Nursing care of a patients after a scrotal hernia surgical repair

This thesis is concerned in a case study of a patient with a scrotal hernia. The patient is Mr. J. M. in age of 82 years, who arrived on the surgery department for a planned surgery of a scrotal hernia. In clinical part, I am concerned in an anatomy of a groin and a scrotum, general characteristics and pathophysiology of hernia, its clinical screen and diagnosis. Following with a consequent surgery healing, possible complications and a general prognosis of the disease. This part also contains medical anamnesis of the patient, a progression of the hospitalization and healing. In the nursing part, I use the Functional health model by Marjory Gordon. I got information about patient health from his health documentation, also by talking with the patient, his family, doctors, nurses and other personnels. I summarized the information to specify 5 actual and 3 potential diagnosis. The order was given by the priorities of the patient with respect to his actual health state. I provided a nursing care plan for solving individual problems. In this part, I am also concerned in progression of the hospitalization and an evaluation of a nursing care in a long-term plan. Nursing care of the patient with type 2 diabetes and with the diabetic foot syndrom

Cílem práce je zpracování případové studie u šedesátisedmiletého pacienta P. F., s diagnózou diabetes mellitus 2. typu, včetně chronických komplikací (neuropatie a syndromu diabetické nohy), který byl hospitalizován na diabetologickém oddělení v únoru, roku 2010. Pacienta jsem ošetřovala po dobu pěti dnů (z celkového počtu osmi dní hospitalizace). Po skončení pobytu v nemocnici byl klient propuštěn do domácího ošetřování. V klinické části jsem začala s historií tohoto onemocnění, následovaly stručné informace o anatomii a fyziologii pankreatu, poté jsem přistoupila k charakteristice diabetu, klinickým projevům, klasifikaci, diagnostice a k základním možnostem léčby. V závěru klinické části jsem se zaměřila na komplikace akutní i chronické, u kterých jsem zvýšenou pozornost věnovala syndromu diabetické nohy. V ošetřovatelské části mi byl předlohou model Funkčního zdraví od Marjory Gordonové. Informace o pacientovi jsem získala rozhovorem s nemocným, prostřednictvím zdravotnické dokumentace a komunikací s ostatními členy zdravotnického týmu. Vše jsem následně zaznamenávala do ošetřovatelské dokumentace, která je ve formě přílohy umístěna v závěrečné části mé bakalářské práce. Nursing care about a patient with diagnosis - diabetes mellitus

In my thesis I am writing about a patient with diagnosis diabetes mellitus 2nd type, who was hospitalized in internal departments for the overall deterioration and decompensation of the disease. I cared about the patient during my practice, on above-mentioned department. The aim of this paper is case study in a patient with diabetes mellitus, 2nd type. In the clinical part of my work I deals with the history, characteristics, anatomy and physiology of diabetes mellitus. His clinical picture, diagnostic tests, therapeutic procedures. Describe acute and chronic complications of this disease. The nursing part of my job I deal with a patient and use the methods of the nursing process. In gathering information and assessing the patient come out of the model Marjory Gordon. Substantial information was recorded in nursing documentation, which I put as part of my work. Analyzing all the information gathered, I have established the current nursing issues and to identify nursing diagnoses that I have lined up according to the importance of the solution. For each diagnosis, I also drew up a plan of nursing care interventions with the implementation and evaluation activities.
