PROBLEMS OF PATIENT RESOCIALIZATION IN PSYCHIATRIC HOSPITAL ČERVENÝ DVŮR

This bachelor thesis called The Issues of Rehabilitation of the Psychiatric Hospital Cerveny Dvur patients deals with both the rehabilitation of the Cerveny Dvur patients and system of the rehabilitation care provided after the stay in this hospital. The thesis is divided into two parts, a theoretical and practical one. The theoretical part contains basic concepts concerning the issues of rehabilitation, specific problems during the rehabilitation which can appear after the return to a normal life, the issues of relapse or recurrence and finally the information about the psychiatric hospital Cerveny Dvur where the research was realized. The main goal of the practical part was to chart the rehabilition of the Cerveny Dvur patients with the emphasis on sub-goals such as the system of the care provided after the stay in this hospital as well as the reasons for the abstinence violation. There were three research issues defined which were focused on the functioning of the aftercare, risk factors during the rehabilitation and the most frequent reasons for the abstinence violation. The research itself was carried out qualitatively since the data were obtained by the method of semi-structured interviewing. Seven patients and three therapists of Cerveny Dvur took part in it and the research was realized just at this hospital. During the discussion I was comparing the results of this research with professional publications. Finally I establish hypothesis and give some recommendations to solve the problems resulting from the research. I think that it is necessary to devote more time to these issues and also not to underestimate the aftercare.
The doctor and nurse cooperation in the center for patients undergoing a principal joint aloplasty

Motoric system diseases belong to the most frequent ones in the present days. All age categories are affected. Congenital defects are included and also injuries, inflammations, degenerative illnesses and others. In the Czech republic are many centers gathering patients with the same diseases. However the centers for orthopedic patients are missing. The dissertation is divided into a theoretic and empiric parts. The theoretic one is focused generally on problems with joint replacements, rehabilitation and relation between a medic and a patient. The dissertation was targeted on: 1. To verify if the informing about the subsequent postop nursing care provided in the center for patients undergoing aloplasty comes up to a patient?s expectations and fulfils their needs. 2. To find out if hospital nurses perceive the difference in the patient engagement in the nursing care according to if he had visited the center before or not. 3. To reveal potential problems in the center running from the patient, nurse and doctor point of view. The research part of the dissertation was implemented with a combination of both qualitative and quantitative survey. For quantitative survey was chosen a method of questioning with a form technique for patients (after a hip or a knee aloplasty) in the Czech republic and Austria and also for nurses working in the orthopedic department and intensive care unit in the Pelhřimov hospital. Three hypotheses were established. 1. The given nursing care informations provided in the center for patients undergoing aloplasties fulfil the patient?s needs. 2. The hospital nurses assess a significant difference in the patient?s collaboration between those who had visited the center before and who had not. 3. In the center for patients waiting for aloplasty daily running there exist problems from the nurse?s point of view. As for the qualitative part of survey a method of questioning with the technique of a semistructured dialog with open questions was used. The research file was created by patients after principal joints aloplasties. In this part we have tried to answer three main questions. 1. What is the relationship between both hospital and the center nurse activities and the patient?s contentment in the sphere of his needs? 2. What sorts of informations for the patients in the center are missing? 3. The patients evaluate the informations provided in the center as orientational or as significant for easier postop care managing? The results of the research survey were analysed and taken in schemes. Patients suffer from disturbing their both physical and psychological needs and so the goal of collaboration is a satisfied and fully informed patient. From the results it is apparent that a nurse in the center closely cooperates with a hospital nurse. The survey revealed that the patients in the center are missing no informations. The patients are prepared for the operation, orient themselves in the related problems know what comes with the rehabilitation as an important part of the postop care. To be concluded: patients are prepared for easier postop care managing. On the basis of analysis of respondent?s answers it was found that the patients undergoing a principal joint aloplasty are sufficiently informed. Although hospital nurses and doctors sense the difference between a patient who had visited the center before and who had not the stimulus for subsequent survey could be how the patients perceive the differentness of info provided within the center and out of it. The survey results could be a stimulus for establishing new centers not only within Vysočina but also in Austria. In the relation with the dissertation results and on the basis of info provided in the center when the patients assessed the info as sufficient for them an informing prospectus was created. The dissertation could serve as an informational material for students not only in the nursing profession.
The effects of mastektomie on social life of women with breast cancer

Breast carcinoma is at present one of the most frequent diseases of women which affects all areas of woman ´s life and becomes all-society problem. It is an oncology disease which brings a woman and her close relatives a huge psychical and physical stress. One of the possibilities to cure the breast cancer is the chirurgical removal of the whole breast (mastectomy), which is a huge change not only in woman ´s social life. The thesis gives information on breast carcinoma, risk factors for the onset of the disease, clinical symptoms, diagnostics and the treatment of breast carcinoma. Further, it describes the possibilities of reconstruction therapy, rehabilitation and psycho- social effects of cancer. The objective of the thesis was to find out if and how mastectomy influences the social life of women. For the data elaboration the method of quantitative research by means of questioning was used. The questionnaires were addressed to women with breast carcinoma after mastectomy. The research group was created by the clients of General Faculty Hospital in Prague. The questionnaires were anonymous and voluntary, in the form of 25 closed and semi-open questions. Hypothesis H1: negative influence of mastectomy depends on the partner ´s attitude and the family support, was proved. Hypothesis H2: mastectomy negatively influences single women and women without children, was also proved. Hypothesis H3: women after mastectomy are of worsened occupational use, was disproved. The research has shown that the question of solving social issue in women after mastectomy is more and more up- to-date. Average age of respondents is 51 - 60 years. In the question concerning the change of life in women after mastectomy were most frequently mentioned the problems with the change in dressing, sport activities and psychical problems. The awareness of women on rehabilitation, reconstruction operations, possibilities of using epithesis and also on the civic partnerships focused on breast diseases should improve.
Application of the ICF classification for a possible inclusion of mentally retarded persons into society

The objective of this dissertation thesis is the application of the International Classification of Functioning, Disability and Health (ICF) with mentally retarded persons, or the examination of ways of application of its data for a possible inclusion of mentally retarded persons into society. The thesis deals with the system of the ICF classification, its application within the framework of research as well as the subsequent possible application in practice when reviewing the situation of disabled persons.The results of the thesis bring a recommendation how it is possible to proceed when reviewing the situation of a mentally retarded client in practice and how to use the ICF classification as a tool of support of psychosocial inclusion of these persons.
Japanese Internal Influences on Foreign Policy after WWII

This dissertation focuses on Japanese policy as a whole, internal and external policy priorities and objectives, in order to illustrate the role, and analyze the level of effectiveness of conducted foreign policy tools. This paper explores some of the reasons and factors of conducted internal and external policy tools. Therefore, it shows how effective and in timely manner reaction of Japanese to state's internal policy, has influenced and played a key role during the country recovery and restoration. Moreover, proper policy objectives not only have restored the country but also made Japan one of the most advanced world centers. Japan is in a number of the advanced countries of the world, participating in modern world political process which dynamics has the general laws and the tendencies of development connected with transformation of the Pacific Rim into the center of global activity of the leading countries of the world. Changes in global scale, aftermath of the World War II affect the significant characteristic of foreign policy, official and economic diplomacy of Japan towards peace and security advocating country.
On Some Aspects of the Relationship between Girls Placed in the Educational Institute Počátky and Their Parents

The thesis deals with the relations of girls placed in the Educational Institute Počátky with their parents. The theoretical part describes the legislative foundations of the existence of educational institutes in the Czech Republic, psychosocial characteristics of girls placed in educational institutions, and especially the pedagogical procedures applied when working with girls placed in educational institutions and with their parents. The practical part is dedicated to the following two research questions: 1st question: What are the relationships between the girls placed in the Educational Institute Počátky and their parents? 2nd question: How do these relationships affect the possibility of reintegration of girls into the society? Only 30% of the respondents shows the positive relationships with parents, without signs of emotional deprivation and negative influence on its reintegration into society. The remaining 70% is confirmed by many studies. It is apparent that for isolated children need for love and belonging is the long run unsatisfactory.
Comparison of nursing care about apallic patients in domestic and institutional care from carer?s viewpoint

As a result of urgent medicine acceleration and neurosurgery care improvement individuals that had minimum chance of life saving in the past survive brain insults. Increasing occurrence of people with apallic syndrome is a consequence of this progress. This thesis compares domestic and institutional care of apallic people. It examines whether the care quality and complexity are comparable. As the domestic care is more economical for the government it has to be supported in the times of insufficient funds for health care. Quantitative research technique of non-standardized interview was used for data collection. The research sample consisted of six healthcare workers and six family members caring for apallic patients. One respondent had a son in institutional care, however she took him temporarily to domestic care. Questions for healthcare staff and for caring family members were prepared separately. The results show that apallic patients in domestic care achieve substantially better results. The physical load is comparable, mental load is higher for family members. The basal stimulation, which is extremely important for apallic patients, is purposefully applied in institutional care only, as it is provided by trained staff there. The results of this work were presented at the 4th National Student Conference organized by the Faculty of Healthcare Studies of the Pardubice University. Distribution of this work among people having individuals with insulted brain around is the best way how to highlight the urgency of the problem.
Comprehensive rehabilitation of children with infantile cerebral paralysis up to three years.

The baccalaureate thesis describes a complex rehabilitation system in the care of children with suspected diagnosis of poliomyelitis (PM) or with diagnose PM under 3 years of age. The thesis is divided in two major parts. The first part is theoretic and the second one focuses on practical application. Chapter 1 characterizes PM and presents PM as combined disability including psychological issues of children with PM. The first part furthermore presents the Tudory of individua rehabilitation means as an integrated system including necessary early care. The practical part highlights theadvantages and principles of good functioning of early care, while mapping the situation of clients in their families. This part also presents case studies of children with PM. The basic research instruments included semi-standardized interview with the patente and participant Professional, observation and analysis of documents dutiny consultations.
Promoting of self-sufficiency by hemiplegic patients after a brain stroke

Nowadays, cerebral apoplexy is no longer a disease that only attacks elderly people, but increasingly, persons at a productive age. The consequences of this didease are diverse and vary according to the affected person, both in extent and severity. This is why this disease fundamentally changes the way of life of both the diseased and their families.The teoretical part of my bachelor thesis is focussed on the disease itself. The reader of this thesis will learn more about the nature and the substance of this disease. This thesis describes the types of cerebral apoplexy, the risk factors, the diagnosis and the methods of treatment. If further describes the princilpes of methods for nursing people suffering cerebral apoplexy,emphasising their self-suffuciency, rehabilitation a rehabilitation targets.The socond part of my bachelor thesis was research work. The applied method was based on a survey in which two types of questionnaries were deployed, one for respondents working in inpatient wards in medical facilities and one for respomdents working as home carers. This part focussed on acquiring information and discovering differences in providing support to hemiplegic patients after cerebral apoplexy to help them to becouse self-suffucient in personal hygiene and to collaborate wiht a physiotherapist, both in an inpatient ward and during home care. The result should contribute to better nursing care provided to patients after cerebral apoplexy.
Physiotherapy after total hip joint replacement

Pain in a hip joint can dramatically affect the chance of having active life. The recent years have seen a significant progress in the results achieved while implanting artificial hip joints. This type of operation is simultaneously becoming increasingly more often especially when we take into consideration the increasing age of population. Implanting total hip joint replacement results in improving the quality of life, which can only be achieved through interdisciplinary collaboration of doctors, complex rehabilitation treatment, and positive approach of the subject alone. The aim of this bachelor?s thesis was to process case reports of patients after total hip joint replacements and to put together rehabilitation plans for these subjects. With the established objective in mind, the author summarizes outcomes dealing with the issues relating to total hip joint replacement, physiotherapy, occupational therapy, and spa treatment following such operation. The research part of the bachelor?s thesis was accomplished by qualitative research. Data were collected by observing, interviewing and analysing documents. The data were gathered by individual in-depth interview and using case reports, which included author?s own observations, a draft of rehabilitation plan and implementation of the therapy, and subsequent outcomes from the assessment of therapy. The research corpus comprised of two patients who underwent a planned total hip joint replacement because they had been suffering from advanced primary coxarthrosis. The research was carried out at an in-patient orthopaedic department of the Hospital Pelhřimov in March 2011, when the subjects were being hospitalised. Quality and appropriately managed rehabilitation allows quicker improvement in the patient?s health following the operation. It is used as a prevention of complications, it improves engagement of functional potential in the operated joint and it consolidates appropriate motion stereotypes. The outcomes of the research confirm that such pre-operational and post-operational rehabilitation carried out during the stay in hospital leads to a decrease or complete removal of pain, an increase in joint?s mobility and muscular power, getting used to walking on flat surfaces and up and down staircases, and the level of independency in everyday activities.
