Nursing care of patients with stroke

This thesis is divided into a theoretical and an empirical part and it deals with nursing and patient care after a stroke. The theoretical part comprehensively summarized the issues related to stroke. It deals with the definition of the disease given, its causes, symptoms, diagnosis, complications and therapy. Very important part is the area of nursing care that due to the nature of the disease, carries many specifics and includes comprehensive care, which may include stroke centers. There were stated objectives for this bachelor thesis: 1. To identify the problematic areas when providing nursing care to the patient after a stroke. 2. To secure the patient´s satisfaction with nursing care. Based on these objectives there were asked two research questions: 1. What are the problematic areas in nursing care of patients after a stroke? 2. Are patients satisfied with nursing care? The empirical part of this bachelor thesis presents the results of a qualitative research. The form of a semi-structured interview and a hidden observation was used. The interview with five nurses working in the neurology department and with five respondents who have had a stroke was conducted on the basis of questions prepared in advance. When analyzing the results, the method of "paper and pencil" was used. The processed results were categorized and then split into subcategories. The hidden observation was focused on five nurses working in the neurology department. Observation results were recorded into an observation diary and eventually evaluated. All of the research questions asked were answered by realizing and processing the survey. The results of the survey point to a very good quality of nursing care provided in the neurology department. However, based on interviews and observations, some minor flaws occurred. One of them is the lack of respect for privacy and intimacy while providing nursing care and inadequate defecation reflex training. It was further found that nurses presented the rehabilitation as the most important part in providing nursing care, but they unfortunately did not take part in it. Another problem mentioned by nurses is the lack of time to be able to individually work on all interventions that have been chosen. As a great positive we can present big satisfaction of respondents with the approach of nurses in providing nursing care. Based on the result of this research an informative brochure was created. This booklet can serve to the nursing staff as a kind of summary of the principles needed to be held when giving nursing care about patients with the issues mentioned above. It can also be used as a teaching tool for students.
Factors influencing the satisfaction of patients with provided nursing care

Current state: The satisfaction of patients is an important factor for the quality of nursing care. Nursing care should be mainly focused on meeting the needs of patients and many other factors. This greatly affects the patient's subjective feeling of satisfaction or dissatisfaction with either the proper nursing care, or other factors. The patient is an active participant in the treatment, not a passive recipient of nursing care. Measuring the satisfaction of patients is one of the requirements for the accreditation of hospitals. Research objective: The aim of the research was to determine the satisfaction of patients with the provided nursing care. Another objective was to demonstrate a statistically significant relationship between the satisfaction with nursing care and the impact factors: sex, age, education, environment and the routine on a standard ward, communication with patients and nurses attitude of nurses to patients. Methodology: The research was realized using quantitative methods. For my research I used a standardized questionnaire called Quality seen by patients - evaluation of inpatient care, which included 34 questions. The questionnaire for the assessment of patient satisfaction was compiled into several areas: satisfaction of patients with the hospital stay, with the nursing care provided, with leaving the hospital, with the general assessment of the health care provider. The survey covered the period from January to March 2015 at the standard surgery and internal medicine department in the hospitals of České Budějovice and Český Krumlov. For the survey I selected the patients who were hospitalized in these departments for at least 3 days. The data obtained by a quantitative research were later tested by SPSS software. For statistical testing we selected the chi-square and the tick chart. The significance level was set at 5%. Participants: As part of the survey, a total of 200 patients were approached. 100 patients were interviewed in the hospital of České Budějovice in surgery and internal medicine departments. The remaining 100 questionnaires were distributed to patients in the hospital of Český Krumlov, again on the internal and surgical wards. The total return of the questionnaires was 100% (200). Results: The research shows that men are less satisfied with the received care than women. An important factor in patient satisfaction showed to be the education of the respondents. University-educated patients were significantly less satisfied with nursing care than respondents with secondary and primary education. Another examined factor was the sleep schedule. Approximately one fifth of the surveyed patients (22%) stated that noise caused by other patients was disturbing. The respondents who indicated that were disturbed by the noise in the department, rated the satisfaction with nursing care significantly worse. Conversely, the patients expressed satisfaction with the wake-up time. In the area of the patient satisfaction with the cleanliness of the rooms and toilets, the research survey showed that satisfaction decreases significantly if the patients are not satisfied with the cleanliness of the toilets and rooms. The environment, in which the patients are hospitalized, greatly influences their satisfaction with nursing care. Another important factor for the patient satisfaction is also the communication and the attitude of the nurses to them. If the attitude of the nurses is compassionate, soothing and they pass information to the patients in an understandable way, the patients have more confidence and are more satisfied. This is true when the patients meet their needs and feel that they are treated with dignity and respect. Conclusion: The research shows that these factors affect the satisfaction of patients with nursing care and in the context of these results the importance of highly individualized care is increasing. Nurses must significantly contribute to improving patient satisfaction.
Rating the quality of nursing care

Abstrakt The title of this bachelor degree thesis is ?Evaluation of quality of nursing care". There are many definitions of quality depending on the individual authors. However, all the definitions have one thing in common. It is the patient and his requirements that represent the main criteria of quality. The objective of the thesis was to find out about the satisfaction of patients with the provided nursing care at the gastroenterology ward. The objective has been met. The patient´s satisfaction is subjective and it is related to the ability to satisfy the patient´s needs. Satisfaction is measured against client´s expectations. If the expectations are exceeded the level of satisfaction is usually high; if the expectations are not met the level of satisfaction is low and the patient is frustrated. The research part of the thesis used a quantitative method. The survey consisted of a questionnaire developed based on information found in specialized literature. We formulated three hypotheses and after evaluation of research results all of them have been confirmed. The results of the research are shown in form of diagrams. The surveyed group consisted of patients hospitalizes at the gastroenterology ward in the hospital in České Budějovice (Nemocnice České Budějovice a. s.). The research has shown that most of the patients are satisfied with the provided nursing care. In practice the results of the thesis may be used for publishing in specialized literature. The contribution of the thesis consists in the mapping of satisfaction of patients with the nursing care provided at the gastroenterology ward and the results of patient satisfaction represent an excellent feedback for the nursing stuff. The findings from this thesis will be provided to the management of the gastroenterology ward and they should serve as an input for reflections on the nursing care.
