Specificity in the provision of nursing care before and after thyroid surgery.

After diabetes mellitus we encounter thyroid disease as the most common disease of the endocrine glands. The most common diseases of endocrine glands are inflammations, tumors, struma or thyroid enlargement, thyroid hyperfunction and hypofunction. Indications for surgery are among others malignant and benign thyroid diseases. In the postoperative care both general and specific principles are applied, which are conformed to the condition of the patient, the type of surgery and the surgeon requirements. Providing the right nursing care should also help the prevention of possible postoperative complications. The bachelor thesis named "Special features in providing nursing care before and after thyroid surgery" is divided into two parts, theoretical and empirical. The theoretical part of the bachelor thesis deals with thyroid anatomy and physiology and its surroundings, its need for iodine, it describes individual thyroid diseases, thyroid disease diagnosis and treatment. Furthermore, it also deals with the nursing care before and after thyroid surgery, the emergence of possible complications and last but not least it deals with home care after the patient is discharged from hospital. The aim of this bachelor thesis was to detect the pecularities of providing nursing care before and after thyroid surgery and to determine regime measures and the knowledge of these in patients after thyroid surgery at the surgical department. Research questions were created to achieve the objectives. Q1: What are the pecularities of the nursing care before thyroid surgery? Q2: What are the pecularities of the nursing care after thyroid surgery? Q3: What are the regime measures in patients after thyroid surgery? Q3: What knowledge do the patients have about the regime measures after thyroid surgery? To obtain necessary data a semistructured interview method was used. To process this thesis a qualitative research was carried out. Semi-structured interviews were used as a technique for data collection. Categorization was compiled on the basis of interviews. A research survey was carried out in the surgical ward, where there are patients with thyroid disease. The research showed that the nurses are aware of care principles and they are sufficiently informed about treating a patient with thyroid disease. The interview showed the principles of nursing care, which should be followed throughout the whole period of hospitalization of patients with thyroid disease. In the interviews the nurses emphasized the differences in providing nursing care in a patient before and after thyroid surgery. Furthermore the research showed that nurses would be in favour of further training seminars that would deal with the given issue, about treating a patient with thyroid disease. Awarness and knowledge of the regime measures was also recognized in patients after thyroid surgery. Based on the data obtained from the interviews with patients, those report that their awareness and also knowledge of regime measures after thyroid surgery is minimal and insufficient. The result of the bachelor thesis was to determine the principles of care for patients with thyroid disease. Principles of nursing care were detected in patients before and after thyroid surgery. Furthermore, regime measures were identified in patients after thyroid surgery and also the knowledge of those regime measures in patients after thyroid disease. This bachelor thesis outlines the pecularities in nursing care of patiens with thyroid disease, policy of regime measures after a patient's discharge from hospital and keeping to those is among others a condition for early patients recovery and his return to a normal life. Specific features of nursing care of obese patients with diabetes mellitus type 2

The occurrence of diabetes mellitus type 2 and obesity in the population has been continually increasing. This phenomenon has been obvious particularly among ever younger age groups. At the end of 2010 the number of registered type 2 diabetics in the Czech Republic was 739 859. The percentage of obese adult population in the Czech Republic is approximately 35 %. This means that the Czech Republic holds a leading position in obesity occurrence. Treatment of obesity and diabetes mellitus type 2 are based on the same principles. Meanwhile, weight reduction improves compensation of diabetes. The care of obese diabetics is highly specific and therefore requires a closer attention. It is important to emphasize which of the specific features of the care are the most significant. Specific features in the care of obese patient with diabetes mellitus type 2 include particularly education about dietary measures, physical activity and complicating conditions about which type 2 obese diabetics should be informed. The following two objectives were established based on the above-mentioned facts. The first objective was to map specific features of nursing care of obese patients with diabetes mellitus type 2. The second objective was to map the knowledge of obese diabetics about the regimen measures associated with their disease. Both the objectives have been met. The research efforts were divided into two parts. A qualitative survey was used to meet the first objective through a structured interview method. The interviews were conducted with seven nurses from the South Bohemian region involved in the care of those patients. The interview investigated specific features of the care of obese type 2 diabetics. A quantitative questionnaire survey was used to meet the second objective. The questionnaire was intended for obese type 2 diabetics. The number of distributed questionnaires for the research survey was 100. The purpose of the questionnaire was to verify knowledge of and adherence to regimen measures among the patients. The surveyed patients were selected in surgeries of specialist physicians, two hospitals and a senior citizen home in the South Bohemian region. The results have shown that patients are familiar with regimen measures associated with their disease and that they adhere to them. Further, the results have shown that specific features in the nursing care of obese patients suffering from diabetes mellitus type 2 include particularly education of nurses about dietary measures, education about physical activity and education about acute conditions connected with acute complications of diabetes and about anti-stress techniques or psychotherapy.The thesis will serve as a document monitoring specific features of the care of obese patients with diabetes mellitus type 2 and will help not only to students interested in the topic but also to all other interested individuals. Another output of the thesis is an information leaflet intended for type 2 obese diabetics.
Acquiantance of university students and adult epileptics with problems related to epilepsy

This work is primarily aimed at awareness of the academically educated students and epileptics of the problems related to the epilepsy. The first aim is to monitor, whether the academically educated students have the information about the epilepsy problems and primarily, whether they should lend the first aid to the epilepsy patients. The other aim was to find out the awareness of the epileptics of the regime measures connected to their diseases and whether these patients keep these measures. There were set two hypotheses. The first one was related to the awareness of the academically educated students of lending the first aid during and at the end of the epileptic seizures. The other one was focused on the awareness of the epileptics of the regime measures and whether they keep them. Both of the hypotheses were confirmed. Research was made in a quantitative way. The first group was formed of the epileptics older than 18 years, who were monitored in the neurological outpatients' department in the South Bohemia district. The other group was formed of the students of the Pedagogical Faculty of the University of South Bohemia in České Budějovice. It was found out, that the epileptics have the information about regime measures available, but that they don{\crq}t keep them. Further it was found out, that students of the Pedagogical Faculty have information on lending the first aid available
The education process for nurses in pediatric primary practice and parents of children with epilepsy

The epilepsy occurs in the whole world without exception of age, race, geographical location and socio-economic structure. The incidence of epilepsy depends on age and influences each time the quality of life of the child and its next relatives. Our children and young people perceive, experience and evaluate their life usually from different points of view than adults including the parents. For this reason the main target of the nurse effort should be the education and involving the family into the process of healing. Also the preventive and educational care is connected with this. The target of thesis was to map if the education by the nurse concerning the regimen measures has influence on knowledge extent of the family of an epileptic child. The work focused, as for quantitative examination, on nurses in the surgeries of physicians for the children and young people (pediatrists) in South Bohemia. By the technology of the questionnaire the possibilities of provided primary and secondary care for children with epilepsy by the nurse and the scope of the co-operation of the nurse with the clubs of epileptics were mapped. Within the qualitative examination, based on the talks with parents of the children with epilepsy, the contribution of the knowledge of regimen measures handed on through the education by nurses in the primary pediatric care was examined. It follows from the results of the quantitative examination that the nurses in the outpatient departments of pediatrists do not devote attention to primary or secondary prevention of epilepsy and simultaneously they do not co-operate with the clubs of epileptics. It was impossible to verify all three hypotheses. It followed from the results of the quantitative examination that nurses in outpatient departments of physicians for children and young people do not sufficiently educate the parents of children with epilepsy. If the education by the nurse in the pediatric primary care has been performed, it was a contribution for parents each time. The results of the thesis lead us to reflection concerning the role of the nurse as educator and simultaneously as defender of the client- child in the outpatient departments of pediatrists. The results of the thesis: I would like to address the nurses in primary care, that they should assert themselves more, educate themselves and get involved into the care for children with epilepsy and hereby help to children to improve their childhood in the way necessary for their healthy growth and development.

ABSTRACT This diploma thesis is focused on cardiac fitness and rehabilitation centres. The reason why I have made this choice for my thesis was the fact that despite all recommendations of professional associations dealing with cardiovascular disease issues, adherence to secondary prevention and treatment regimen measures is absolutely poor also in patients who had been given superior and costly care during their acute medical condition. Adherence to secondary prevention guidelines includes treatment regimens and measures to reduce risk factors. Theoretical part of the thesis lists risk factors highly involved in coronary diseases. An integral part in maintaining good physical condition of cardiac clients is residential spa treatment and following care in cardiac fitness and rehabilitation canters. Involvement of nurses in care for patients with coronary diseases would be just in the area of secondary care and regimen measure adherence. Use of their competencies and joining non-medical facilities addressing this issue could improve the gaps in secondary prevention. The goal of this thesis was to map out the patient requirements for cardiac fitness and rehabilitation centers within secondary prevention. To determine the difference in secondary prevention effectiveness between clients visiting cardio clubs and clients living in location without any cardio club. Another goal was to research the connectivity of cardiac clients in the follow-up care after their discharge from ward departments and possibilities of cardio clubs and in cardio club management, to find out the possibility to involve a nurse in cardiac client care. Qualitative research with semi-structured interviews and quantitative research relied on a questionnaire have been used in this study. Research group consisted of healthcare professionals and cardiac patients. Goals of the thesis have been met, research questions answered and hypotheses confirmed. The results can be served as a base for establishment of other cardiac fitness and rehabilitation centers and involvement of nurses. Output of this thesis is an encouragement for general nurses and can be used in risk factors of coronary diseases.
Role of a nurse in the care of patients with post-puncture cephalalgia after an operation in spinal anaesthesia

The objective of this diploma paper was to answer the question whether nursing for patients after an operation in spinal anaesthesia has any effects on the occurrence of post-puncture cephalalgia and on the satisfaction of patients with spinal anaesthesia. The paper is divided into two parts: a theoretical one and a practical research one. The theoretical part sums up the current knowledge of spinal anaesthesia; it provides anatomic and physiological aspects, possible complications and the importance of nursing in this type of anaesthesia. It deals with the syndrome of post-puncture cephalalgia in detail, the mechanism of its origin, typical symptoms, its treatment, and the importance of nursing for patients suffering from this syndrome. With regard to the importance of education in nursing jobs, one chapter of the theoretical part deals with education and educational process. The research part consists of qualitative as well as quantitative surveys. The research was carried out with a group of patients who were hospitalized at the surgical and gynaecological departments of Domažlická nemocnice a.s. (Domažlice Hospital Ltd.) and have undergone an operation in spinal anaesthesia. The quantitative survey was done using a questionnaire that contained 27 questions. Seventy-eight patients, selected at random, took part in this research stage. The objective of the qualitative survey was to find out the following: whether patients were aware of the aspects of spinal anaesthesia, whether they have experienced any complications in relation to the administration of spinal anaesthesia, how have they been informed, and what was their overall satisfaction with spinal anaesthesia. The qualitative part involved 10 patients who had been divided randomly into two groups: a group with ``standard{\crqq} education and a group with above-standard education using a purpose-made brochure. A semi-standardized interview, consisting of 18 questions, was carried of with each patient. The patients in the latter group were also asked additional questions aimed at the benefit of the informative brochure and the completeness of the presented information. The former group was asked whether they would be interested in a brochure focusing on spinal anaesthesia. Based on the results of this survey, the basic research question of the paper can be answered. Nursing does have positive effects on the occurrence of complications and on the satisfaction with spinal anaesthesia. A detailed result analysis shows that patients do not feel any subjective need to be educated by a nurse and they are not interested in the brochure. Most of the patients think they are informed sufficiently and they do not want more information. However, the results of the qualitative survey into the group educated by a nurse show a higher informedness about the method, lower occurrence of complications, higher satisfaction, better observance of recommended regimen measures, and highly reduced level of stress in relation to spinal anaesthesia. The educative brochure was accepted by the educated patients as beneficial and useful. Based on the above-listed results, the informative brochure, being an integral part of this paper, will be introduced as a standard education tool for patients undergoing an operation in spinal anaesthesia at Domažlická nemocnice a.s.
The influence of information on observance of the regimen precations in patients with deep venous thrombosis hospitalized in the internal ICU.

The aim of the thesis was to find out if the awareness of patients with deep vein thrombosis hospitalized in internal ICU influences the observance of regime precautions. The thesis is divided into the theoretical part and the part focused on the research. The theoretical part is focused on the data on the deep vein thrombosis: anatomy data, pathophysiology, causes, clinical signs, investigation methods, complication and treatment. Regime precautions are the necessary part of the treatment. Their observance influences not only easing the complaints, but also prevents the onset of complication development which might result in patient ´s death. A patient with deep vein thrombosis is hospitalized at an ICU of internal department. A part of the thesis is the description of the department and the description of patient{\crq}s admission to the department. Education, interview and observation are the part of nurse ´s work. That is why I described the bases of education, educational process and observation. Qualitative research was applied. The research was carried out at ICU of internal department in Český Krumlov. Research group was created by 8 patients; each second of them was educated in super standard way. In all patients the introductory interview was carried out. The aim was to find out if a patient understood the information given by a doctor, if he or she knows about the regime precautions and their reasons. Further, the part of the patients was educated in super standard way in the course of the whole hospitalization. For this education an educational plan and an information brochure were created. After that the observation of all 8 patients was carried out. The observation was divided into two parts; the plan of observation was created. In the first part called the period of immobilization, the record was carried out during the first three days of this period. This part was focused on the regime precautions concerning the rest of a patient in bed, hydration, elevation of an affected limb, application of cold compresses and movement activities carried out in bed. The second period concerning the first three days when a patient could leave the bed, it was observed if the patients keep the application of elastic roller bandages before they leave the beds, hydration and movement activities according to the doctor ´s recommendation. The aim of my thesis was fulfilled. The research question was aimed at a question if the patients{\crq} awareness influences the observance of regime precautions in case of the deep vein thrombosis. The results proved that the education of patients significantly influences the observance of regime precautions. These patients moreover ask and are interested in the problems of the illness. On the basis of research results hypothesis were stated: H1: The education of patients with deep vein thrombosis positively influences the observance of regime precautions in the period of immobilization. H2: The education of patients with deep vein thrombosis increases their interest in regime precautions at the discharge into domiciliary nursing. The results of the research will be used at the professional hospital seminar for nurses. On the basis of the research the educational plan focused on the regime precautions for the patients with the deep vein thrombosis and informative brochure for the patients were created. The brochure was adopted and started to be used at the education of patients with deep vein thrombosis.
