The possibility of pain management in prehospital emergency care
GUIDA, Mario
I chose the topic Possibilities of pain management in pre-hospital emergency care for my bachelor thesis as pain belongs to the most frequent symptoms of patients in the care of the Emergency Medical Service. The goal was to map the possibilities of pain management in pre-hospital emergency care in South Bohemian Region. The second goal was to map the awareness of pain management in pre-hospital emergency care among South Bohemian medical rescuers. The theoretical part deals with the present situation of the Emergency Medical Service, education and competences of rescuers. In the next part I focused on various types of pain, assessment and subsequent treatment of pain. The last, theoretical part describes the caring process in pre-hospital emergency care for a patient in pain. The research was performed by means of quantitative method in the form of a questionnaire with semi open and closed questions. The questionnaires were distributed at Emergency Medical Service regional centres of South Bohemia. The results show that pain is sufficiently managed in pre-hospital care. Another fact was confirmed, that rescuers are familiar with the pain management possibilities and prefer pharmacological methods of pain management. We also found out that non pharmacological interventions were used less frequently than they could be. One of the hypotheses was refuted, two were confirmed. The quantitative research results show that there is still a relatively large reserve in using non pharmacological methods. Rescuers might use them more, particularly the quick emergency crews, or they might be applied in situations not requiring immediate pharmacological treatment. Educational seminars at the individual workplaces might improve the awareness of non pharmacological intervention techniques. A patient in pre hospital emergency care suffering from pain of various intensity should be given primary professional care from qualified rescuers.
Monitoring of gas exchange as a part of technical equipment of Emergency Medical Service vehicles
KADLEC, Jan
The name of the bachelor degree thesis is "Monitoring of gas exchange as a part of technical equipment of Emergency Medical Service vehicles." The objective of my thesis was to map the knowledge of paramedici working for the Emergency Medical Service (EMS) of the South Bohemian region about the work with pulse oxymetr and kapnometr. The second objective was to identify or to compare possibilities of measurement of levels of cabroxyhemoglobine in blood. I would like my bachelor degree thesis to be used as a teaching material about pulse oxymetry, capnometry and cooxymetry for paramedics. The results of the research might serve as an impulse for the management of EMS of the South Bohemian region to make the user training for the devices more consistent, comprehensive and particularly uniform for all participants. One interesting finding was that the paramedics have a relatively good knowledge of the pulse cooxymetry which is in contrast with the fact that the method is not available for them to work with it.
Motivation of choosing a career as a paramedic
PETRŮ, Michal
The expression ``motivation`` describes the fact that the human psyche is affected by internal driving forces, both conscious and subconscious. Holland´s vocational personality typology presents six basic types, namely Realistic, Investigative, Artistic, Social, Enterprising and Conventional. This thesis is aimed at finding and evaluating motivation factors which influence prospective emergency medical technicians in their professional choice. To get the research data, the combination of quantitative and qualitative techniques was used. The research was conducted among emergency medical technician students at the college and university and, as a supportive research material, among Czech and Scottish Emergency Medical Service professionals. The students´ primary motivating factor influencing their vocational choice to become an emergency medical technician is their interest in this field, sometimes supported by a healthcare worker in the family. Over 80% respondents prefer working in the field which they study. An emergency medical technician belongs to the most popular positions while an emergency dispatcher to the less popular ones. The fact that women respondents like working positions connected with driving a car was a surprising discovery of the research. Corresponding to the recent professional literature, the most frequent career types among professionals and students are Realistic and Social types. This thesis can contribute to further research into this topic (working position preferences, career types). Component findings can be introduced into personnel and human resources management of the Emergency Medical Service, especially searching for Investigative and Conventional career types of workers and their subsequent engagement in pedagogical and administrative work. The research can also be helpful in other issues, e.g. how to make a position of an emergency dispatcher more popular.
The organization of emergency medical treatment with intention to the medicine of disasters and mass catastrophes
CHALOUPEK, Milan
The medicine of disasters and mass catastrophes is an interdisciplinary medical specialism exploiting scientific discoveries made and lessons taught by experience in the other branches of medicine while attending to emergencies and carrying out rescue, demolition and sanitation operations. Practices already applied to handle mass catastrophes are being analyzed in case the humankind has to face similar events in the future. Related to the analyses are requirements for organizing urgent care. To illustrate the types of cooperation; to exemplify organization of the urgent care on specific cases; to compare the ways of cooperation between health-care facilities and the constituent elements of the Integrated Rescue System (IRS); and to cope with large-scale disasters, the mass catastrophes occurring in the Czech Republic have been treated to an analysis. Evaluated in the relevant cases was the level of security that the Czech Republic can rely on in case of emergencies - catastrophes and major disasters. Comparison was made between what was done by large health-care facilities and what was taken care of by the Integrated Rescue System, with special focus on the triage of the victims. As follows from analyses of different mass disasters, the medical care provided to save lives is satisfactory, and the urgent care is organized properly. The information on organizing urgent care oriented towards the medicine of disasters and mass catastrophes has been extracted from a variety of sources and through inquiring competent professionals.
South Bohemian inhabitants opinion of activity of basic bodies of Integrated Rescue System
DUŠKOVÁ, Ilona
The main objective of this study was to identify and evaluate the opinion of the South Bohemian region inhabitants on activities of the Integrated Rescue System basic components. The objective of the research was to carry out an analysis of data obtained through the questionnaire survey and evaluate the outcomes showing the view of the South Bohemian region inhabitants of activities of the Fire and Rescue Service, the Emergency Medical Service of the South Bohemian region and the Police of the Czech Republic.
Possibilities of artificial pulmonary ventilation in urgent care and anesteziological resuscitation department
BINTEROVÁ, Jana
The aim was to explore the possibilities of mechanical ventilation in prehospital emergency care and Anesthesiology and Intensive Care qualitative research method. The first part of the research was focused on finding out how the use, including the most frequently used modes of ventilation, and frequency of artificial ventilation. Research has found that in pre-hospital care are used methods of invasive and noninvasive ventilation and ventilatory modes most widely used in prehospital care are IPPV, CPAP and SIMV. In the second part of the research focuses on the possibility of mechanical ventilation on Anesthesiology-Intensive Care and neužívanějších detection schemes. They are IPPV, BIPAP, SIMV, APRV, PS and CPAP. The last part of the research deals mainly comparing the possibilities of mechanical ventilation at the base emergency medical services of the Liberec Region and South Region. I ran into very different views on the importance of mechanical ventilation in prehospital emergency care, but the result is the possibility of artificial ventilation in different regions does not differ much.
The most frequent causes of death in emergency medicine
GRECHOVÁ, Michaela
From the general point of view, cardiovascular diseases are considered to be the most common causes of death in developed countries. Diseases of the arteries of the brain also rank among serious diseases and are a common reason of not only death but also invalidity. Although the region of South Bohemia is behind the nationwide average, the number of deaths related to diseases of the circulatory system is very high and remains a frequent cause of intervention of the emergency medical service. The objective of my thesis on the theme "The Most Frequent Causes of Death in the Acute Medicine" was surveying the Region of South Bohemia in terms of the most frequent diagnoses resulting in daeth, in the period between 2006 and 2007, and comparing them with the statistics. The basic document to obtain general information was the Annual Medical Bulletin of the Region of South Bohemia. This information constituted the basis onwhich I drew up three hypotheses that were then verified by the actual research. In the practical part of my thesis I conducted the quantitative research by the method of the document ontent analysis and comparison. The set studied consisted of patients of the Emergency Medical Service in the years 2006 and 2007 with the NACA score = 7.One of three hypotheses was verified, two were not.
Problems of the psychiatric patients-direction by means of emergency medical service and alternative possibilities
PETRŮ, Michal
Treatment, direction, transport and placing psychiatric patients in hospitals have been proved as the most problematic activity of Emergency Medical Service (EMS). The range of health facilities is limited. Psychiatric cases belong to the most difficult diagnoses which ambulance crew can meet. Even if they can profit from the attendance of a patient family members they work under time pressure and cannot prolong the treatment of the patient as there can be other casualties in emergency. Non-indicated admission to mental hospital can harm the patient. On the other hand, omitting professional psychiatric intervention of indicated patient can cause his death (depression, suicidal tendency). The aim of this work consists in the analysis of community care for psychiatric patients within EMS compared to its potential and suggestions of possible ways in their alternative placing in the framework of labelling prevention. The attitude of EMS staff to psychiatric patients and their knowledge of community treatment were under observation. The students of a study course of Paramedic were chosen as a reference group. The thesis is based on quantitative study, via questionnaire technique. The potential of community treatment use within EMS is low but it exists. Respondent comments on the necessity of helpline teams which could assist relatives in case of sudden death (especially children) were inspiring. Some research outputs are surprising, e.g. hardly any negative attitude to psychiatric patients. Valuable asset of this work lies in the fact that all findings can be implemented into EMS and can also be used in further research into this problematics, e.g. how much the emergency dispatch uses helpline system or how often an ambulance crew contacts crisis centers.
Crisis measures in anonymous annoncement of a bomb explosion in Římov Dam
MIHALIČ, Petr
The thesis deals with activities of components of the Integrated Emergency Management System of the Czech republic in the event of an anonymous bomb threat on the dam of Římov reservoir and evaluates preparedness and professional conduct of individual fundamental components of the Integrated Emergency Management System (IEMS) in this emergency incident. The content of the thesis is based on findings obtained by reading up available materials and information about actions of components of the IEMS in the course of individual bomb threats from the past. The objective of the thesis is to analyse the problem of an anonymous bomb threat, to assess preparedness of individual components of the IEMS to manage the situation in the event of anonymous bomb threat on the dam of Římov reservoir and to suggest possible procedures of the IEMS as a response to this criminal activity. Evaluation of preparedness of components of the IEMS is performed by questionnaire research the results of which are presented in charts. I also describe possible scenarios of this emergency incident and a scenario of the response of components of the IEMS, possibly resulting in state of emergency. In the last part of the thesis I evaluate preparedness of the Integrated Emergency Management System components in this emergency incident and I point out possible shortcomings which could occur in reality and which resulted from the analysis.
Crisis management after the use of so called dirty bomb, focused on integrated emergency system operation, especially Czech Police organ
VILDMAN, Radek
The main goals of this dissertation were to discover wheather there exists some methodology for actions of the integrated emergency system in this king of emergency and to create planning for and responding to the emergency. This work includes a simple instrument for basic orientation for members of the Police of the Czech republic and members of other emergency services that may deal with the emergency arising after an explosion of a dirty bomb. The work presents a concrete model situation of the above mentioned emergency and the function of each emergency service, especially the Police of the Czech republic and description of their work, with the references to present legislation. This work points out the differences between theory and practice, which emerged after thorough examination of the single actions and facilities of each part of the integrated emergency system. It also contains suggestions for future solution.
