The position of nurses in internal expert outpatients{\crq} departments.

This bachelor thesis deals with the position of nurses in internal expert outpatients{\crq} departments. The current position of nurses in the internal outpatients{\crq} departments is special and demanding. We cannot imagine how the health care service would work without them. An outpatients{\crq} nurse has several tasks that she should master and carry out smoothly {--} it is the role of an educator, an attorney, a manager, a provider of health care, a mentor, a researcher and a carrier of all changes. The relation between the doctor and the nurse is also highly important; it should be friendly, considerate and mutually respectful, and their cooperation should be based on equal rights. Internal outpatients{\crq} department provide health care to patients with various diseases of internal organs. Nurses are a significant part of the medical team. The departments serve as places of the first contact of the client with the medical staff. Professional approach and communication with the patient are expected. A quality medical and nursing care is unthinkable without communication between the client and the nurse. Patients vary in their behaviour and reactions but certain manifestations of behaviour are common to certain groups of people. The objective of my work is to find out the position of nurses in selected internal expert outpatients{\crq} departments. In order to fulfil this objective I applied the qualitative research. The method of collection of data consisted in half-structured interviews with 35 standardized questions carried out with nurses in selected internal outpatients{\crq} departments. The interviews were completely anonymous. Three nurses were interviewed in every selected internal outpatients{\crq} department. The whole research was carried out in the Motol University Hospital in Prague. 9 nurses took part in the research (3 nurses from the department of diabetology, 3 nurses from the department of endocrinology and 3 nurses from the rheumatologic department). The interviews with nurses from individual outpatients{\crq} departments were the basis of case histories. Those served as the research groundwork for elaboration of 11 categorizing charts that summarize the most important results of the research. Thanks to our research findings and meeting the objective of our research we found out that the position of nurses in selected expert internal outpatients{\crq} departments is good though financially undervalued. High demands, mainly in the sphere of communication with patients and other medical staff, are placed on them. The research further showed that the nurses mainly work as educators and their job description mostly consists in administrative and nursing work. They have adequate working conditions. They also provide information to patients orally as well as by means of practical demonstrations. On the other hand, nurses in outpatients{\crq} departments suffer from more displays of stress.
Possibilities of transport of children by ambulance service

The topic of the bachelor thesis is ``Possibilities of transport of children by ambulance service.{\crq}q The main objective of the thesis was to determine how the Emergency Medical Service personnel in the Moravian-Silesian and South Bohemian regions ensure safe transport of children patients in ambulances. In the theoretical part I focused on the history of ambulances and beginnings of medical emergency services in this country and also on factors affecting transport of children, observation of labor safety regulations, safe transport of children patients, consequences of car accidents and international procedures and standards for transport in health services. The first hypothesis stated that the personnel of EMS in the South Bohemian and Moravian-Silesian regions are not familiar with principles of safe transport of children to medical facilities and it has not been confirmed. The second hypothesis stated that the personnel of EMS of the South Bohemian and Moravian-Silesian regions do not apply the principles of safe transport of children to medical facilities and it has not been confirmed either. The questionnaire research has confirmed that respondents from both the regions prefer transport of children with the use of a restraint system. The research has also exposed some shortcomings in the safety training for the use of aids which ensure safe transport of children and in some cases also insufficient availability of such aids. Workers of the Emergency Medical Service in both the regions have demonstrated that they are concerned about safety of the transported children, as children are the future or the society. They are able to think about these issues and they are familiar with the principles of safe transport of children into destination medical facilities. This thesis has produced a favorable result to the effect that we can rely on workers of the Emergency Medical Service (EMS) in the Moravian-Silesian and South Bohemian regions at any time and entrust to them our children as they are well-prepared to transport them safely. This bachelor thesis should serve as an impulse to develop an optimized procedure for the personnel of response teams of the Emergency Medical Service (EMS) in the Moravian-Silesian and South Bohemian regions in order to protect children patients during transport into medical facilities and also as an input material to ensure the so-far insufficient aids necessary for safe transport of children by the Emergency Medical Service.
Holistic approach in nephrology out-patient care

This Bachelor´s work deals with the holistic approach to clients of nefrological departments.It would confront the approach of nurses with patients´ expectations. Nefrological patients are people of any age with acute or chronic renal dysfunction which needs special care of medical staff. A nefrological care is a process of planning, evaluating and special care and treatment of nefrological patients. People are cultural and spiritual beings who acquire a mind-body-spirit healing care. A holistic approach helps us get more information of clients and with it meet their mind-bodyspirit needs. The aim of this survey has been to gain information about the care and medical staff approach to patients in nefrological departments, if nurses can see mind-bodyspirit aspects in their patients. The hypotheses and research questions were set like this: Clients are not given a holistic care. The attention is not paid to mind-body-spirit needs of clients. Can nurses see clients of nefrological departments holisticly? The hypotheses assumed at the beginning were not proved. The patients of the nefrological department where I did the survey are treated holisticly and a special attention is paid to meeting mind-body-spirit requirements of the clients. The nurses of these departments can see their patients holisticly. The results of this work are really valuable and beneficial because nefrological departments are basic and important parts in the complex preparation of patients to further therapeutic methods.
The role of a nurse in dermatology and venerology out- patient department

The theme of my bachelor´s work is a nurse´s job in the dermatologic-venerologic outpatients´. Its object was to determine the nurse´s duty in the dermatologic-venerologic outpatients´ and the strenuousness of her work. The chapter of the current situation includes the history and the present of dermatology-venerology and the particularity of the nurse´s treatment of this specialization. It also includes anatomy and epidermis and its other organs physiology. There are analyzed nurse´s individual roles and communication importance, the most frequent nursing operations at that workplace, strenuousness of the nurse´s work, legal problems concerning the dermatology-venerology and prophylaxis of hospital infections. The qualitative investigation was conducted in the form of observation and non-standard conversation that was recorded with dictaphone and than transcribed in seven summarizations.The investigation was conducted in private surgeries of dermatologic-venerologic doctors and in the surgeries with the same problems that are associated with the ward blocks. In the summarizations there is the description of nurse´s duties that can be divided into the nursing and administrative part. There are also the particularities of the nurse´s work in the special surgery, nurse´s cooperation with the doctor and communication with the patient. This work can be used as a groundwork for teaching subject of nursing care in dermatology-venerology. The work could be used for the presentation at nursing courses or workshops.
The role of nurse about information of patients in ambulantion care in Hospital Ceske Budejovice

Communication is an essential part of provided nursing and medical care. Nurses, who are able to communicate efficiently, can easier initiate changes in patients and these changes lead to the support of their health. Patients´ awareness is a current problem and a weak part of medical care. Nurses should provide information clearly, patiently and reasonably with respect to the circumstances and patient{\crq}s state. Despite the fact that the patient spends in the ambulatory department only for a short period of time and the nurse has only limited possibilities. More stress should be put on the content of information, which is provided to the patient after examination or before going home. Changes which would help improve outpatient care regard the whole system of medical care. There are limited possibilities how to solve this problem, but if every nurse were able to communicate correctly with the patient, eventually with his or her relatives, this might help the patient to overcome the first fright, anxiety or uncertainty. The objective of this Bachelor{\crq}s thesis was to find whether the nurse has sufficient time and possibilities to inform the patient according to the needs and current status in the area of ambulatory care. The second objective was to find whether patients feel that they are given necessary amount of information, which is necessary for success of the nursing care. In the research three hypotheses have been set: H1: The nurse in the ambulatory care does not have enough time to educate patient {--} has been verified; H2: The nurse in the ambulatory care does not have sufficient space to educate the patient {--} has been verified; H3: Patients would welcome more additional information from the nurse in the ambulatory department {--} has not been verified. The quantitative research was performed in a form data collection through the questionnaire. The objectives of this thesis were fulfilled. The research was made at Hospital in České Budějovice at ambulatory wards of selected departments. The results can provide information about satisfaction and dissatisfaction of patients and nurses with the current situation to the hospital{\crq}s management and can be used as a support for changes or improvements in particular departments.
Possibilities of managing the situacion when first aid´s crew is attacked

Abstract Currently media more and more often inform about various attacks, violence, criminal acts or aggression. These are basically types of interpersonal relationships with that we live daily. We may observe such negative aspects of human behaviour even on professional level. That concerns in particular activities of so called assistance jobs. Assistant jobs include but are not limited to services of ambulance providing daily pre-hospitalization emergency health care to patients in acute situations. Number of rescuers who were attacked and injured is increasing. Problematic situation at the place of their response is often not suitably solved by ambulance staff and that makes it difficult to provide good quality care to patients. A quantitative research has been conducted within this Bachelor essay. Data was collected based on responses to questions. The quantitative research has been performed in form of a questionnaire. The subjects of research were ambulance rescuers, response nurses, doctors and ambulance drivers of South Bohemian Ambulance. The research has been performed in April 2008. The Bachelor essay includes two sections, one practical and one theoretical. The purpose of the essay was to map the issue of ambulance crew attacks. The objective has been achieved. The following three hypotheses have been formulated. Hypothesis No. 1: Health rescuers feel to be threatened by patients/clients more than ambulance drivers. This hypothesis has not been confirmed. Hypothesis No. 2: Ambulance doctors feel to be threatened by patients/clients less than health rescuers. This hypothesis has not been confirmed. Hypothesis No. 3: Health rescuers have better self defence skills than nurses working at ambulance. This hypothesis has not been confirmed. Results of the above research are to be published at www.zzsjck.cz. The will form a base for implementation of self defence courses in cooperation with Faculty of Health and Social Studies of South Bohemian University in České Budějovice.
Education of a patient with Atopic Dermatitis

The topic of this bachelor´s work is {\clqq}Education of a patient with atopic dermatitis``, and deals with education of patients performed by nurses working at outpatient departments of dermatology. There were objectives and hypotheses determined for the work and then verified with the help of a quality research. As an instrument of the research a questionnaire was used for the patients visiting outpatient departments of dermatology in Vysočina and Středočeský regions. Another instrument of the research used here was a structured interview with nurses as a group of research working at outpatient departments of dermatology in Vysočina and Středočeský regions. The objective of the research was to determine if the patients with atopic dermatitis are satisfied with education of nurses, and to find out the forms of education of the patients with atopic dermatitis. After handing out the questionnaires, information was processed and consequently hypotheses either confirmed or disconfirmed The research showed that the patients are satisfied with nurses education concerning atopic dermatitis. The first hypothesis was verified by the research. We found out that nurses educate the patients in atopic dermatitis in the form of information leaflets. The second hypothesis was verified as well. The research confirmed that the patients search for information about atopic dermatitis in a very active way, which, at the same time, verified the hypothesis number three. This disorder belongs to demanding life situations both emotionally and mentally. A nurse task here is to offer her empathetic and holistic approach and participate in health improvement of the patient. In the course of education, a nurse completes and affects the knowledge, abilities, approach and behaviour of the patient to encourage his own effort and help him this way to improve his health condition.
Using services of the Centres of wet healing of wounds by primary care nurses

Treatment of chronic wounds is a complicated multidisciplinary problem. Modern therapeutic dressings which are permeable and keep the wound wet play an important role in the treatment. At present many professionals strive for support and wider spread of wet wound healing in the Czech Republic. They are mostly nurse organisations, journals, pharmaceutical firms and ``Centres of wet wound healing``. The Centre of Vysočina region is in Jihlava. Centre staff help nurses, doctors and patients to improve the care of wounds. As seen from the results, in spite of all obstacles which nurses have to face while using modern therapeutical dressings the method of wet wound healing in the treatment is spreading. More and more nurses use this method. The Centre of wet wound healing and departments for chronic wounds help towards this effort.
Nurse {--} patient relation in emergency room sector

"Nurse {--} patient relation in emergency room sector{\crq}q was the topic of my thesis. The bachelor thesis was divided in two parts. The theoretical part dealt with questions of those factors which influence the nurse - patient interrelation. The thesis tried to describe the most important factors which effect the establishing of the patient {--} nurse interrelation; it tried to describe nurses{\crq} points of view as well as patients{\crq} points of view because their views can be fundamentally different and in a way they can zoom in some out-patients{\crq} aspects. The length of waiting time for treatments, overall impressions related to their visits in emergency rooms, patients{\crq} characters and the level of emergency room nurses{\crq} stress play significant roles. Two targets were set in the practical part of my thesis. The first target was to map factors which take share in forming a positive client {--} nurse relation in emergency room care. The second target of my thesis was to map factors which take share in forming a positive nurse {--} client relation in emergency room care. Furthermore, five hypotheses were set. The first hypothesis was: ``Waiting time before treatment influences nurse - client relation.{\crq}q The second hypothesis was: ``Women are more willing to cooperate than men.{\crq}q The third hypothesis was: ``The shorter the client{\crq}s waiting time for treatment, the more willing s/he is to cooperate.{\crq}q The fourth hypothesis was: ``Emergency room nurse{\crq}s high stress influences her relation to clients.{\crq}q The fifth hypothesis was: ``A large number of clients waiting for treatment are a strong stress factor for nurses. The qualitative method with the anonymous questionnaire technique was used for the real status discovery. Two kinds of questionnaires were used. The patients{\crq} questionnaire contained 24 questions. The questions were closed ones and half-open ones - for amending of brief answers. The introductory questions were identifying ones. The other questions were focused on positives and negatives which are closely connected with visits in emergency rooms. Nurses{\crq} questionnaire contained 29 questions, which were closed ones and half-open ones - for amending of brief answers and one question was open one - for expressing of their own opinion. The introductory questions in the questionnaires were identifying ones. The other questions were focused on positives and negatives which are connected with providing care to patients who come into emergency rooms for treatments. Treated patients, emergency room nurses and ordination nurses in Písek, Strakonice and Brno were my research population. 200 questionnaires were distributed; 100 questionnaires were distributed to emergency room nurses and 100 questionnaires were distributed to patients who were treated in emergency rooms. Findings were illustrated in diagrams, which reflect the answers of nurses and patients. The discussion was confronted with technical bibliography. In the conclusion there are stated the summarised results of the thesis, corroborated or refuted hypotheses as well as possible suggestions how to influence positively the forming of interrelations between nurses and patients in emergency rooms.
The role of the nurse in the ambulatory therapy of rheumatoid arthritis
