The Effect of Functional Electrical Stimulation Applied by the WalkAide Neurostimulator on Balance and Gait in Patients with Multiple Sclerosis

Author: Bc. Veronika Maršíčková Title: The Effect of Functional Electrical Stimulation Applied by the WalkAide Neurostimulator on Balance and Gait in Patients with Multiple Sclerosis. Objectives: The main aim of this diploma thesis is an assessment of therapeutic effect of functional electrical stimulation applied with using of the neurostimulator WalkAide on balance and gait of people with multiple sclerosis against standard outpatient physiotherapy. Partial aims are comparisons the immediate and persistent therapeutic effect of functional electrical stimulation. Methods: The thesis is a randomized controlled trial. For its realization it was necessary to create an intervention and a control group of probands. Both groups received 2 different rehabilitation programs. The intervention group received the WalkAide neurostimulator (for 2 months) for home use. The control group underwent a standard outpatient physiotherapeutic program with application of 20 therapies (at least) lasting 60 minutes (i.e. 2-3 times a week for 2 months). Data collection was performed by validated motor tests and questionnaires, which were focussed on endurance, speed and skilfulness of gait, static and dynamic balance and subjective perception of balance, overal mobility and pathological fatigue. Comparison of electrostimulation systems WalkAide and NESS L300 in therapy of foot drop in patients after stroke.

This bachelor thesis consists of a theoretical and a practical part. The theoretical part provides a brief introduction into problematics of disability of stroke patients and mainly focuses on gait impairment, specifically on foot drop syndrome that affects about 20% of all stroke patients. Following section contains characterization of functional electrical stimulation and its application to the peroneal nerve, which is used in therapy of foot drop. The theoretical part is then concluded by neat description of two peroneal stimulators that are currently available in Czech Republic, systems WalkAide® and NESS L300®. The practical part includes therapy records of two stroke patients, in which both mentioned devices were used, and filled questionnaires that were directed to representatives of distributor companies. These documets focus on financial problematics of systems in question. The outcome of the practical part is a comprehensive overview that summarizes the use of both stimulators in therapy of foot drop and their accessibility for patients in Czech Republic.
