolated under -200 mmHg (ASC-L). However, the ASC-L was more active in osteogenic differentiation, as manifested by a higher intensity of fluorescence of alkaline phosphatase and osteocalcin in these cells. The adhesion and growth of ASCs were then studied on Ti-6Al-4V samples either unmodified (K, roughness parameter R-a = 280 nm) or modified by shot blasting and tarnishing A, R-a = 200 nm), vibratory finishing (B, R-a = 100 nm) and vibratory finishing, shot blasting and polishing (C, R-a = 80 nm). All modified samples were more wettable than the K samples. On day 1 after seeding, the size of cell spreading area on some modified samples was lower than on K samples, namely on B samples in ASC-L, on B and C in ASC-H and on A in control MG-63 cells. On day 3, the cell number on some modified samples became higher than on K samples, namely on C samples (ASC-L), on B (ASC-H) and on B and C s (MG-63 cells). On day 7, the numbers of ASC-H and MG-63 cells on all modified samples evened out and became significantly higher than on K samples. On ASC-L samples, the highest cell numbers were obtained on A samples. Thus, all studied modifications of Ti-6Al-4V enhanced growth of ASCs and human osteoblast-like MG-63 cells.
Nursing care in aesthetic medicine from clients´point of view
BLÁHOVÁ, Vanda
This bachelor thesis focuses on nursing care in aesthetic medicine, particularly on operations of augmentation and liposuction type, which are most demanded by clients. Aesthetic medicine has been a more and more demanded surgical field by women in recent years. Thanks to high demand among women various operation methods and techniques are being developed, aesthetic surgeons are professionals in their field, however nursing care is also indispensable in this sphere. Nursing care in aesthetic medicine is very important for successful course and result of an operation intervention. The theoretical part of the bachelor thesis deals with plastic and aesthetic surgery in general. A chapter on history and development of plastic and aesthetic surgery in the world as well as in the Czech Republic follows. As aesthetic surgery is also psychosurgery (it helps women gain their lost self-confidence in all the above instances), we can find a chapter on psychological aspects of aesthetic surgery here. We should also mention the chapter focused on information of clients undertaking an aesthetic operation, as information for clients is extremely important for provision of quality nursing care. Awareness of clients is linked with professional approach and communication with them. More than a half of the theoretical part of this thesis deals with the nursing care from long-term preoperative preparations including psychical preparation of a client via familiarization with and signature of the instructed agreement, to client's decision for an operation. The nursing care is also described in detail in the chapters dealing with augmentation and liposuction operation type. These chapters focus on admittance and discharge of a client to home care. Two goals were set for the bachelor thesis: 1: To examine perception of the nursing care by clients before and after augmentation and liposuction. 2: To find out whether the clients undertaking augmentation and liposuction are sufficiently informed on the course of the perioperative care. Three research questions were asked: 1: How do clients undertaking augmentation and liposuction perceive the nursing care? 2: In what spheres are the clients undertaking augmentation and liposuction informed? 3: What information do the clients undertaking augmentation and liposuction miss in the field of perioperative care? The empiric part of the thesis is processed by a qualitative method in the form of semi-structured interview and involved observation. The obtained results were categorized and then subcategorized. The involved observation results were entered in an observation sheet and subsequently evaluated. The research results show that clients undertaking a plastic and aesthetic operation are satisfied with the nursing care. The clients assessed the approach of nurses and communication positively. Nevertheless, privacy and intimacy of clients is not ensured properly during redressing. The awareness of clients in the sphere of preoperative preparation, postoperative care and nursing in domestic environment is sufficient, but there is always something to improve. All the clients are moreover informed in written by a so called informed agreement and a discharge report. The research has shown that the clients are poorly informed on compressive garments, which also emerged from both, the interviews with the clients and the involved observation of the nurses. The involved observation has also shown that nurses do not measure the dimensions for determination of the correct size of compressive garments with a tape, but they just estimate them visually. The bachelor thesis outlines the nursing care in aesthetic medicine, namely in augmentation and liposuction type operations. It focuses on the problems of postoperative compression garments. An informational brochure "Postoperative Compression Garments".
The nursing care with clients after aesthetic plastic surgeries
MÍLOVÁ, Veronika
Aesthetic plastic surgeries are highly specific operations which help patients both physically and mentally. This bachelor{\crq}s thesis focuses on breast reshaping, reduction and augmentation, liposuction, face-lift and abdominoplasty, predominantly on nursing care for clients after these operations. Nurses working in the department of plastic surgery must be perfectly skilled in preoperative and postoperative nursing care, but they should also be familiar with surgical techniques related to nursing care. Nurses must know about compression garments, the monitoring of physiological functions, postoperative massage techniques, and so on. Nurses working in this area of surgery should remember that the client is a biopsychosocial-spiritual being with psyche, perceptions, and feelings. They should provide clients with support and help, while maintaining a professional approach. The investigative part of this thesis examines nursing care for patients after aesthetic plastic surgery from a nurse{\crq}s viewpoint. We decided to study this issue in particular because every aesthetic plastic operation has certain specifics. We wanted to find out whether nurses are aware of these differences and well versed in them. This knowledge should include in particular choosing the correct size of compression garments and how to apply them, parameters and values of physiological and vital functions, communication with foreigners, analgesia, redressing, recovery from anaesthesia and surgery. Moreover, rehabilitation nursing, and help with the patient{\crq}s self-care are also part of a nurse{\crq}s work in the department of plastic surgery. The primary objective of this thesis was to find out whether nurses are skilled at providing nursing care after aesthetic plastic surgery. The other objective was to reveal if they know the specifics of nursing care after respective types of operations. Both these objectives were met in this Bachelor{\crq}s thesis. A total of five hypotheses were formulated as follows: nurses are proficient in providing nursing care after aesthetic plastic surgery, they know specifics of nursing care following breast surgery, liposuction, abdominal surgery, and face-lift. All these hypotheses were confirmed. The work is divided into two parts, theoretical and practical. The theoretical part is focused on aesthetic plastic surgery, specifically breast surgery, abdominal surgery, face-lift and liposuction, and nursing care before and after these procedures. The practical part is based on data acquisition using anonymous questionnaires, and candid objective observations. The research population were nurses providing nursing care after aesthetic plastic surgery. The questionnaires were filled out by nurses from Prague, České Budějovice, Plzeň, and Třinec. Observations were conducted at the Department of Plastic Surgery in České Budějovice. This work can be used in practice for example during seminars or to formulate standards of nursing care.
