Medical documentation in pre-hospital care

The topic of this bachelor thesis is Medical documentation in pre-hospital care. This work was divided into two parts. The first part was theoretical, the second practical research. In the theoretical part, the emergency medical service is described with its basic elements and history. Furthermore, the issue of medical documentation was described in general view with its most important aspects. In the last part, the pre-hospital medical documentation service was described by actual examples. Four goals were set in the practical part. The first was to find out the opinion of paramedics on the documentation they are filling and writing as a part of the prehospital care. The second goal was to find out the opinion of paramedics on this documentation in case of need to prove their lege artis procedure. The third goal was to find out the benefits of changes of the software that is used to manage medical documentation. The last goal was to find out how this documentation is subsequently used in the hospital after the handover. The method of qualitative research in the form of semi-structured interviews was chosen for the practical part. The research samples for the first two goals consisted of 12 paramedics, each of whom was asked 19 main questions, additional subquestions were asked in a case the interview led to them. For the third goal, the research sample consisted of 8 informants, who were asked 6 main questions, which were also developed appropriately by additional subquestions if needed. For the last goal there was research sample of 5 informants from the ranks of nurses in the hospitals. The goal of the research is to point out the most important matters of the pre-hospital documentation that is filled in by the paramedics and their opinion on the changes that were made during last year. The work can serve as a basis for possible changes in the management of medical documentation in the conditions of the emergency medical services.
Telephone Assisted Immediate Adult Patient Resuscitation by Emergency Medical Dispatcher

This bachelor's thesis deals with Telephone Assisted Immediate Adult Patient Resuscitation by Emergency Medical Dispatcher. This thesis was divided into two parts. The first part was theoretical and the second part was practical. The main focus of the theoretical part was on medical emergency service and medical operations centre. Heart anatomy and cardiac arrest are also mentioned in this thesis, as well as problematics of an urgent resuscitation, chain of survival and telephone-assisted immediate resuscitation. The second part was the research part. Two research questions and two objectives were established in this part. First objective was to map knowledge of dispatchers of a medical operations centre in telephone-assisted immediate resuscitation. Second objective was to map how dispatchers of a medical operations centre proceed in provision of telephone-assisted immediate resuscitation. Semi-structured interview method was performed in the practical part of this thesis. Sample consisted of 10 dispatchers of the Regional medical operations centre of the South Bohemian Region who were asked 27 questions. For the sake of clarity, collected data were divided into 14 categories and processed into tables. Work objectives were fulfilled and research questions were answered. Research showed that dispatchers have stronger theoretical knowledge of telephone-assisted immediate resuscitation management of an adult patient, rather than of a pediatric patient. Research also mapped out the procedure of telephone-assisted immediate resuscitation of each individual responder. Results are rather positive.
The Preparedness of Emergency Medical Services in the Pilsen Region and The Upper Palatinate for Emergency Situations

The aim of this diploma thesis is to present and analyze the preparedness of ambulances of the Pilsen region and the Upper Palatinate to solve emergencies with a focus on cross-border cooperation. The theoretical part describes the integrated rescue system of both territorial units with emphasis on the medical rescue service and its system of providing pre-hospital emergency care and the system of dealing with emergencies affecting more people. The important part is also the legislation of possible cooperation and an important project of cross-border cooperation, thanks to which this activity even started. The research part is performed in the form of controlled interviews with managers of ambulances and managers of the cross-border cooperation project. These workers regularly participated in joint exercises and their awareness of this issue is at a high level. Based on the data obtained from the interviews, a SWOT analysis is performed, and the results are interpreted. The research demonstrated problems in the communication of the ambulances of the Pilsen Region and the Bavarian Red Cross, the problem in the language barrier and the diversity of procedures in solving emergencies. The advantages include quality training of employees and benefits from previous exercises in the form of new planning documentation for cross-border cooperation. Based on the data obtained and the SWOT analysis performed, an alliance strategy is chosen to take advantage of opportunities to eliminate or mitigate the weaknesses of cooperation. In the discussion, is the result of presenting shortcomings and a proposal for possible improvements that would help to improve cross-border cooperation in the future. These proposals will be presented to the employees concerned at the next meeting of the cross-border cooperation project.
Crush syndrome from the perspective of the paramedic

The topic of the bachelor thesis is Crush syndrome from the perspective of the paramedic. In the theoretical part of the work, readers can learn what the crush syndrome is, when and in what situations it arises, what processes take place in the affected person's body, how it then manifests itself and what is its therapy. In the practical part, one goal was chosen. To map the level of awareness of non-medical medical staff about the issue of crush syndrome. A qualitative method of data collection in the form of semi-structured interviews was used to process the research set, where there were 7 main questions, on the basis of which the interviews were further developed. The research sample consisted of 12 respondents. 6 paramedics performing their service in pre-hospital emergency care and 6 members of nursing staff performing their profession in an acute care inpatient facility. The output of the bachelor's thesis is an overview that shows the awareness of the respondents about the issue of crush syndrome. It draws attention to minor shortcomings in both theoretical and practical knowledge of the issue. The work could serve as a basis for a better understanding of crush syndrome in the training of paramedics.
The organization of proviving pre-hospital emergency care in combat conditions

My bachelor thesis titled Organisation of Providing Pre-hospital Medical Care in Combat Conditions is divided into a theoretical and practical part. In the theoretical part I focus on the history of care provided in field conditions, on the legal sphere concerning providing pre-hospital medical care, and on the education of the non-medical workers, including the courses available for the Czech Armed Forces. I also focus on the providing of the first aid in combat and extreme conditions according to the approach of Tactical Combat Casualty Care, where the chapter is separated into parts regarding medical treatment under fire, medical treatment in tactical field care, and medical treatment during the transportation. Chapter First Aid Algorithms follows, consisting of report of the incident, sorting of the wounded, and injury examination. In the last section of the theoretical part I examine medical security, medical equipment, and medical documentation. The research part has one goal: "to elaborate a unified summary of processes of providing pre-hospital medical care in a tactical environment used by paramedics of the Czech Armed Forces and to evaluate their possible implementation into the conditions of the civilian sector, i.e. their possible use by off-site groups of emergency medical care providers." Based on this goal were set down two research questions. Question number 1: "Is it possible to employ the processes of providing pre-hospital medical care used by the Czech Armed Forces in a civilian sphere of emergency medical care providers?" Question number 2: "Is it possible to employ military medical tools and devices also in a civilian sphere of emergency medical care providers?" Answers to these questions were acquired with the help of a qualitative inquiry in form of semi-structured interviews. The inquiry assemblage consisted of six Czech Armed Forces paramedics, five civilian paramedics, and one civilian emergency medical services physician. The respondents were asked ten questions, which were then divided into ten categories. The research showed that the Czech Army Forces and the civilian sphere of emergency medical services are closely connected. This applies to procedures in certain situations or to material equipment.
The Work of an Operator of Emergency Medical Service

This bachelor thesis mostly dealt with the activity of medical controllers as well as the issues of medical operative center in general. Many people don´t know what it is all in all. It is only undemanding sitting at the table and answering the phone for most people. I would like to point out the facts related to this profession and refuted this claim of the majority. This thesis mentions the medical operation center, its history in the Czech Republic, and it is interested in its tasks, layout and equipment. There are also mentioned the activities of emergency medical service-controllers themselves, including their education or criminal responsibility. Emphasis is also placed on the stress on medical service-controllers in the performance of this profession. A large part of the work is devoted to taking the emergency call, telephone-assisted emergency resuscitation and telephone-assisted first aid. Mapping the theoretical knowledge of medical service-controllers and finding out the individual procedures for providing the telephone-assisted resuscitation of both, adults and children, was one of the aims of this bachelor thesis. Another goal was to determine the satisfaction of service-controllers within the performance of their profession. There was performed a qualitative research using the interview technique. The interview included knowledge and satisfaction with this profession as well as the issues of telephone-assisted resuscitation of adults and children. According to the results of this survey there was found that the medical service-controllers are not sufficiently prepared to perform this profession immediately after the graduating their studies. They use the same or very similar procedures within their telephone-assisted resuscitation of an adult or child. The survey also shows they are satisfied with their job.
Indications of Emergency Medical Services Responding

The bachelor's thesis is focused on a closer understanding of the issue of indications of emergency medical service (EMS) departures through the perspective of medical emergency dispatch centers. The issue of indications of emergency medical service departures is one of the still addressed and current topics in the pre-hospital emergency care. The activities of outreach crews as well as the activities of the dispatch centers are governed by Act 374/2011 Coll., however non-emergent, calls to the emergency line are becoming more and more frequent. The thesis maps the knowledge of the indication list of individual dispatchers and the attitude of dispatchers towards the indications itself. At the same time, it deals with the solution of unindicated calls and also the abuse of emergency services. The aim of the work is to compare the knowledge and procedures of dispatchers of both regions. The research took place at the Medical Emergency Dispatch Center of the South Bohemian Region in České Budějovice and at the Medical Emergency Dispatch Center of the Pilsen Region in Pilsen. The aim of the research was, among other things, to compare approach to indication issue between both regions. The research method chosen was a qualitative type of research that allows deeper examination of the topic. Research was executed by semi-structured interviews. The sample of respondents consisted of 8 dispatchers of the medical operations center i.e., people directly responsible for receiving calls and coordinating crews. The interview questions are focused on both the reception of the emergency call and the operational management. Above all, they focus on the urgencies N3 and N4, ones where we most often encounter not entirely unambiguous indications. It also addresses the relationship between the use of formalized and intuitive procedures in operators' decisions. Last but not least operators are confronted with the notion of EMS abuse and the solution of unindicated calls. The interviews provided information on a wide range of approaches to dispatching work but there was often agreement on key points. The results show that dispatchers are familiar with emergency procedures and legislation, however, despite the application of a largely formalized approach to classifications, it is necessary to act partly intuitively, as no call is ever the same as a regulatory case. There is a greater effort to define the terms unindicated calling and abuse of emergency medical services, which, however, are perceived individually by each of the dispatchers, as well as perceived by every rescuer who comes into contact with them. These concepts often evoke great emotions among dispatchers and rescuers. However, as the results show, even this problem does not have only one point of view, but it has several of them and all of them must be taken into account when solving it.
Specifics od Integrated Rescue System from the General Public View in Jihlava

The Bachelor thesis "Specifics of Integrated Rescue System from the non-professional public's view in Jihlava" pursues the issues of Integrated Rescue System and its bodies in the Czech Republic. The view on the Integrated Rescue System and the opinions about the Integrated Rescue System given by the citizens of Jihlava are considered to be the greatest aspect of this thesis. The theoretical part of the thesis introduces and describes basic and other bodies based on specialised literature. The main focus is on bodies operating in the city of Jihlava, the capital of Vysočina Region. The thesis gives detailed information from the historical pint of view within the whole country and also on the local bases. The practical part focuses on the non-professional public's view on the Integrated Rescue System through a semi-structured questionnaire. There were three hypothesis formed at the beginning of the practical part of the thesis. Following the gained research data these three hypothesis were verified and subsequently confirmed or disproved. The results of the research are presented by graphs, charts and tables for closed-ended questions and word processing of respondents' opinions for open-ended questions. Photographs showing symbols of main bodies of Integrated Rescue System are included in this thesis.
Emergency situation in practice of a paramedic

Providers of the emergency medical service are the basic bodies of the integrated rescue system. The paramedics that work in the emergency medical service participate in dealing with emergencies in cooperation with other bodies of the integrated rescue system. Unlike the common operations of the emergency medical service, during emergencies it is necessary to perform the rescue and clean-up work according to the appointed rules of the integrated rescue system. An essential part of the paramedics´ duty are various emergency activities exercising. The aim of the diploma thesis is to introduce a concept of an emergency and participation of paramedics in emergency and distress events. Furthermore, the thesis highlights frequent flaws in paramedics´ knowledge and points at such mistakes that might appear straight during an operation. It should contribute to improvement of an educational system for the paramedics who participate in dealing with emergencies. There are two research questions and two hypotheses set up for this aim. Research question 1: How do the paramedics perceive an emergency? Research question 2: What kind of experience of emergency solutions do the paramedics have? Hypothesis 1: The knowledge of a paramedic concerning the procedures during an emergency is influenced by the length of their practical duty. Hypothesis 2: The knowledge of a paramedic concerning the procedures during an emergency is influenced by the experience in dealing with the procedures. The research part comprises interviews with ten paramedics who practise their profession at least one year. Furthermore, a questionnaire construction with 106 respondents who work as paramedics at least the same time. The set of respondents and informants is composed of some members of the regional emergency medical services of the Czech Republic and other/private providers of the emergency medical services. The results of the thesis provide some flaws in paramedics´ knowledge from the field of the integrated rescue system as well as in cooperation with its other bodies, especially in operations coordination during the emergencies. The results provide an observation that even with a longer practical duty in emergency medical services and with better experience in dealing with emergencies, the knowledge and often also communication and coordinating skills of the paramedics will not improve. The weak points are the fields of terminology, function and structure of the integrated rescue system and their implementation in practical duty. The thesis fully reveals that during an operation of the professional bodies of the integrated rescue system in emergencies, mistakes occur in coordination and communication. It happens that the set competencies are not respected by individual bodies.

The thesis describes Medical Rescue Service of South Bohemia Region as the provider of pre-hospital urgent care in the zone of emergency planning of Nuclear Power Plant Temelín. In case radiation emergency arises, its main urgent target is to save as many affected people as possible and to limit the effects on their physical disabilities. Medical intervention by the Medical Rescue Service of South Bohemia Region crews is provided to people with serious health problems or in immediate life danger who participate in rescue and emergency work directly in the epicentre of the radiation emergency of Nuclear Power Plant Temelín. It also provides pre-hospital urgent care to the inhabitants who have already been evacuated right in the zone of emergency planning, on the evacuation routes, on the decontamination points and based on the emergency call also in the receiving centre or emergency accommodation points. The main aim of the thesis is to identify and clarify basic principles of tasks completion and workload of outgoing Medical Rescue Service of South Bohemia Region crews in case of an emergency event occurrence - the radiation emergency at Nuclear Power Plant Temelín. The theoretical part describes the status of the Medical Rescue Service of South Bohemia Region within the Integrated Rescue System in the case of radiation emergency. Attention is also paid to material and technical equipment aimed at personal protection during the intervention in the contaminated environment. Content analysis of key executive documentation and legal regulations research have been made in this part whose synthesis leads to a comprehensive professional text predicting the procedure of urgent medical intervention made by the medical rescue service in the zone of emergency planning. The practical part includes the network analysis of transportation availability of individual decontamination points by the medical rescue service vehicles. Based on the findings, the index of alert has been calculated that reflects the quality and availability of pre-hospital urgent care to the evacuated inhabitants in the decontamination point as the key point of evacuation from the zone of emergency planning. The results proved readiness of the Medical Rescue Service of South Bohemia Region in the decontamination points to tasks completion and workload of medical provision of evacuation from the zone of emergency planning of Nuclear Power Plant Temelín. The thesis has become a concentrated overview as well as operating material for the paramedics working at Medical Rescue Service of South Bohemia Region who could deal with the uneasy task of intervention in the zone of emergency planning of Nuclear Power Plant Temelín. It also reflects the need of preparing a unitary documentation describing the procedure in radiation emergency.
