Coronary atherosclerosis imaging using CT coronary angiography in patients with high risk of coronary heart disease
Baxa, Jan ; Ferda, Jiří (advisor) ; Vymazal, Josef (referee) ; Mechl, Marek (referee)
Coronary atherosclerosis imaging using CT coronary angiography in patients with high risk of coronary heart disease Coronary heart disease (CHD) belongs to most frequent disorders and in industrial countries is responsible for most deaths in population. The costs of treatment of the clinical manifested CHD are high, so there is tendency to detect this disease in early phase and ideally before development of clinical symptoms. Aim of our study was to evaluate the value of coronary CT angiography (CCTA) in detecting of coronary atherosclerosis in early (preclinical) phase. Our goals were estimation of prevalence of atherosclerosis in group, influence of main risk factors on grade and character of atherosclerotic changes. In follow-up we concerned on occurrence of cardiac events. 205 patients with increased cardiovascular risk underwent CCTA using dual-source CT. Coronary findings were analyzed by character of atherosclerotic changes and degree of event. stenosis. Statistical evaluation of influence of risk factors was made using the odds ratio. The statistically important differences were tested between subgroup of type 2 diabetics and non-diabetics. All patients were under follow-up for two years. Overall prevalence of atherosclerosis in analyzed group was 73 %, calcified lesions were found in 63.9 %...
Regional differentiation of quality of housing and human capital in the Liberecký Region at the beginning of the 21st century
Baxa, Jan ; Chromý, Pavel (advisor) ; Poštolka, Václav (referee)
The main goal is to analyse the quality of housing and human capital in the Liberecky kraj in the beginning of 21st century. The survey gets on constructed aggregate indexes, which aspire to refer important spatial differentiations in quality of housing and human capital measured on municipalities and microregions. Aggregate indexes of the quality of housing rely on own housing quality definition gets out of the interdisciplinarity and complexity this topic, whereas definition and indicators of the quality of human capital are assumed from other publications. In the thesis are two main methodologies based on collecting, analyse and valuation the statistical data on the one hand and preparing, execution and valuation on the other hand. The thesis conduces to second housing, peripheries, rural areas and housing policy research. The influence of private and public institutions on housing development is emphasized and the research deals with new trends of housing in the interest area. The results inhere in approving high housing quality out of the central areas, to these days very intensively built-up. The place and price of housing are the most important factors. 10
Prostate Cancer Diagnostic Algorithm
Sedláčková, Hana ; Hora, Milan (advisor) ; Baxa, Jan (referee) ; Záleský, Miroslav (referee)
Prostate cancer diagnostic algorithm Aim: The aim of the study is to implement the latest scientific knowledge in the diagnosis of prostate cancer (PC). We focused on tumor markers, imaging methods, prostate biopsy methodology and we created a diagnostic algorithm based on a review of current literature in combination with our own experience. Material and methods: The algorithm is divided into several branches, which have been individually subjected to clinical studies. Due to the low sensitivity and specificity of PSA, prostate health index (PHI) was added to the first line of patient stratification. 787 patients were primarily examined and these subsequently underwent radical prostatectomy. PHI levels were compared with definitive staging and grading. Cut-off values for PC detection and high-risk stratification, including locally advanced PC were determined. Next, 320 patients underwent prostate biopsy followed by radical prostatectomy. The cohort was further divided into two subgroups, patients with GS = 6 and patients with GS > 6. The ability of PHI to distinguish between insignificant and significant prostate cancer was evaluated. In a multicentric study with 395 patients, PHI with additional markers (tPSA, PSAD) and multiparametric magnetic resonance imaging of prostate (mpMRI) was assessed....
Functional Imaging Methods of Lung Carcinoma
Baráková, Táňa ; Baxa, Jan (advisor) ; Eliáš, Pavel (referee) ; Čtvrtlík, Filip (referee)
Topic: Functional imaging methods of lung cancer MUDr. Táňa Baráková, Klinika zobrazovacích metod FN Plzeň Introduction: The aim of my work was to assess the use of direct quantification of dual-energy CT (DE-CT) radiation in the diagnosis and monitoring of non-small cell lung cancer (NSCLC) therapy. The work consists of three studies that were performed on Department of imaging methods. The main objectives were to assess the contribution of iodine quantification from DE-CT examinations, to verify the setting and to incorporate the DE-CT examination of the chest into a routine investigation. Furthermore, to investigate the possible benefit of dual- phase DE-CT in evaluating both primary non-small cell lung cancer itself and lymph nodes in terms of iodine and size relationship in chemotherapy or targeted therapy, and finally to investigate the possible benefit of staging FDG-PET / CT to evaluate the effect of therapy on lymph nodes. Methodics: In all presented studies, the same DE-CT protocol was used in the early (arterial) and late (venous) stages. Using a dedicated software prototype, iodine content analysis was performed as the equivalent of blood circulation in lymph nodes and tumors. The main evaluated parameters were iodine content (mg), iodine uptake (mg / ml) and ratio of arterial enhancement...
Geographical approaches to assessing the quality of housing in Czechia
Baxa, Jan
Choice of housing is one of the most important decisions of inhabitants. It is a relatively new process in Czechia which did not work absolutely freely till 1989.Inhabitants accustom to choice of housing and learn this process which is based on the relatively short experience. On the one hand, the importance of the choice of housing is conditional by low willingness of the Czech inhabitants to move and on the other hand, it is amplified by the high purchase price, which usually means the greatest investment in life of households. Thus, people consider many aspects in the wish to satisfy one of the basic life needs during the choice of housing. The place of future residence is one of the most important of them. Housing research is a relatively new topic in geography. Even though systematic research on housing and quality of housing in Czechia takes place more intensively (in the social sciences, for more than two decades), there were not performed territorial differentiation of the quality of housing assessment and comparison of municipalities according to suitability for. The concept of quality of housing has appeared almost exclusively in connection with the dwelling yet, the basic unit of housing. Housing permeates all basic areas of human and social life. Therefore the evaluation of the quality...
Optimization of Exposure Values in Computed Tomography. Perfusion CT of the Brain
Bejček, Jiří ; Baxa, Jan (advisor) ; Čtvrtlík, Filip (referee) ; Weichet, Jiří (referee)
Topic: Optimization of exposure values in computed tomography MUDr. Jiří Bejček, Department of Imaging Methods, FH Pilsen Introduction: Stroke is one of the most frequent diseases in the human population, the second most frequent cause of death in economically developed countries. Early diagnosis and rapid treatment play a key role. Computed tomography can be used to diagnose stroke and select optimal therapy for the patient. However, rising CT scans increase the radiation exposure of the population. Aim: The aim of this work was to analyze the stroke protocol used in the Department of Imaging Methods in Pilsen, which is used in the diagnosis of acute cerebral ischemia. In addition, the aim was to design a modification of the protocol in order to reduce the radiation exposure, maintain the diagnostic quality of the image for evaluation and verify the feasibility in daily routine. Methods: The circulation times of the original (test) protocol were analyzed retrospectively. Based on this analysis, a shortened protocol was set according to test bolus performed in CT angiography. 134 patients were examined with this protocol. Subsequently, a reduction of the voltage to 70 kV was performed and 108 patients were examined. Further protocol adjustments were made according to the width of test bolus curve. 38...
