The first work, which has looked at medication errors was published in the nineties of the 20th century by the American Institute of Medicine "To Err is Human: Building a Safer Health System's" issued in November 1999. The report is based on analysis of several different studies, concluding that 44.000 to 98.000 people die each year because of preventable drug errors. In comparison, fewer than 50.000 people died from Alzheimer's disease and 17.000 died of illicit drug use in the same year. Each administration of the drug to the patient is a kind of experiment, which rely on available population data of pharmacokinetic and pharmacodynamic properties of drugs. Today patients are more than ever informed about their disease and the available and appropriate treatment, due to the high volume of information that can be acquired through the Internet. The traditional role of pharmacists was the provision of medicines to the population. However, nowadays, this role has been changed so as to include other tasks, such as management of prescribed medicines, and promotion of health, since pharmacists can serve as a source of expertise knowledge to the customers. The uniqueness of the study flows from the characterisitics of analysed patients'group with sorted serious blood malignities requiring bone marrow transplantation. Moreover the thesis' aim is to judge, whether the correctness, accuracy, frequency of the administration and the sufficiency of the used daily dosages of the antimicrobial drugs in the cliníc was in order. The script also addresses to the field of combined antimicrobial treatment. It also scans, which drugs are administered in the hospitál department and under what circumstances and those empirical data compares with the guidelines. This diploma work observes as well the popularity of administered medicaments and examines their usage during two investigated years. Analysis of treatment of patients with epilepsy dispenzarized in University Thomayer's Hospital in Prague Student: Frýdlová D. Tutor: Vlček J., Tlustá E. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Kralove, Charles University in Prague, Czech Republic Background: Studies on antiepileptic drug utilization are important for evaluation of epilepsy treatment and risk of antiepileptic drugs (AEDs). Aim: To describe group of patients with epilepsy, AEDs utilization patterns and adverse drug reactions (ADRs) of AEDs. Methods: Data were retrieved from medical records in University Thomayer's Hospital in period 2005 - 2006. Epileptic outpatients (n = 97) aged over 16 years were considered for analysis. Results: The age ranged from 17 to 72 years (mean 55), 62 % were women. Partial seizures were diagnosed in 62 % of patients. 31 % of patients were more then 2 years without seizures, 39 % of patients had more then 12 seizures per year. More then half (65 %) of the patients were treated with polytherapy. Lamotrigin was the most commonly prescribed AED (20 %), followed by valproate (18 %) and carbamazepine (17 %). Title of rigorous Thesis: Analysis of consumption of antibiotics in patients treated with dialysis Introduction and objective: This study observes consumption of antibiotics in patients treated with dialysis in The University Hospital of Hradec Králové in years 2006 and 2007. Pilot project investigate, which kind of antibiotics used dialyzed patients (DP) the most of all. Studies further compare information from two sets of DP from Czech Republic (Group I) and Lithuania (Group II) in specific aspects of antibiotic's consumption. Methodology: Retrospective analysis resulting from data evaluation from Nephris database of University Hospital of Hradec Králové by DP (2006&2007). Set of data of the Group II from Clinics of Kaunas Medical University - Lithuania was process by retrospective analysis too. GLMZ mode was used for statistical analysis. Results: Group I: from average number of 125 hospitalized patients were treated by antibiotics 71 (57%) patients. Knowledge of patients with diabetes mellitus about hypoglycaemia.

Nowadays, diabetes mellitus belongs to wide-spread diseases. 4-7% of population suffer from diabetes in developed countries and the number is increasing. Education of patients with diabetes mellitus ensures effective treatment and it contributes to the delay of diabetic complications. Therapy itself without correct and effective patient education can hardly lead to successful treatment. One of the frequent diabetic complications is hypoglycaemia, which can be fatal due to patient ignorance. The aim of this pilot study was to evaluate the degree of patient knowledge about hypoglycaemia, to monitor its distribution among patients and to analyze its possible risk factors. Cross-sectional observational study was used for the analysis. Data were obtained from structured direct interviews carried out among diabetic patients. The study was performed in Faculty hospital in Hradec Králové. Submission of patients into the study was random. Patients with diabetes mellitus types 1. and 2. visiting the diabetologic centre in Faculty hospital in Hradec Králové were included. Information was gained from 100 patients (109 addressed (92 %)). Background: Medication errors or drug related problems (DRPs) have a negative impact on the whole process of health care and are significant health risk for a patient. Aim of study: The aim of this study was to describe and evaluate the role of pharmacist in identification and solution of prescribing errors. And to discuss the possibilities of the further development of safe medication practices in the Czech republic. Methods: 33 pharmacists from community and hospital pharmacies participated in a study. Data were collected by web questionnaire for a period of 15 days. The following DRPs were collected: incorrect indication of drugs or contraindicated drugs, prescribing of drug for incorrect patient, wrongly prescribed name of drug, contraindications, duplications of therapy, potential drug interactions or problems with dosage (e.g. overdosing).
