This bachelor thesis is focused on nutritional of patients with diabetes mellitus of the second type. The theoretical part defines type 2 diabetes mellitus. The incidence of type 2 diabetes mellitus has been increasing in our country and all over the world. Although the genetic predisposition is necessary for the development of diabetes, we can prevent this disease. The obesity as well as the other components of the metabolic syndrome prevention is very important in type 2 diabetes prevention. Physical activity is also very important, as it has a significant effect on insulin sensitivity and the dietary influences, which this bachelor thesis particularly deals with. The practical part is based on the survey, which contains the results of a questionnaire survey carried out at the 13rd International day of diabetes mellitus on Charles square in Prague. This part of the work shows how are their eating habits and activities. Keywords: diabetes mellitus, type 2 diabetes mellitus, diabetes, insulin resistance, obesity, metabolic syndrome, dietary prevention
Long-term effects of short-term low calorie and very low calorie diets
Čmerdová, Kristýna ; Matoulek, Martin (advisor) ; Mikeš, Ondřej (referee)
Starting points: LCD and VLCD are one of the possibilities, which can be used when battling obesity. They are not however advised separately. There must always be present another form of treatment. What follows is either bariatric treatment, or a home reduction plan in cooperation with a nutrition therapist. LCD or VLCD should always be under the supervision of a doctor. Goals: To, in detail, describe a complex of patients, who were on weight reduction hospital stay (later only WRHS), as a foundation of a statistical analysis of dynamic changes concerning the reasoning of LCD and VLCD hospitalization, basic anthropometric data and deduce the short and long-term effect of WRHS with LCD and VLCD. Methods: Entry data were collected retroactively from hospital records of patients who were hospitalized for reduction of weight in the years 2014-2015 in inpatient ward D3 of the third Internal clinic of the General University Hospital in Prague (Všeobecná fakultní nemocnice v Praze). This data was statistically processed and its results discussed and charted in this paper. Main results: The most common reasoning for WRHS was sole reduction of weight; next was bariatric operation after WRHS. Patients who didn't undergo bariatric operation within a year of their 1.st WRHS, and their weight after one year is...
Assessment of the Resting Energy Expenditure in Obese Individuals
Sadílková, Aneta ; Matoulek, Martin (advisor) ; Vilikus, Zdeněk (referee)
Introduction: Weight loss programs are based on inducing a negative energy balance by decreasing energy intake while increasing energy expenditure. Energy needs of obese patients are difficult to determine. The value of resting energy expenditure, as the largest component of the energy needs, is usually calculated to asses an adequate energy intake for obese patient. In clinical practice, predictive equations are most commonly used for this purpose, because of their simplicity, although many clinical studies have shown that in obese individuals they may generate errors large enough to impact the outcome of treatment. It leads to the prescription of an inadequate diet that does not meet the criterion of long-term sustainability. Objective: The aim of the thesis is to compare the values of resting energy expenditure (REE) measured by indirect calorimetry with values calculated with Harris-Benedict predictive equation in a selected group of obese individuals. Methods: The sample includes 38 subjects, 28 women and 10 men, patients of the General University Hospital in Prague. The average age is 48 ± 11,71 years, the mean BMI is 42,88 ± 9,09 kg/m2 . For each subject, resting energy expenditure was measured by indirect calorimetry and calculated by the Harris-Benedict predictive equation. Indirect...
Monitoring of energy intake and expenditure during reducing stays
Pražanová, Ivana ; Matoulek, Martin (advisor) ; Piecha, Roman (referee)
Obezita představuje ve vyspělých zemích závažný zdravotní problém a hlavní rizikový faktor rozvoje kardiovaskulárních onemocnění, cukrovky 2. typu, nemocí pohybového aparátu a některých nádorových onemocnění. Pacienti průměrně zredukovali hmotnost o 7,6 kg (SD ±3,54). Bioimpedanční metoda vykazuje příliš vysokou variabilitu a není...
Individual nutritional intervention for patients with diabetes mellitus type 2.
Bohnerová, Beáta ; Matoulek, Martin (advisor) ; Váchová, Alena (referee)
The goal of this diploma thesis is to evaluate changes in the body structure, selected laboratory values and blood pressure for patients with prediabetes or diabetes mellitus of 2nd type who attended nutritional therapy in diabetic ambulance. The patients were divided in 2 groups based on their preferences. The first monitored group monitored energy intakes and expenditures on web page www.kaloricketabulky.cz. The first group had also an opportunity to be supervised or to communicate with its nutritional therapist on web page www.casprozdravi.cz. The second monitored group monitored energy intakes and expenditures manually in the diabetic notebook. The body changes were monitored by bioelectric impedance scale Tanita BC-545 N during each consultation. Laboratory values were taken on consultation 1 and 5. The blood pressure was measured on consultation 1 and 5. Rated values - overall body weight, waistline, percentage of body fat, glycemia, HbA1c, overall cholesterol, HDL cholesterol, LDL cholesterol, triglycerides and blood pressure. The results of this research were weight reduction in both groups by an average of 3.2 %, waistline by 4.5 %, body fat by 1.7 %, glycemia by 16 %, HbA1C by 28.6 %, overall cholesterol by 15.1 %, HDL cholesterol by 6.5 %, LDL cholesterol by 23.6 %, triglycerides by...

The results: Data analysis revealed that respondents with psychiatric disease show more type 2 diabetes mellitus, heart disease and high blood pressure (89%) in the family and personal history. The results also showed that these individuals are more frequent smokers. Of the selected respondents, 46% were smokers, whilst 60% were smokers in the control group. According to 51% of respondents, overeating and according to 57%, little movement has the biggest impact on weight gain. In food preferences, psychiatric respondents reported the highest representation (62%) of vegetables and fruits. In...
Monitoring of blood glucose level in patient with type 1 diabetes in specific situations
Patková, Barbora ; Matoulek, Martin (advisor) ; Vařeka, Tomáš (referee)
Introduction: Type 1 diabetes mellitus is an autoimmune disease, which often disrupts normal regime during the life of the patient. During the day there are fluctuations in blood glycaemia levels, so a very important part of therapy is so called self-monitoring, in which the individual is continually monitors his own blood sugar level. Using self-monitoring of blood glycaemia can demonstrably improve diabetes control. With the advent of new technology, such as sensors, insulin pumps and others patients are, in their cooperation, enable to optimize the regime during normal life. Objectives: Monitor glycaemia at patients with type 1 diabetes mellitus in the last trimester of pregnancy and after childbirth. Determine the change in HbA1c during pregnancy and after childbirth, compared to previous years. Identify, describe and evaluate situations and errors of the patient that affect the blood glucose fluctuations and monitor insulin requirments. Furthermore, closely monitor three days before and two days after childbirth, depending on diet and physical activity. Methods: Data were collected using records from sensors in the program CareLink® Personal of company Medtronic, from diet regimen of program Kalorické tabulky and exercise activity from program Evito.cz and informations about HbA1c were...
