The risk of transfer HIV in ambulance services

HIV and the infection of AIDS is a great threat for the inhabitants of the entire world, in particular for rescue services professionals and health workers who daily come into contact with blood and risk situations that require a rapid solution for the rescue of a human life. Their work is to treat the patient as quickly and safely as possible and not to endanger themselves or their surroundings. The objective of this work is to ascertain the awareness of this illness, and if health workers at rescue services are sufficiently protected against this diseases, so far untreatable. Through questionnaires I wanted to ascertain the level of knowledge and prevention from infection.
The use of immobilization tools in urgent prehospital situations

The topic of the thesis ``The Use of Immobilisation Aids in Pre-Hospital Urgent Care{\crqq} was chosen for my own interest in these problems. Immobilisation aids are very important for the work of a paramedic. The aim of the thesis was to map the use of immobilisation aids during the action of South Bohemian Region{\crq}s Health Rescue Service. Two hypotheses were stated for the research. Hypothesis 1, that the most used aid is a stretcher, was proved. The results are shown in Graphs 16 and 14. 62 % of informants marked a stretcher (21 times and more in a month) as the most used aid. A stretcher is used at indications of polytrauma (68), perforating injury of abdomen (63), chest trauma (65), and spine injury (54). Finally, as shown in Graph 20, the paramedics would use a stretcher for the transport from hardly accessible terrain. Hypothesis 2, that paramedics use the scoop-frame the least, was also proved, as the results show in Graph 8 where the frequency of scoop-frame use was marked the answer 0-3 times a month the most, which was 87 (91 %). The results of the research point at the frequency of immobilisation aids use in South Bohemian Region, experience of paramedics with various aids for the purpose of immobilisation and their theoretical knowledge needed for the right use of a particular aid. We cannot omit practical experience which are very valuable and which have shown that the paramedics with long-term practice have more experience with immobilisation aids than the paramedics with short-term practice. I would like to draw the attention to the education of paramedics during the exercise of their professions. During the research I encountered a very interesting result. In Graph 1 the results show when the paramedics became familiar with immobilisation aids: 39 (41%) during studies and 8 (8%) during training for Health Rescue Service. I think that the solution could be the introduction of regular training concerning immobilisation aids, their adjusting and use at Health Rescue Service. The results of my thesis might be used in the area of ambulance equipment with immobilisation aids and mainly in the area of extension of compulsory equipment. Further, they might be used to educate students of medical branches at universities.
Life style of a rescuer

Process of the stressed situation and the following settlement with the given fact is given through the adaptation of the living conditions, surrounding and the types of tasks in the connection with human possibilities. Mainly in the sphere of helping professions inadequate stress can appear in the participation with the emotional, psychical and physical demandingness of the adequate activity. Administrative activity, ability to treat the ill clients in the time pressure during difficult circumstances is embodied in the work of the health worker. All these demands on the worker performance of the health worker are very demanding, often very exhausting on the man´s personality. This work is topically oriented on the mapping of the life style in the profession of a rescue worker. Research was taking place among the rescue workers in the North Moravian District in the from of the quantitative research. The aim of the work was also the problems solution analysis connected with the worker performance and the proposal of possibilities of psychically and physically demanding situations managing. The first hypothesis that should confirm the influence of the rescue worker profession on the life style was confirmed by 35 (62%) from 57 (100%) respondents. The following hypothesis that should verify deprecation of the problems on the rescue service workplaces connected with the worker performance of the rescue worker was also verified. For the right negotiation and behaviour in the demanding life saving situations it is, according to my opinion, necessary to appeal on the maintenance of the psychical and physical balance at very health worker in the rescue services.
Patient - rescuer relationship

The purpose of this essay is to learn about basics of correct communication and outline its principles. The objective was to map situation concerning relationships between ambulance rescuers and rescued patients. Data has been collected on the basis of a qualitative study conducted in the form of moderated interview concerning the given issues and circumstances. Data was collected in České Budějovice, Tábor and Karlovy Vary. The target and subject of the research were nine health rescuers and nine patients rescued by ambulance. Analysis of all nine interviews maps aspects of health rescuers communication skills applied during emergency pre-hospitalization care. The results indicate prevailing agreement on the positive aspect of the profession, which is helping to people and life saving. This essay also gives patients who were in contact with ambulance crew the opportunity to express their opinion. The readers may find number of citations and their interpretation.
Approach of rescuer to craniocerebral injury

In the theoretical part of my bachelor thesis I address the injury of neurokrania, anatomy, diagnosis, classification and possible complications of injuries, mainly in relation to paramedics and prehospital care. Marginally I also address paramedics themselves, particularly their responsibilities and competencies. In the practical part of my thesis I work with the results of my research for which the quantitave method was mainly used. Especially with the results of my questionnaire, which was directed to paramedics, students and other workers who are affected by the issue. The hypothesis was set: "A paramedic does not have sufficient knowledge and skills in an approach to craniocerebral injury." The research showed that respondents have knowledge, but not at a satisfactory level. And that they would appreciate the opportunity to extend or complement their knowledge, not only for themselves. The hypothesis was largely confirmed. This is why I decided, based on previously known information that I received while writing my thesis, to prepare a summary of information - an algorithm, that will serve those interested in the issue to expand and complement the information obtained so far.
The role of a medical rescuer during cardiopulmocerebral resuscitation in the ambulance crew of emergency rescue help,emergency medical help and randez vous system

The thesis surveys the performance of a rescue worker by cardiopulmonary resuscitation in a rescue team (squad) of emergency care. This care is provided by several kinds of squads ? by ALS (Advanced life support vehicles) ? large ambulance vehicles comprising a paramedic and a driver/rescue person), by RRV (Rapid response vehicles) ambulance carrying a physician, and also by the rendezvous system, where a physician works together with an ALS crew in situations when a patient's life or health is in serious danger. The theoretical part of the thesis deals with education and competences and powers of a medical rescue worker. Anatomic-physiological information on the heart, the respiratory system and the central nervous system follow. At the same time, the personnel of the medical rescue are explained and the emergency medical stations (bases) in Southern Bohemia are mentioned. Further on, the procedure during basic and extended cardiopulmonary resuscitation and the handling of external defibrillator are described. Then, the practical part follows. Its objective is to survey the competences of a rescue worker in the above mentioned emergency squads and the procedure by the cardiopulmonary resuscitation. The survey had the form of a questionnaire and 100 rescue workers from Southern Bohemia responded to 13 questions related to the cardiopulmonary resuscitation. The gathered results brought the answer to the third hypothesis. The geographical dislocation of emergency medical service stations of Southern Bohemia and the table of the stations of Emergency Medical Services of Southern Bohemia ? types of emergency rescue squads and their composition according to the type are enclosed in the appendix.
Basic clinical examination of the client by the medical rescuer in the pre-hospital urgent care in the South-Bohemian Region

The issue of the clinical examination is the topic being more a part of physician work from the historical point of view. This state changes under the influence of the development of manning the crews in the rescue service. This bachelor thesis called Basic clinical examination of the client by the medical rescuer in the pre-hospital urgent care in the South-Bohemian Region summarizes the basic examinations which should be theoretically known as well as practically used by each medical rescuer. The target of the thesis was to find out the knowledge of the present medical rescuers working in the Medical Rescue Service of the South-Bohemian Region and if they observe the procedures of clinical examination. The contents of the theoretical part is the characteristics of rescue service, its last development and trend, also the young specialization of ?medical rescuer? is described, i.e. the possibilities of study, powers and contents of work. In the next part, the thesis treats the clinical examination. At first the contents of clinical examination is described, the access to it and its importance is explained, than the individual parts of the examination are analyzed. In the theoretical part, it is possible to find the information about anamnesis, methods of physical examination of all the body parts and organs. At the end, also the basic appliance method is mentioned, contributing to the clinical examination in the pre-hospital care as well as the good attitude to the patient, who is an important part also in this topic. If follows from the results of the quantitative examination that there are big differences between various degrees of the education. The graduates of secondary medical schools, of the study (after GCE) of anesthesiology-resuscitation care and intensive care and of higher professional schools have a lower knowledge in this field than their colleagues with the university education. In total the rescuers do not observe the procedures of the clinical examination more often. Both hypotheses were confirmed. It follows from the thesis that it is necessary to try to close the gap. During the studies, the clinical examination should be more accentuated, we should not reconcile with the present state. Also after the study of the branch, the topic should be repeated at the workplaces by means of theoretical lectures and practical seminars. In future, the medical rescuer will have to substitute also the physician in clinical examination, but he/she is not prepared for this up to now.
The characteristic work of paramedic in the operations centre- stress factors and possibilities of their elimination

In my bachelor thiesis of the Characteristics of the paramedical rescuer´s job at the control centre - stress factors and possibilities of their elimination, I concentrated on the Health control centre and the employees working there. Particularly, on their work and stress factors that affect these employees. The issue of a job at the Health Control Centre of Health Rescue Service is really wide and interesting. In a wide range, operators are able to increase the quality of human rescue due to their knowledge and skills. The theoretical part of my thesis concerns paramedics, health rescue service, the topic of work at health control centres, further stress, reactions to stress, as well as possibilities of stress eliminations. In the research part, I focused on the stress situacions that occur in this profession and that affect every operator. The research was carried out with the qualitative form, where the qualitative research was performed with the technique of a controlled dialogue. The data acquisition was realized with the questioning method followed by registracion and evaluation. The first aim of the thesis was to analyze paramedicals´ work processes at the control centre. As the second aim, I set to find out the stress factors specific for the operator´s profession, to characterize them more closely and suggest their elimination. It resulted from the research thatemployees working at the control centre know their work procedure, their proceed in accordance with them and are further educated in their branch. Moreover, it turned out that operators constantly face stress situacions and tasks which they have to solve. However, these situacions can be partly influenced, as written at the end of this thesis. In conclusion, I would like to mention that this thesis contains my opinions on this topic and suggestions for the improvement of the present situacion. At the same time, the thesis offers space for a discussion and consideration.
The possibility of pain management in prehospital emergency care

I chose the topic Possibilities of pain management in pre-hospital emergency care for my bachelor thesis as pain belongs to the most frequent symptoms of patients in the care of the Emergency Medical Service. The goal was to map the possibilities of pain management in pre-hospital emergency care in South Bohemian Region. The second goal was to map the awareness of pain management in pre-hospital emergency care among South Bohemian medical rescuers. The theoretical part deals with the present situation of the Emergency Medical Service, education and competences of rescuers. In the next part I focused on various types of pain, assessment and subsequent treatment of pain. The last, theoretical part describes the caring process in pre-hospital emergency care for a patient in pain. The research was performed by means of quantitative method in the form of a questionnaire with semi open and closed questions. The questionnaires were distributed at Emergency Medical Service regional centres of South Bohemia. The results show that pain is sufficiently managed in pre-hospital care. Another fact was confirmed, that rescuers are familiar with the pain management possibilities and prefer pharmacological methods of pain management. We also found out that non pharmacological interventions were used less frequently than they could be. One of the hypotheses was refuted, two were confirmed. The quantitative research results show that there is still a relatively large reserve in using non pharmacological methods. Rescuers might use them more, particularly the quick emergency crews, or they might be applied in situations not requiring immediate pharmacological treatment. Educational seminars at the individual workplaces might improve the awareness of non pharmacological intervention techniques. A patient in pre hospital emergency care suffering from pain of various intensity should be given primary professional care from qualified rescuers.
The Problem of Distal Cannulation by Medical Rescuer in Pre-Hospitalization Care

At present, an absolute necessity for health workers is knowledge of the way how to access the bloodstream both in an area of the pre-hospital care and in an area of the health care. The bloodstream is an ideal choice in cases when times plays a major role in the onset of a drug. In my bachelor thesis I focused on issues regarding an assurance of the peripheral bloodstream by a medical rescue worker in the pre-hospital emergency care. I concentrated on the assurance of an intravenous access and at present more and more discussed intraosseous entry. Nowadays, when a lot of medical rescue workers yearn for raising their competences, it is necessary to think if they really know and apply these basic skills while assuring the peripheral bloodstream. An aim of the thesis was to determine if medical rescue workers have an appropriate theoretic preparation and if they know principles of the assurance of the peripheral bloodstream, if they follow these procedures in practice and if there are any differences in the issues of the assurance of the peripheral bloodstream at chosen regional centres of Medical Rescue Service of the South Bohemian Region given by a different driving distance to a hospital facility. A quantitative research was selected as a method applied. An anonymous questionnaire was used as a technique of the data collection. It was distributed to medical rescue workers operating at Medical Rescue Service of the South Bohemian Region. The results of the research showed that medical rescue workers did not have very deep knowledge in case of the intravenous entry but on the contrary they had good knowledge of the intraosseous access. It was also proved that medical rescue workers did not follow prescribed procedures during the intravenous cannulation and not all of them used safety devices. Another result of the research is a fact that medical rescue workers have theoretic knowledge of the intraosseous cannulation but they do not have enough practical experience with it. The last of the most important results of the research was the finding that medical rescue workers in České Budějovice cannulate with a lower frequency than medical rescue workers from other regional centres where there are not so noticeable differences. On the grounds of the data obtained I think that the results of the research included in the bachelor thesis could be used to deepen theoretical knowledge of medical rescue workers in the area of cannulation issues of the peripheral bloodstream.
