FRAGILITY AS A TOPICAL NURSING PROBLEM

Introduction: Frailty within geriatrics is still more viewed as a current problem in the world than in the Czech Republic and it is more commonly mentioned. We can say that in the Czech Republic this problematic is still developing, updates and is trying to find its place in nursing as such. Objective: The objective of the presented thesis is to find out if frailty is perceived as a current problematic in the current geriatric nursing. Further, we focused on the geriatric nursing and specifics of nursing care in this field; on what is the awareness of medical staff at aftercare department about geriatric frailty and in which way, within nursing care, they apply their gained knowledge in practice. Methodology and research sample: To achieve the set objective the qualitative research conducted using the technique of semi-structured interview with nurses at aftercare department was used. The basis of this interview was 24 open questions which were predetermined and are documented in attachments (attachment 1) of the thesis. During the interview these questions were extended as needed by additional questions. The interviews were conducted with 10 general nurses at aftercare department of one unnamed South Bohemian hospital. The analysis of interviews was done using open coding paper-and-pencil and the individual information were classified into categories and subcategories. The results grant us comprehensive overview on the nursing care at aftercare departments within the geriatric nursing, utilization of knowledge of nurses about geriatric frailty within their practice and whether nurses perceive the concepts such as geriatric patient, geriatric syndromes and the already mentioned geriatric frailty. It was found out that some addressed nurses at aftercare department of one unnamed South Bohemian hospital never heard some of the mentioned terms and never registered them during their practice either. Other nurses say that they only heard those terms during their college studies, but they also agree that they never met them during their practice. Despite that, all nurses were able to accurately and aptly characterize the terms geriatric patient, geriatric syndromes and geriatric frailty based on their experience and knowledge. Furthermore, it was proven that they realize frailty during the care for patient and can apply their knowledge in nursing care. Conclusion and practical use: The results of the submitted thesis will serve as a template for educational seminar.

Introduction: The 60-year-old population is one of the most vulnerable groups in terms of health. It is necessary to provide highly qualified and professional care to maintain or improve the quality of life. One way to achieve this is to use evaluation tools correctly and appropriately. Aims: The dissertation aimed to determine the attitude of nurses to currently used assessment tools, to determine the incidence of geriatric syndromes in the population over 60 years and their potential impact on quality of life, and the last goal was to validate the Rapid Geriatric Assessment tool. Methodology: The research was carried out using the quantitative and qualitative parts. The quantitative research survey used questionnaire data collection limited to the South Bohemian Region, both for the population older than 60 years, research group A. For nurses providing care for the population older than 60 years, research group B. Questionnaires were created for both research files. They consisted of a non-standardized part and a standardized part. For seniors, a non-standardized survey focused on evaluating the care provided, facilities, the performance of routine activities, and the evaluation of mental and physical health. The standardized part included an RGA tool for geriatric mapping syndromes and a WHOQOL BREF for quality of life monitoring. For nurses, the non-standardized part was intended to evaluate currently used tools and the new RGA tool and for the evaluation of designated areas of health and social care. The RGA tool was translated into the Czech language with the author's consent, tested in a pilot, modified as needed, and then entered as part of a questionnaire survey. The qualitative research survey was conducted using the focus group method and was conducted with nurses who provide nursing care to seniors in a hospital or other type of facility or in-home care. Results: The results showed fragility, sarcopenia, weight loss, mild cognitive impairment, and dementia. Furthermore, the negative impact of geriatric syndromes on the quality of life of seniors has also been demonstrated. For nurses, it has been shown that the level of education and length of practice influence the view of assessment tools and also affect the actual use of assessment tools. Furthermore, the influence of the stay of seniors on the subjective assessment of mental and physical health and on satisfaction with physical and mental health was found. The impact of the most seriously perceived disease on meeting the needs and performing normal daily activities in the elderly was also found. Conclusion: The dissertation provides a comprehensive view of the use of assessment tools in the population over 60 years of age. Modified nursing documentation was created, which should better serve for a basic assessment of the health status of seniors.
Balance training in seniors using force platform

Abstract: This bachelor thesis deals with the issue of balance disorders due to ageing and the usage of the virtual reality in case of these disorders. The aim of this work is to evaluate the effect of home based balance training using the force platform and visual biofeedback. For the objective evaluation the Footscan® system is used. The thesis consists of two parts. Theoretical part summarizes the knowledge related to the balance provision, old age and ageing, involution changes which leads to balance disorders. This part also describes the virtual reality, its usage in rehabilitation and includes several studies of using gaming systems in elderly. In the end of the theoretical part the system which is used in the practical part is described. The practical part describes four weekly home balance training in the form of case reports, charts and graphs. There are analyzing data of seven respondents in the senior age. Typology of Clients Senior House Bechyně

Theme of bachelor thesis is Typology of clients senior home Bechyně. Main goal was to find out, which clients are typical in senior home Bechyně from sociology point of view. Work is contained from two parts so as theoretical part and research.Teoretical part should present the most typical model of client in senior home Bechyně and the importance of adaptation the client in home for the elderly that should not to be underestimated. It also points out the important part of collaboration with family, importance of communication, saturating of needs, care and activating activity, which could help the client in this life period. Research was interpreted by method qualitative research through half-structured interview. Communication partners were chosen from senior home Bechyně. Ten seniors were asked to do the talk according to their maintained cognitive function so they were capable of answering the questions. Complementary method to comprehend look of selected issue was also made interviews with 8 employees of senior home from social and health care department. Research was made in two phases. At first was performed pre-research and after that the main research. Interviews were recorded on dictaphone and rewrite literally. Gained data were evaluated via method creating clusters. From results of the research emerged, that typical client of senior home Bechyně is more than 60 years old senior who has diabetes and suffers from dementia. Most of clients are coming from home environment. During the moving is important to care about adaptation in new environment and it should not be underrated. Gained knowledge and conclusions of bachelor thesis are going to be used as feedback for senior home Bechyně. The conclusion could also be used as foundation of next research or as addition to education material in looking after seniors from health and social aspects.
