The contribution of echocardiography in patients with cardiac resynchronization therapy

Introduction Chronic heart failure (CHF) is the subject of increased interest in the medical community, because of the high number of affected persons and also with regard to the resulting socio-economic impacts. Understanding the relationship between echocardiography and cardiac resynchronization therapy will allow better identification of potential responders and non-responders, which is very important because of the relatively high costs of this treatment. Objective Based on our own experience and comparison with literature data to evaluate the importance of echocardiography in cardiac resynchronization therapy (CRT). Methods One day before implantation, and at 3 and 15 months after implantation, we assessed: NYHA class, quality of life (QoL) and the 6-minute walk test (6MWT). In addition to a standard echocardiography, we assessed the presence of ventricular dyssynchrony and right ventricular (RV) systolic function by tricuspid annular plane systolic excursion and by pulsed tissue Doppler imaging (myocardial peak systolic velocity was measured at the tricuspid annulus). Atrial Fibrillation - Morphological and Electrophysiological Changes of the Atrial Myocardium

Atrial fibrillation - morphological and electrophysiological changes of the atrial myocardium Atrial fibrillation (AF), the most common sustained cardiac arrhythmia, was considered as pure functional disorder for long time. In recent years, there were identified atrial locations, which are involved in the initiation and maintenance of this arrhythmia. These structural changes, so called remodelation, start at electric level and later they affect contractility and morphology. We attempted to find a possible relation between morphological (scarring, isolated atrial amyloidosis (IAA), left atrial (LA) enlargement) and electrophysiological (ECG features) changes in patients with AF. We examined grossly and histologically 70 hearts of necropsy patients - 44 with a history of AF (subclassified into two subgroups: PERM - patients with permanent or persistent AF - 20, and PAROX - patients with paroxysmal and newly diagnosed AF - 24), and 26 control cases without any history of AF. Premortem ECGs were evaluated. The patients with AF had significantly heavier hearts, larger LAs, more advanced fibrosis in atrial myocardium and more severe amyloidosis in both atria. Financial Stability Transparency and Interest Rates Petr Pařízek Abstract This paper examines the relationship between financial stability transparency and nominal interest rates on a panel data with more than 50 countries in years 2000 to 2010, controlling for macroeconomic conditions. We investi- gate the same relationship using monetary policy transparency, we compare the results and based on the existing literature we check for non-linear ef- fects of transparency. Moreover, we examine how this relationship changes in financially good and bad times in terms of financial stress. We find nega- tive relationship between both financial stability and monetary policy trans- parency and short-term interest rates. Especially in the case of monetary policy transparency our results suggest non-linear relationship and the exis- tence of an intermediate optimal level of transparency. For financially good and bad times our findings are miscellaneous, some in accordance and some in contradiction with the literature. Generally, our results suggest that the effect of financial stability transparency on short-term interest rates is weaker than the effect of monetary policy transparency. Determinants of Claims Satisfaction in Insolvency Proceedings in the Czech Republic

This paper examines determinants of creditors' claims satisfaction in insolvency proceedings in the Czech Republic. To our knowledge, it is the first research to such extent in the Czech Republic covering this field. Combining microdata from Insolvency Register, Business Register, Ministry of Finance and other sources, we construct a unique dataset of more than 2,600 cases. We identify several basic determinants of satisfaction: a higher share of secured claims, real estate and cash in assets of a company, submission of financial statements into Business Register, selling the business as a whole within the proceedings, and entrepreneur cases. We find no such effect for the audit of financial statements or for the age of a company. Moreover, we search for other indicators that may result in the lower satisfaction of claims, out of which five are statistically significant: a homeless person in statutory body, registered office at a firm nest, being listed as unreliable VAT payer, a connection to persons that have multiple records in Insolvency Register and a substantial increase in depreciation in the period between the last two submitted financial statements.
