Surgical treatment of mortal complications of acute myocardial infarction as basis of secondary ischemic heart disease prevention
Kačer, Petr ; Adámková, Věra (advisor) ; Táborský, Miloš (referee) ; Velemínský, Miloš (referee)
Cardiovascular disease (CVD) represents the leading cause of death in the Czech Republic. The principles of the secondary prevention of CVD are well-known, precisely elaborated and highly effective. Myocardial infarction may be accompanied with mechanical complications. One of the later is a post-infarction rupture of the free wall of the left ventricle (FWR) resulting in high mortality rates during hospitalization (60-90%) and surgery (11.8-61%). Due to the rarity of the diagnosis, the surgical treatment and its secondary preventive effect on the patients' life have not been described sufficiently. The aim of the work presented was to describe and evaluate the methods of surgical treatment of FWR as a basis of prevention of secondary FWR by means of analyzing a set of FWR patients (comprising two groups as per the actual FWR form) concentrated on the patients' performance evaluated by the ejection fraction (EF) of the left ventricle (LV) and self-sufficiency, and suggesting an optimum FWR treatment strategy emphasizing the function preservation of LV - the tertiary prevention of the rupture of the free wall of the LV. The study comprises a set of 19 patients, i.e. 11 (57.9 %) males and 8 (42.1 %) females, who underwent surgery in the IKEM Heart Center between 01. 01. 2006 and 31. 12. 2012. The RLK...
Changes of systolic blood pressure figures in dependance on measuring point, body position and the level of physical load among girls from 11 to 13 years
Pavlů, Regina ; Bartůňková, Staša (advisor) ; Kohlíková, Eva (referee) ; Velemínský, Miloš (referee)
Thesis is a pilot study to get reference figures for evaluation of relevancy of hemodynamic development disorders of coartation of the aorta. 18 healthy girls aged from 11 to 13 were tested. In total 450 measurements were carried out and 2700 figures were measured within 1 year in 5 positions. Blood pressure was measured on arteria brachialis and femoralis, supplemented by electric impedance technology on lower limb. The following positions were measured: standstill lying down on a bed, stand after passive verticality up to 66 degrees, standstill seat on a bicycle ergometer and load of 1W/kg and 2W/kg of body weight for 3 minutes. This thesis confirmed the differences in circulation reactions. The systolic pressure gradient among healthy population was lower than among patients with coarctation of aorta and during the physical load was decreasing. The results are important for diagnostics examinations of evolutionary defects. Keyvords: BMI, coarctation of aorta, systolic blood pressure, pressure gradient, load testing
Content of macronutrients and salt in the nutrition of toddlers
Radomská, Věra ; Velemínský, Miloš (advisor) ; Stránský, Miroslav (referee)
The present diploma thesis is engaged in the content of macronutrients and salt in the diet of toddlers. The theoretical part examines the characteristics of the toddler age, delineating the physiological and psychosocial development of toddlers. In addition, the theoretical part analyses in detail the individual components of a healthy diet, fluid intake and technological procedures of meal preparation. The theoretical part further provides the characterization of correct and incorrect eating habits. The objective of the research part is to determine the knowledge level of parents with respect to a healthy diet of toddlers; another objective is to ascertain the intake of macronutrients and salt on individual days and a four-day average, based on the programme Nutriservis Professional. The third objective is to compare results relating the intake of macronutrients and salt with DACH recommendations, binding in the Czech Republic. On the basis of the evaluation of our results, as the fourth objective, recommendations for parents as to the diet of toddlers will be prepared. The research part was completed under the application of quantitative and qualitative research methods. The first part of the research was conducted by form of a questionnaire survey. The other part was carried out by making...
