Implementation of Electronic Health Record for Shared Healthcare using International Standards and Nomenclatures

Authors deal with development procedures of shared electronic health record (EHR) which consist of implementation of communication standard HL7 and its deployment among existing hospital information systems (HIS); and semantics modeling of transferred data. Main part of the solution are so called HL7 brokers which arrange communication among incorporated systems and each of them implements methods defined by the HL7 v.3 standard. Data models that represent communicating systems are used by the HL7 brokers and are based on the original data models of HIS, but comply with all restrictions defined in the HL7 standard. To achieve semantic interoperability it was necessary to create a mapping of existing data models to particular international nomenclature. The conclusion contains discussion about situation in the Czech Republic and how there are used international standards in comparison with established national standards.

This article describes procedures of development of an electronic health record for shared healthcare which include implementation of communication standard HL7 v.3, its application in the environment of existing hospital information systems (HIS) and modeling the semantics of the transferred data. The main part of the solution is so called HL7 broker that serves as a mediator in the communication between the two incorporated systems and implements procedures defined in the HL7 v.3 standard. Data models which represent the communicating systems towards the HL7 broker are based on the original data models implemented in HISes and are in the proper form, demanded by the HL7 standard. In order to achieve the semantic interoperability of incorporated system the creation of mapping of existing data models to international nomenclatures was necessary. Finally the possibilities of usage of international standards and nomenclatures in comparison to the national ones are discussed.

Synchronizace datových modelů v elektronickém zdravotním záznamu MUDRlite

Medical knowledge is evolving very fast. Even EHR systems that are built using best practices vary in their data models (DM) over time. There exist software tools for “DM synchronizations” (DMS) on the market. Within this paper we describe special requirements and put these tools to the test. Further we present a new open source DMS tool Schemagic that was tested during dental medicine knowledge formalization using MUDRlite EHR. Finally, we state how Schemagic multiplies the power and scalability of EHR.

The goal of the project named Information Technologies for Shared Health Care is the improvement of possibilities of sharing data between hospital information systems (HIS) and electronic health record (EHR) applications. Fulfilling it led to development of a pilot EHR system called MUDRLite EHR, that provides interfaces to include user-defined components. We applied these interfaces to a high-advanced component representing the dental cross - a crucial part of healthcare documentation in dental medicine.
Flexibilita datových modelů v aplikaci MUDRLite

MUDRLite application was developed within the research in the EuroMISE Centre. It connects the user interface described in XML file with data layer. The structure of data stored in the database is not fixed. It is designed to fulfil needs of the target application. The data structure is called ER-model and in a real database it is stored as a database schema. Sometimes it is necessary to slightly modify the ER-model. In the process of data model evolution a database schema synchronising tool is very useful.

The project"Information technologies for development of continuous shared health care" deals with the development of the methods and technologies helping to ensure the continuous shared health care by creation of the unified system of electronic health documentation including the use of chip cards and security aspects. The technological solution will be based on the results of previous research in EuroMISE Centre, results of European research and international standards of health informatics.
PKCS Standards for Medical Documentation Purposes

The paper reviews usage of external cryptographic devices for purposes of electronic medical documentation. Advantages of such devices include high protection of private keys, i.e., a private key is generated inside the device and can never be removed. Communication standards, such as PKCS or MS CAPI, should be used to maintain independency on a particular cryptographic device. The paper manly focuses on PKCS #11 and its usability in the project “Information technologies for continuous shared health care”.
