Hostile and aggressive behaviour of patients towards nurses in clinical practice

Current state: Aggressive and hostile behaviour of patients towards nurses in clinical practise is an alarming and common phenomenon that jeopardizes the safety and well-being of healthcare personnel. Objectives: The first objective of this thesis was to evaluate the number of adverse events in the "Behaviour of Individuals" category reported to the "National Portal for Reporting Adverse Events" during the period 2018-2021. The second objective was to assess the occurrence of aggressive behaviour of patients toward nurses in clinical practise. The third objective was to describe a spectrum of preventive nursing procedures that minimize the risk of aggressive behaviour of patients towards nurses. Methodology: The first research method involved a secondary analysis of statistical data from the National Portal for Reporting Adverse Events (ÚZIS ČR, 2022). The second research method utilized a quantitative study conducted through the administration of a standardized questionnaire called "Aggression of Patients towards Nurses in the Course of Their Profession (Lepiešová et al., 2015) ". This survey technique involved nurses from internal and surgical departments. Additionally, third research method was used, and it involved qualitative research through in-depth interviews with nursing care managers. Results: The results indicated that the most frequently reported event to the National portal of Reporting Adverse Events was category called "pressure ulcers ". The results of the qualitative study showed that patient aggression was not related to the field of care (p= 0.587). Furthermore, the results revealed that nurses with longer clinical experience encountered aggression from patients more frequently (p< 0,001). The results of the qualitative study described the most common preventive measures to minimize the risk of patient aggression towards nurses, including training and education of nurses and proper reporting of adverse events, addressing staff shortages, use of restraining measures and pharmacological interventions and appropriate communication with patients and their families. The hospital management prepares for the risk of patient aggression through staff training. Conclusion: The practical outcome of this diploma thesis is a set of recommendations for non-medical healthcare personnel.
Aggression in patiens with psychosis

Most psychiatric patients do not behave aggressively, but they very often become victims of aggressive behaviour. However, in the practical part of this work, we focused on the phenomenon of aggressive behaviour in psychotic patients, and we address victimization of psychotic patients in the theoretical part because it is one of the most important predictors of aggressive behaviour not only in psychiatric patients but also in the general population. In the literature, we encounter three basic immediate causes of aggressive behaviour - psychotic, impulsive, and predatory (planned) - which we observed in hospital conditions. Specifically, we focused on impulsive behaviour in aggressive patients, given that it is assumed that psychotic patients have difficulties in this area. The aim of this study was to examine the immediate causes of aggressive behaviour and provide data from the Czech Republic on their prevalence. Bullying of teachers by pupils and solution options

The bachelor thesis deals with the issue of bullying of teachers by pupils. This is a significant problem, but one that has often received insufficient attention in the literature. Nowadays, it is becoming even more relevant, and therefore this phenomenon has been discussed more frequently. The present paper is a literature search based on original empirical research on teacher-pupil bullying. The aim of the thesis is to provide an insight into the issues surrounding this form of bullying and to summarise the basic knowledge associated with it. The thesis also looks at possible solutions. The main purpose of the literature search is to sort out information about the frequency of bullying and its causes. Understanding the reasons may help to reduce the incidence of this phenomenon. The last part is devoted to possible solutions. Approaches that could lead to a reduction or complete elimination of bullying.
Challenging behaviour in pupils with intellectual disability and autism

The diploma thesis deals with challenging behaviour in pupils with intellectual disability and autism. The aim of the thesis is to map the current reality of education of these pupils through the statements of pedagogical staff from pre-schools and elementary schools established in accordance with section 16, paragraph 9 of the Education Act. The occurrence, frequency, intensity and specific manifestations of challenging behaviour are examined, as well as the underlying causes influencing challenging behaviour, prevention and preventative approaches, solutions to the occurrence and escalation. The theoretical part provides a comprehensive overview of the problem and challenging behaviour in our country and in the Anglo-Saxon world and it deals with approaches to challenging behaviour. It also focuses on autism, neurodiversity and intellectual disability. The research part defines the aim of the thesis, research questions and methodology. The research is carried qualitatively using semi-structured interviews. The results of the research draw on substantial statements of fourteen members of pedagogical staff. The use of restraints in internal branch departments in Hospitals of the South Bohemia Region

This Bachelor thesis is both theoretical and practical. The first part of the theory focuses on an aggressive patient, the causes of aggression, expression, prevention, treatment and access to an aggressive patient. It also mentions aggression in hospitals. In the second part, the theme is restrictive devices, which are often used precisely in aggressive patients. It discusses in more detail the types of restraints, the principles in their use, the laws governing them, the documentation and the nursing care of the patient under restriction. Research goals: In this bachelor's thesis, 3 goals were set. The first was to map the nurses' experience with restrictive devices in practice. The second aim was to explore the nurses' attitude to these means. The third aim was to identify what shortcomings nurses saw in the issue of the use of restraints. Research questions: Three research questions were identified on the objectives of the work. 1. What experience do general nurses have in the use of restraints? 2. What is the general nurse's position on restraints? 3. Do general nurses perceive any shortcomings in the use of restraints? Methods: A qualitative research strategy was used in the practical part. The data collection took place through semi-structured interviews conducted with general nurses in the internal departments of hospitals of the South Bohemia Region. The selection of informace was deliberate, with a total of 12. Results: Using the pencil paper method of data analysis, we created 3 categories of questions and 21 subcategories. All nurses have experience of using curtsies and medication from their practice. The frequency of their use varies according to the ward they work in. Almost half of the nurses had a bad experience concerning the improper use of restrictive medication. Nurses' attitudes are slightly more positive positive mainly due to patient and staff protection. From the nurses' perspective, the positives outweigh the negatives when it comes to restrictive measures. Half of the nurses expressed that they were aware of the shortcomings of this issue. The most frequently mentioned was the lack of availability of orderlies or security guards on the ward.
Impacts on men's mental health when limiting their personal freedom- from the perspective of people working with this target group

The diploma thesis deals with the impact on men's mental health when personal freedom is restricted from the point of view of people working with this target group. It is based on the results of the research "Psychological aspects of the consequences of criminal activity in convicted men serving a prison sentence" from 2019, conducted by Mgr. Marie Ouředníková. This work thus shows a view of life in custody or prison from the other side of the bars. It thus reveals the rules that prevail here among men who face the strict regime of the prison every day, but also the internal hierarchy itself. Such an environment subsequently negatively affects the mental health of the men who have to deal with it, in different ways. The diploma thesis reflects these impacts and thus gives a comprehensive view of the mentioned issue. It also has up-to-date research and statistics pointing to current events in such an environment, such as institutions restricting personal freedom. The diploma thesis is divided into a theoretical and a practical part. In the practical part, there is a description of the research methodology itself, followed by Ouředníková's reflected results with the opinions of people with professional experience in this practice.
Growing up in dysfunctional family

The diploma's thesis focuses on the topic of growing up in dysfunctional families with a focus on aggressive and auto-aggressive behavior. The thesis' aim is to describe the dysfunctional family and problem behavior based on professional sources and then to find out the impact of growing up in a dysfunctional family on an individual's life (especially in the area of problem behavior) with the help of research. The thesis introduces the basic concepts related to the family and the dysfunctional family, mentions the functions and types of the family, the importance of the development of the attachment and further defines problem behavior. The practical part focuses on the description of growing up in a dysfunctional family by individual respondents. In the practical part, qualitative research is used. A semi-structured interview is used for data collection. The data is processed using the thematic coding method.
De-escalation of the patient's aggressive behaviour in a psychiatric ward

The bachelor's thesis "De-escalation of aggressive patient behavior in the psychiatric ward" deals with the issue of aggression, its prevention and effective solution. The aim of the work was to determine the frequency of aggression, its type, cause and method of solution. Another aim of the work was to find out the frequency of trainings that the respondents completed and their interest in a possible offer of training focused on managing aggression. The theoretical part of the thesis provides a basic overview of the issue, a description of de- escalation techniques and other options for managing aggression and its prevention. The empirical part of the work presents the results of own research and their comparison with the results of other professional research. The method of the survey was a self-designed questionnaire, which was distributed to general and practical nurses of the Psychiatric Hospital Bohnice. 108 duly completed questionnaires were included in the study itself. Results: It was found that more than 58% of respondents experience verbal aggression at least once a week and almost 29% of respondents experience brachial aggression at least once a week. Medical and sanitary staff reaction on physical attack in a health-care institution

This bachelor thesis deals with the matter of physical attack on medical personnel in health-care institution, especially on the emergency unit. Medical staff can get into confrontation with pacient, who can be influenced by drugs, alcohol, serious mental illness, or by difficult life situation, which evoke conflict behavior. This confrontation then can be dangerous for safety of health workers, but also their patients. Theoretical part will be based on characteristics of violence, agression and theoretical methodology during encounter with agressive pacient. Study define violence and it´s categories. It also shows resolving methods already escalated confrontation. In practical part study searches and compares aggression resolution between two regional and two district hospitals. It mainly identifies, which techniques health-care institution workers use in order to prevent physical attack from patient, then how health-care workers react to it and also how often do they encounter verbal and physical aggression during their shift. For this research the qualitative technique, along side with inquiring method and in-depth conversation will be used, whereas the research sample is going to be medical staff on the emergency department i.e. nurses and paramedics. Result of the study shows, that health-care workers are in contact with verbal aggression almost every workshift, with physical then only occasionally, eventhough the number of cases have grown. Result of thesis tells, that there is bigger number of cases in district hospitals, than,logically, in regional hospitals, especially because of higher number of patients in district hospitals. But in percentage sign it could be told, that the number is same, no matter the location. The more the health-care personnel are in contact with aggressive behavior, the more they had to adapt to it. And so if they notice, that the situation cannot be deescalated, they will rather call for security service of the hospital, or for the police, so they would not have to risk their own, or the patients' health. But primarily they are trying to handle the situation with principles of correct communication. Unsettling discovery is, that patients' conflict behavior is for hospital personnel on daily basis and it is no longer suprising for them. Prevalence It seems, that for society agrressive behavior became standart way of how to achieve it's goals. There should be enough attention for society to realise, that aggressive Aggression prevalence in hospital instituon points to more komplex society problem, where aggressive behavior is more and more seen as legitimate way, of achieving goals. Study is warning before aggressive prevalence in healt-care institutions. Solution of this problem stands on cooperation of several pillars. Hospital headquarters should provide enough safety for their workers and patients from aggressors and also organise educational programs about correct approach towards aggressive people a eventually elementary courses of self-defence, if needed. These results should also alarm media companies, which should aim for primary prevention a should actively draw attention and convey people about that there is no place for aggression in health-care instition without legitimate reason, because resolution of conflicts is exactly what holds off personnel from providing quality and effective health-care.
