Nutritional care about immobile patient on standard department of internal type

Nutritional care for immobile clients at a standard department of internal type becomes still more important, because nutritional care is important for maintaining good health and supporting quality of life. Nutritional status of immobile clients is very important to diagnose as early as possible in order to avoid all risks of potential complications. Adequate nutrition is also very important to maintain good mental health. The central objective of this work was to explore the specifics of nursing care in the field of nutritional care for immobile client at a standard department of internal type. Following this goal, three research questions were set up: How is the nutritional status of patients at a standard department of internal type assessed? What are the possibilities of application of nutrition for immobile client at a standard department of internal type? What are the specifics of nursing care in the fields of nutrition of immobile client at a standard department of internal type? The empirical part of the thesis was processed using qualitative research, which was realized through semi-structured interview technique. The interview contained 3134 questions, which focused on nutrition care for immobile clients on standard unit of internal type. The research sample consisted of nurses working in the standard department of internal type in the České Budějovice Hospital, a.s. Overall, this study leads to two findings, both positive and negative. Within the positive findings we can say that nurses provide adequate care in the application of nutrition per os. Negative findings relates to the fact that the interviewed nurses provide only basic care in case of the introduction of enteral and parenteral nutrition. They do not know more detailed specifics of enteral and parenteral nutrition. Based on this finding, we think it would be useful to organize regular seminars and trainings for nurses to expand their knowledge about this type of care, or at least remind it. This thesis can serve as an informational material for nurses in practice or students of other health professions. This work can serve also as a basis for further research.

Practical use of theatroterapy in the activation process of adults with intellectual disability

The aim of my bachelor thesis "A Practical Usage of Theatrotherapy During a Stimulation of Adult People with Mental Handicap" was to create, execute and evaluate a theatre performance focusing on a stimulation of mentally disabled adults. During leisure activities, it is possible to offer to mentally disabled people such activities that develop their personalities universally, lead towards their own activity and creativity. However, the mentally disabled people´s selection of activities is limited in comparison with the intact ones. Considering an offer of leisure activities for the target group, I decided to use theatrotherapy which has been created for them and then to implement a theatre performance. According to the specialised literature, by using it, it is possible to reach positive results in the field of communicative and locomotive skills as well as the memory and social relations. This thesis consists of three chapters. The target group will be defined in the first chapter in which I define the term of mental disability, characterise adult people with medium or severe mental disability and specify their social, mental and locomotive limitations in details. I define the theatrotherapy, describe its development, aims and methods in the second chapter. Simultaneously I mention it as a means of stimulation in the conclusion of this chapter. Theoretical part of the work is followed by in the third, practical chapter. I prepare a theatre performance based on specialised literature which I implement and evaluate here. The theatre performance was based on principles and rules of theatrotherapy. An original script adapted to the target group was used for its implementation. Classical teaching methods and drama teaching methods were used for rehearsing, namely methods verbal, graphically demonstrative, practical skills gaining, roleplaying, methods and techiques of training games and exercises, evaluation and reflection. During the evaluation and objective technique was used which aims for an analysis of actor´s activity, and a subjestive technique which aims for self-evaluation and actor´s feedback. The above mentioned activity was supported by IN Civic Society and ten people with medium and severe mental disability aged 25-45 participated. There was a process of involvement during a making of the performance and positive results in the field of communicative and locomotive skills were reached. The actors experienced new and unusual situation during the performance. They met new people too because some students of the University participated too. This resulted in social integration. The bachelor thesis is meant as an inspiration for the personnel of institutions providing social services.

Gluten-free diet - reality in school facilities

The topic of this bachelor thesis is gluten-free diet - reality in schools facilities. The goal of this work was to determine the number of school cafeterias in the Vysocina Region which provide gluten-free meals, to evaluate awareness of the canteen staff about preparation of the gluten-free diet and to describe the cooperation between the school cafeteria and family of the pupil with gluten-free diet. School meals are important to provide optimal intake of nutrition during the time the child spends in school, to establish proper eating habits and to better integrate the child into the group of its peers. Gluten-free diet is a necessary medical precaution especially for those with coeliac disease, in which case gluten can lead to villous atrophy, and for those with gluten allergy for whom gluten intake can cause anaphylactic shock. Adherence to the gluten-free diet is recommended also to those with non-coeliac gluten sensitivity diagnosis, although gluten causes only short-term complications, usually intestinal. The method of quantitative and qualitative research was used to make this thesis. The research sample consisted of 8 school cafeterias from the Vysocina Region which provide gluten-free meal to their pupils. Awareness about gluten-free diet principles of the managers of the cafeterias or of the cooks responsible for preparation of gluten-free meal was determined by means of semi-structured interviews. The calculation of the two-day menu of each cafeteria by means of the Nutriservis Profesional program was also part of this thesis. The results were compared with the recommended daily intake of energy, proteins, fat, and saccharides for pupils of elementary and secondary schools. The results of the quantitative research show that gluten-free diet is provided by only a small percentage of school cafeterias in the Vysocina Region. The qualitative research prove sufficient awareness of the cooks about gluten-free diet, with awareness of those cooks directly responsible for preparation of gluten free meals being at much higher level. It was also determined that the cooperation with parents of pupils with gluten-free diet is a usually one time occasion limited to an arrangement of nonperishable gluten free meal packages.

Card of Cryptosporidium infections by humans and animals

The representatives of the Cryptosporidium genus are one of the causes of gastrointestinal tract diseases in humans and animals. In animals the host may even be a respiratory apparatus. The disease is called cryptosproridiosis. In majority of cases this infection can bypass without any major problems, but in immunosuppressed individuals it can cause serious health problems. Cryptosporidium has a monoxenous development cycle, which basically means that the entire development takes place in a single host. Exogenous stage is represented by oocysts, which are in case of a gastrointestinal disease excreted in faeces. In case of a respiratory disease the oocysts make they way out via respiratory and nasal secretions. This leads to contamination of the environment or water. In general, it is expected that Cryptosporidium isolates, which are present in one class of vertebrates, are not infectious to a non-specific host from other classes. It is also expected that cryptosporidia have low host specificity. But this does not exclude that some kinds have gradually extended its specificity to more species. It is also apparent that cryptosporidium infections are common in animals that inhabit the external environment, so even a human can be endangered by this zoonosis, although the incidence in the Czech Republic is low. The reason for the low numbers may as well be that parasitological examination is not performed very often, so the estimated prevalence in the population is probably much higher. Therefore to reveal the originator of this disease it would be appropriate to perform a parasitological examination in persons who are in contact with animals, this way the cryptosporidium infection would be excluded or proven.

Narcolepsy and other seizure disorders

The thesis deals with the issue of seizure disorders from the perspective of special neurology. In terms of content, it is focuses on two major areas. The first part is theoretical in nature and has descriptions of the individual disorders, their clinical manifestations, diagnoses and current treatments. It is focused on the following seizure disorder group with regard to one of the objectives of the work: epilepsy, tetany, Meniere's syndrome, migraine and more specifically narcolepsy. These findings are supplemented with information on current knowledge regarding the treatment and diagnoses of these diseases, which make the lives of thus disables people very difficult and prevent them from leading their lives the way they could if they were in full health. The aim of the work is not just a theoretical description of the currently available knowledge of these diseases from literature, but also a practical confrontation thereof from experiences, which in this case result from the true stories of people suffering from seizure disorders. A specific seizure disorder has been selected for this second part of my thesis, which needs to be talked about more in order to become better known. It is a disorder that is interchangeable with regard to its symptoms, which are similar to other disorders, and it is difficult to diagnose or is only diagnosed after an inconveniently long time - i.e. Narcolepsy . The thesis therefore deals with the issue of narcolepsy in most detail, with regard to the practical focus in the next section, where in addition to aforementioned basic knowledge, the author describes the activities and the importance of sleep laboratories and centres as institutions involved in treatment. In order to penetrate into the issue of this disorder even more deeply, she adds the aforementioned anonymous stories of people affected in this way. This part is also complemented with illustrative images of the sleep laboratory and the stories of these people as a better visual complement of the thesis for the reader. A descriptive analysis (comparison) of each symptom was chosen as a comparative method of the aforementioned thesis objective (to compare the theoretically given symptoms of narcolepsy with the symptoms of individuals suffering from this disease), in which the goal was to choose the common symptoms these people which make their lives difficult and compare these data with information (symptoms of narcolepsy) found in contemporary literature. In conclusion, we visit the most important information from scientific literature, the views of the professional community on the current research and studies on narcolepsy, i.e. the specifically examined causal links of applying the Pandemrix vaccine (against swine flu) and narcolepsy and a summary of the results of other studies investigating the etiology of this disease. The aim of this section is to present the reader with, and identify, the current developments in narcolepsy research. An evaluation of the historical development of interest in sleep disorders and an evaluation of the conclusions drawn by comparing symptoms specified in professional literature on narcolepsy with actual symptoms of people suffering from this disease are also included. The aim is to provide the reader with a bio-psycho-social view of a person with this disease, which is not always emphatically discussed in every piece of professional literature. The result of the entire thesis is a summary of the current state of knowledge of these diseases and a reflection on the research of the current possible causes of narcolepsy. It continues to compare the theoretical background of the narcolepsy disorder with the experience of living with this disease, including stories of selectively chosen individuals suffering from narcolepsy.

Students´ motivation for the work in social sphere

The topic of this bachelor thesis is the motivation of students to work in the social sphere. Selecting a domain is an important milestone in the life of each of us, the aim of the work is to determine what factors affect the young person during the election field with a social focus and how his motives during his studies and during his practical experience varies.The work is divided into two parts, theoretical and practical. The theoretical part describes the social work and its history, definition, ethical standards and goals. The work deals with the role of a social worker and describes his competence. Presents preconditions for occupational choice and the factors that influence this choice. It also deals with motivation, defines it and acquaints us with concepts such as motive, value or need. Work is affected by motivation theories and recalls the hierarchy of needs of Maslow. In the seclusion does not stay the work motivation in helping professions, questionable motives or burnout, which occurs currently in the social sphere more frequently than in other sectors. The practical part is focused on quantitative research, when the survey begins with primary school pupils who have just chosen their future focus. This part aims to determine what percentage of these students opted for the social sphere and what lead to this decision. Further quantitative research conducted among high school and college students who attend branch with a social focus. Here, the work reveals the most common and most important motives of students, satisfaction with the chosen field of study and their expectations of future studies and career. Here we will also learn how many students want after graduation stay in the field. In the practical part there is also a qualitative research, which complements the whole job. Semi-structured interviews are conducted with students, but also with a social worker. These respondents describe us closer their motivation for working in the social sphere. According to the results which this research brought, it can be stated that most of the students choose a subject with a social focus to have the opportunity to help the others. But is true that many students are choosing a school prematurely and are influenced by the place of residence, their friends or the nature of the study, most of these people don´t know what they can expect from the field and are often discouraged during their studies. From this work could in practice benefit pupils and students who decide for the field of social affairs. For teachers working in schools where the research took place,could this work serve as a feedback. Especially the part which describes the differences between expectations and reality can be used.

Use of e-learning education of universal nurses in Militari university hospital - University hospital Prague

The diploma work submitted is called The Use of E-Learning Education of Nurses in the Central Military Hospital Military University Hospital Prague. The theoretical part of the diploma work deals especially with the significance of e-learning, distance learning, history of education of nurses in the Czech Republic and e-learning in health care. In addition, the work includes information of the Central Military Hospital and describes its present circumstances and use of e-learning in the working process. 5 aims and 4 hypotheses were set for the quantitative research. The first aim was to determine the time demands of the e-learning courses study. The second aim was to determine the attitude of general nurses to e-learning education. The third one focused on contribution of e-learning education for general nurses and for the hospital management. The last aim focused on satisfaction of general nurses with the form, contents and diversity of e-learning courses. The fourth aim was used to determine reasons leading for development of e-learning courses for general nurses. Four hypotheses were determined: H1: It takes less time to complete e-learning course studies than regular studies. H2: Nurses younger than 30 years of age prefer e-learning education unlike nurses older than 30. H3: The effectiveness of regular studies is better than the effectiveness of e-learning education. H4: E-learning courses are not as financially demanding as regular courses. The data collection technique consisted in our own anonymous non-standardized questionnaire containing 32 items. The research file consisted of 212 general nurses working in clinical departments of the Central Military Hospital. An additional method was based on a dialogue with the Education Team Manager of the Central Military Hospital. The research results related to the hypotheses show that completing e-learning course studies does not take less time than completing a regular course. Nurses younger than 30 years of age do prefer e-learning education contrary to nurses older than 30. The effectiveness of regular courses in contrast to e-learning education has not been confirmed as well as the fact that financial demands of an e-learning course are lower than financial demands of a regular course. Further, nurses prefer regular forms of studies, mostly with their passive participation in an educational event; however, they find completion of e-learning courses more convenient. Most respondents also expressed a positive opinion on the form, contents and diversity of e-learning courses. Courses include enough information for preparation of a final test and nurses find the number of courses necessary for periodical evaluation adequate. What they consider limiting is the impossibility to retake the final test; they mostly suggested it should be allowed to take the final test twice or three times. The research implies that a higher percentage of the respondents spend as much time as they want studying. Nurses still prefer regular forms of studies with passive participation to the e-learning form. Most nurses have a positive opinion on the current contents of courses in the form of a text format. The course topics are mostly based on suggestions by members of the Non-Medical Professions Education Team of the Central Military Hospital. The research results will be presented at the next regular meeting of members of the Non-Medical Professions Education Team of the Central Military Hospital. Procedures for improvement of the form of the current courses will be presented and proposed as well as the possibility to develop communication between the author and students, the limiting impossibility to retake final tests and the suggestions concerning the number of possibilities to retake them.

The effect of pyrethroid based pesticides on fish

Pyrethroids are ones of the most used pesticides worldwides. The widespread use and high stability of pyrethroids lead to the assumption of that their occurrence in the environment could be quite frequent. They can reach water ecosystem as pollutants. Residues of pyrethroids are not only detected in the water column, but also in sediments and in fish tissues. The first study was devoted to the product Nexide containing 60 g.l-1 of active substance gamma-cyhalothrin. Tested Nexide concentrations were 5, 25, 50, 100, and 250 &microg.l-1. Early life stage test was used.Common carp (Cyprinus carpio) was tested. There were significant mortalities in all concentrations except the lowest concentration during the trial. The lowest concentration tested 5 &microg.l-1 only caused a slightly increased mortality. This lowest concentration influenced the growth in length and weight negatively, decelerated ontogenetic development, and made the body surface of the individuals darker. Histopathology of individuals from this concentration revealed dystrophy in liver. Examination of kidney, intestine and gills did not show significant histopathological differences compared with control. The evaluation of selected parameters of oxidative stress demonstrated a significantly higher activity of detoxification enzyme glutathione-S.transferase (GST) and a significantly lower activity of defensive enzyme glutathione peroxidase (GPx) compared with the control group. The other examined parameters of oxidative stress such as catalase (CAT), glutation reductase (GR), and lipid peroxidation determined by using the thiobarbituric acid-reactive substances (TBARs) were comparable to the control group. Changes in oxidative stress parameters suggest that exposure of the organism to the product Nexide in the given concentration leads to dysbalance of defensive enzymes. The second study was devoted to the product Cyperkill 25 EC containing 250 g.l-1. Tested Cyperkill 25 EC concentrations were 7.2, 36, 72, 144, and 360 &microg.l-1. The procedure of the trial was the same as the preceded one. There were 100% mortalities in all concentrations except the lowest concentration during the trial. The lowest tested concentration 7.2 &microg.l-1 allowed 90% of individuals to stay alive till the end of experiment. The lowest concentration influenced the growth in length and weight negatively and decelerated ontogenetic development compared with the control. Any individual exposed to this concentration did not reach juvenile stage until the end of the trial. Dark pigmentation was visible in 68% of these exposed individuals on the last day. Similar darkening was visible in individuals from higher concentrations shortly before death too. Histological examination did not revealed significant changes in intestine, liver, kidney, and gills compared with the control group. Evaluation of selected parameters of oxidative stress demonstrated significantly lower activities of GST, GR, and GPx. Activities of CAT and TBARS were comparable with the control group. Changes in oxidative stress parameters suggest that exposure of the organism to the product Cyperkill 25 EC in the given concentration could induce oxidative stress and interfere with the activities of antioxidant enzymes. The presented thesis summarises actual data about pyrethroids and their influence on fish. The demonstrated effects confirm high susceptibility of early developmental stages of fish to tested pesticides. When interpreting the results, we have to take into account the fact that studies showed this risk even on single pyrethroid substances. But water organisms are exposed to many other more or less toxic products and substances in a real environment. These xenobiotics could react with each other and their mixture could even potentiate negative effects. The performed studies also clearly show the significant differences in the sensitivity of embryonic and embryolarval tests.

Iniciace pohybu byčíku, signalizace a regulace pohyblivosti spermií ryb: fyzikální a biochemické řízení

The current study attempted to shed light on the regulatory processes and response arrangements of fish spermatozoa during the course of maturation and motility initiation. The first intent of this study was to improve the understanding of the mechanism underlying the acquisition of potential for sperm motility in sturgeon. Up to present work, the physiological process underlying sperm maturation in this species has not been described at all. Our results showed that sperm maturation in sturgeon occurs outside the testes because of dilution of sperm by urine and involves the participation of high molecular weight substances as well as calcium ions present in seminal fluid and/or urine. The second aim of the present study was to investigate the coping mechanisms in fish spermatozoa with osmotic and ionic activating mode, as well as in spermatozoa of euryhaline fishes, to various environmental conditions. We showed that alteration of environmental osmolality might affect the fish sperm in different ways, depending on fish species and modes of spermatozoa motility activation either osmotic or ionic mode. In response to osmotic stress caused by hypotonicity, carp spermatozoa regulated the flow of water across their cell membrane and increased their cytoplasmic volume during their short motility period. In contrast, no indications of sperm volume changes were observed neither in sterlet nor in brook trout spermatozoa, both of which having an ionic mode of motility activation. We also examined the mechanism by which sperm motility triggering in euryhaline fishes can adapt to a broad range of environmental salinity. Our results demonstrated that spermatozoa of euryhaline tilapia, Sarotherodon melanotheron heudelotii, reared in fresh-, sea- or hypersaline water can be activated in hypotonic, isotonic or hypertonic conditions of swimming milieu, provided Ca2+ ions are present at various levels. It was established that the higher the fish rearing salinity or the more hypertonic ambient media at spermatozoa activation, the higher extracellular concentration of Ca2+ ions is required. The results obtained in the present study allow suggesting that osmolality is not the main factor inhibiting sperm motility inside the testis in the S. melanotheron heudelotii. A third aim of this study was investigation of the regulation of motility initiation process and description of flagellar beating initiation in chondrostean spermatozoa. We detected that K+ inhibition of sperm motility in sturgeon can be by-passed due to the pre-exposure of sperm cells to a high osmolality shock prior to its transfer to K+-rich swimming media. Thus, we hypothesized that sturgeon spermatozoa may be activated by use of an unexpected signaling pathway, independent from regular ionic stimulation. The successive activation steps in sturgeon spermatozoa were investigated by high-speed video microscopy, using specific experimental situation, where sperm motility initiation was delayed in time up to several seconds. At motility initiation, the first couple of bends formed at the basal region begins to propagate towards the flagellar tip, but gradually fades when reaching the mid-flagellum. This behavior repeats several times until a stage where the amplitudes of bends gradually reach similar value, what eventually leads to sperm progressive displacement. The total period needed for the flagellum to switch from immobility with rigid shape to full activity with regular propagating bends ranges from 0.4 to 1.2 seconds. In conclusion, the results of the current study bring valuable pieces of information into the general understanding of the processes of maturation of fish spermatozoa, their adaptability to different physical and biochemical circumstances, the extra- and intra-cellular signaling as well as the regulatory mechanisms of motility activation in fish spermatozoa.

Territorial Administration in the Czech Republic - the direct election of mayors

The starting point of the diploma thesis is a discussion of the direct election of mayors in the Czech Republic. The aim of the thesis in this context is to assess whether the direct election of mayors is appropriate in the normative framework of territorial administration of Czech municipalities and if so, under what circumstances. A partial aim of the work, intended to lead to the fulfilment of the main aim, is the description of the formal role of mayors in the administration of municipalities, the description of the manner of appointment of mayors into their positions, as well as the description of the anchoring of the position and election of mayors of municipalities in selected European countries. Another partial aim is the description of starting points for the direct election of mayors, stemming from the attitudes of mayors of selected municipalities. The subject of study will be the normative setting of the mayors position in the regional administration in the Czech Republic and simultaneously the manner of their election. Another subject of study will be the attitudes of various mayors in relation to the issue of their formal position and election. The data on the mayors attitudes will be obtained by the focus group technique. The criterion for the selection of respondents will be the municipality size. The purpose of the work is to generalize the data obtained and to assess it on the basis of the prevailing opinion.
