Harmonization of Clinical Content in EHR: HL7 Messaging vs. OpenEHR Approach
Nagy, Miroslav
This paper sums up the last stage of the project called ”Harmonization of Clinical Content in Electronic Health Record (EHR)”, which is being solved as author’s PhD project. The aim of this project was the clinical content modeling of EHR and transportation of medical data via communication standards (HL7 v3 and CEN EN 13606). An experiment was conducted where archetypes and openEHR templates were used to send patient’s information to another EHR-S. The results were compared to the previous solution based on HL7 v3 messages, which was described at the PhD Conference ’09. At the end of the paper the author discusses contemporary semantics description of real clinical data by means of ICD-10 code-list, which covers the qualitative point of view quite well. However, the quantitative view on semantics of clinical data is much more difficult and the inevitability of medical ontologies usage is emphasized.

Electronic Health Records as an Instrument to Potentially Optimize the Work Flow in Small Medical Practices
Bannach, Anne-Kathleen ; Král, Petr (advisor) ; Cook, Gina (referee)
This master thesis contributes to a growing body of literature analysing electronic health records (EHRs) and their importance to potentially optimization of the work flow in small medical practices. Additionally, general information about different health care systems is given, especially about the Canadian health care system. It gives a theoretical overview of information technologies used through health care providers. This thesis addresses the new technologies for communication and describes them. Problem: The "lack of clear channels of communication in patient transfer between care facilities leads to fragmentation in care." To prevent this, new communication channels need to be more effective in improving communication. Before trying to enhance communication channels used between care facilities, locally and nationally, the current focus needs to be on communication channels used within care facilities. This is of great importance especially for patients and other stakeholders who deem it as the most critical health care item. The slow implementation of EHRs in small medical practices makes it difficult to improve and evaluate performance and to ensure the confidence of patients in new technologies. On the other hand, the lack of implementation in this area has a negative effect on other health care providers, e.g. hospitals which already implemented the system. Through the existing gap the delivery of health care information is not complete possible and does not help to make the system safer for users. Research Question: The main goal is to obtain an understanding of individual physicians' attitudes and barriers to EHR. The thesis will show the advantages and obstacles as well as the pros and cons for small- and medium-sized practices to adopt EHRs. Result: It is not necessary anymore to discuss if EHRs should be implemented. It is more important to discuss how they should implement. The main problem for implementation is the financial barrier for small medical practices as well as personal attitudes connected with the age distribution of physicians and patients.
Data Security in Biomedicine
Horňáková, Anna
This thesis analyzes current state of use of biometrics in computer security. It provides an overview of the most commonly used anatomical-physiological and behavioral biometric identification methods. The result of the work will be a new set of methods, which allows reliable identification of the user in the most comfortable way. These new principles of data security will be used to enhance the protection of specialized health record. This will contribute to expansion of generally conceived EHR MUDR concept to other application areas.

Aplikované informační technologie pro vývoj kontinuální sdílené zdravotní péče
Nagy, Miroslav ; Hanzlíček, Petr ; Dioszegi, Matěj ; Zvárová, Jana ; Přečková, Petra ; Seidl, L. ; Zvára, K. ; Bureš, V. ; Šubrt, D.
This article describes procedures of development of an electronic health record for shared healthcare which include implementation of communication standard HL7 v.3, its application in the environment of existing hospital information systems (HIS) and modeling the semantics of the transferred data. The main part of the solution is so called HL7 broker that serves as a mediator in the communication between the two incorporated systems and implements procedures defined in the HL7 v.3 standard. Data models which represent the communicating systems towards the HL7 broker are based on the original data models implemented in HISes and are in the proper form, demanded by the HL7 standard. In order to achieve the semantic interoperability of incorporated system the creation of mapping of existing data models to international nomenclatures was necessary. Finally the possibilities of usage of international standards and nomenclatures in comparison to the national ones are discussed.

Synchronizace datových modelů v elektronickém zdravotním záznamu MUDRlite
Nagy, Miroslav
Medical knowledge is evolving very fast. Even EHR systems that are built using best practices vary in their data models (DM) over time. There exist software tools for “DM synchronizations” (DMS) on the market. Within this paper we describe special requirements and put these tools to the test. Further we present a new open source DMS tool Schemagic that was tested during dental medicine knowledge formalization using MUDRlite EHR. Finally, we state how Schemagic multiplies the power and scalability of EHR.

Využití komponent systému MUDRLite při sdílení dat elektronického zdravotního záznamu v stomatologii.
Špidlen, Josef ; Pieš, Martin ; Teuberová, Z. ; Nagy, Miroslav ; Hanzlíček, Petr ; Zvárová, Jana ; Dostálová, T.
The goal of the project named Information Technologies for Shared Health Care is the improvement of possibilities of sharing data between hospital information systems (HIS) and electronic health record (EHR) applications. Fulfilling it led to development of a pilot EHR system called MUDRLite EHR, that provides interfaces to include user-defined components. We applied these interfaces to a high-advanced component representing the dental cross - a crucial part of healthcare documentation in dental medicine.
Flexibilita datových modelů v aplikaci MUDRLite
Nagy, Miroslav ; Špidlen, Josef
MUDRLite application was developed within the research in the EuroMISE Centre. It connects the user interface described in XML file with data layer. The structure of data stored in the database is not fixed. It is designed to fulfil needs of the target application. The data structure is called ER-model and in a real database it is stored as a database schema. Sometimes it is necessary to slightly modify the ER-model. In the process of data model evolution a database schema synchronising tool is very useful.
Elektronický zdravotní záznam v kontinuální sdílené zdravotní péči
Hanzlíček, Petr ; Ďurovec, Ján ; Nagy, Miroslav ; Zvárová, Jana ; Špidlen, Josef
The project"Information technologies for development of continuous shared health care" deals with the development of the methods and technologies helping to ensure the continuous shared health care by creation of the unified system of electronic health documentation including the use of chip cards and security aspects. The technological solution will be based on the results of previous research in EuroMISE Centre, results of European research and international standards of health informatics.
