Coordinated rehabilitation in a child with cerebral palsy

The aim of the submitted diploma thesis was to describe the influence of coordinated rehabilitation on the overall development and health status of a child with cerebral palsy. The partial goal was to find out to what extent the intervention, through coordinated rehabilitation, affected the life of the child and his family. The work is divided into two parts. The theoretical part deals with the current state of affairs, mentions the associated diseases accompanying this disease, characterizes the various components of the system of coordinated rehabilitation and informs the reader about the possibilities of using various methods and treatments that allow children with cerebral palsy and their families to improve life with this healthdisadvantage. It acquaints the reader with an outline of the psychological impact of changes in health status on the personality of a child with cerebral palsy and explains their impact on the quality of life of the whole family. The practical part acquaints the reader with the course of research and its results. Through my own research, I tried to verify the knowledge that I gained by studying the literature in the processing of the theoretical part of the thesis. I obtained data with the help of methods and techniques of qualitative research. Through a semi-structured interview conducted with the informant's parents and with himself, I helped readers to look into the life of a family with a child with cerebral palsy, to present their daily joys, problems and pitfalls they face. In many places, I also inserted analytical excerpts from the medical documentation into the text of the practical part to show the severity of the disease, findings from my own observations and work as a therapist. Based on this knowledge, I created a case report of the user. With the done research I had a chance to make sure how important a fluent continuity of particular parts of a coordinated rehabilitation for a patient is, that it is necessary for minimalizing consequences of a disability and for improving life of these individuals and their families. I reached an opinion that despite the best care of all specialists, we can not forget the limits which a certain individual has and that everything does not have to develop in the way we would expect it to. During the research I also found out that it is often very problematic to assemble a team of 5 specialists that is willing to cooperate effectively. However if this research could contribute to a better understanding of a specific problematics of a disease and reach a better quality care of children with cerebral palsy and their families, this research would meet its purpose. I believe that this diploma thesis can serve not only experts, but also students to better orient themselves in such a broad field as coordinated rehabilitation really is.
Co-ordinated rehabilitation after acquired brain injury

The thesis focuses on coordinated rehabilitation with persons after acquired brain injury and they are healing in rehabilitation institute in Kladruby. Purpose of this work is to describe total health and social situtation of adressed clients, which began them and their family after acquired brain injury. The thesis is divided into two main parts - the theoretical part & the research part. The teoretical part includes familiarization with the topic of the diploma. It is divided into seven main chapters. The first and the second part include divison, description and function of the nervous systém & of the brain anatomy. The third chapter is focused on acquired brain injury and its division into strokes, traumatic brain injuries a non-traumatic brain injury. The fourth chapter explains coordinated rehabilitation and its parts - therapeutic, social, educational and work rehabilitation. The fifth chapter theme is preventive function of the rehabilitation, which is very important part of human health either and it is closely connected with risk factors. The sixth chaptor divides individual rehabilitation facilities. The seventh chapter theme is the care in rehabilitation institute in Kladruby. It is described acquaintance with history of the insitute, acquintace with present of the institute and acquaintance with the vision of the rehabilitation care in the future in this rehabilitaion institute. Chapter includes division of complex care provided in rehabilitation facility. The research part includes given goals & research questions. The research and the diploma are supported with agree of ethics comittee of rehabiliation institute in Kladruby. The main goal of this work is to describe basic health and social problems at addressed clients. Partial goal is to identify the most beneficial component of coordinated rehabilitation during the visit the rehabilitation facility. Two research questions are established following research goals. The first question is to find which limits addressed clients perceive as the most essentials. The second question is to describe the intervention which are rated by clients withing the rehabiliation as the most beneficial. In the theoretical part is discribed, which method is the work processed and which methods was used. It explains the choice of research sample of clients, thanks to them, this work is writed. In this work is used the method of qualitative research. The research file is selected by deliberate selection of clients and it is created by four clients - the two women and the two men. All of the addressed clients were acquainted with goals of this work, with the research questions and with possible acquisition of this work in healing the persons after acquired brain injury. The techniques of the data collection is semi-structured interview. The interviews were realized in quiet ambience during the healing of clients in rehabilitation institute. The analysis of the interviews were processed in Atlas.ti 8 software. Resultant diploma informations describe and explain the health and social problems, which are formating to the clients after the brain damage and which way the problems influence their actual life. Next, the results identify priorit part of the coordinated rehabilitation and the sigle components of the complex rehabilitation. It is focused closely on the main part of healing rehabiliton, these are physiotherapy and ergotherapy. The results describe healing in rehabilitation institute in Kladruby, they show importance of the coordinated rehabilitation for persons after brain damage and suitability of rehabilitaion facilities, where is complex care of clients realized.
The use of Arttherapy in coordinated rehabilitation

This master's thesis deals with the issue of coordinated rehabilitation and the issue of art therapy as its component. Coordinated rehabilitation is an interrelated process of therapeutic, pedagogical, occupational and social rehabilitation, whose main goal is to minimize the consequences that arise from an individual's disability. Art therapy is a therapeutic and cultivation process using psychological and artistic means to impress the client. Successful rehabilitation is based on early, smooth and coordinated efforts to involve persons with disabilities in everyday activities of life as quickly and as widely as possible. This activation can also be achieved by involving art therapy. The theoretical part consists of two basic chapters. The first chapter deals with the concept of coordinated rehabilitation and its theoretical level. The following subchapters deal with the history of coordinated rehabilitation, international classification of functional abilities, disability and health, as well as topics such as disability, disability classification, multidisciplinary up to health team, as well as social and occupational rehabilitation. The second chapter describes art therapy and its theoretical definition. Afterwards, art therapy will be described in terms of components of coordinated rehabilitation and its concept or legal definition. The aim of this thesis is to find out how can art therapy in the system of coordinated rehabilitation contribute to successful rehabilitation of the client. For the empirical part a qualitative research method was chosen. This type of research was chosen due to the limited number of respondents and in order to gain new knowledge in the above mentioned disciplines. The obtained results can be used to create a feedback tool in the research facility, which can then be used to improve the quality of care provided. This thesis can be further used as a source of information for future clients and their families.
Rehabilitation of patients with spastic form of Infantile Cerebral Palsy from the perspective of physiotherapy

The theoretical part describes the introduction to the issue - the definition of the correct terminology, and problems of cerebral palsy - its characteristics, prevalence, classification, diagnosis, etiology and appropriate intervention. The practical part is focused on physiotherapy intervention. The aim of the thesis is to determine the impact of selected interventions on quality of life of people with cerebral palsy. The Use of a Coordinated Rehabilitation in the Organisation of the Focus

The diploma thesis is dedicated to the way of using and way of functioning of the coordinated rehabilitation and the multi-disciplinary team in the organization Focus in Pelhřimov which works with the people with mental disorders. The thesis is divided in the theoretical and practical part. The theoretical part describes the history and function of the coordinated rehabilitation, mental health care history, psychiatric reform and multi-disciplinary cooperation functioning. There is used the strategy of quality analysis in the research part. It was done by the questionnaire and the technique of half-structured interview. The results coming out of the interview were focused on the functioning of multi-disciplinary team in the institution and the cooperating subjects in the first part. The second part was dedicated to the basic parts of the coordinated rehabilitation provided in the organization in view of the fact that there were other participating professionals and non-formal participants during the care of the person with mental disorder. The aim of the research was to map the sectors of the coordinated rehabilitation used by the individuals with mental disorders in the organization Focus in Pelhřimov. The research team was created with the help of the method of the snow ball and basic criteria was employment of an "communication partner" in the particular organization. Their ability to evaluate their job and take part in the lead interview depended on their experience in Focus. In connection with the thesis aim there was also set the main research question: What parts of the coordinated rehabilitation are used in the organization Focus in Pelhřimov? Partial questions were devolved from that which were connected to the particular parts of the coordinated rehabilitation and the function of the multi-disciplinary team. The first contact and the next cooperation is developed by the seriousness, necessity and wishes of the client. The work with the client with mental disorder is based on the philosophy of recuperation, not recovery. In the relationship and the concentration, the work is done in all sectors of the coordinated rehabilitation. The client has the closest relationship to the key worker who provides the care in the multi-disciplinary team and other professionals in the organization. There is the conclusion from the results that there was an agreement that the coordinated rehabilitation is used in this institution and no other from the basic sectors can be omitted. The results of the diploma thesis can be used as feedback for the organization Focus in the practice where it can be useful for mapping of the sectors of coordinated rehabilitation and making clear the needs of the individuals with mental disorder. Taking into consideration the recommendation to use the feedback of the respondents about information sharing, to prefer common space of the whole team and bigger promotion.
Coordinated rehabilitation for clients with multiple sclerosis

The topic of my Bachelor Thesis is the coordinated rehabilitation of clients with multiple sclerosis. I was concerned with the state of knowledge of this method and with the experience of clients with the procedures. The main goal was to learn which aspects of the coordinated rehabilitation are for the clients most protable. Next, I endeavored to nd out how the clients were informed on the method, and which of its parts were typically being undervalued. The text is divided into two parts: the theoretical one, and the report on the practical research. The first part of the Thesis is devoted to the current state of diagnostics and medical treatment of multiple sclerosis, and then also to the state of the coordinated rehabilitation. The individual parts of the method are being discussed, and in some of them also the legal limits are mentioned. In the eld research I decided for the qualitative investigation strategy. My data collection was based on a semi-structured dialogue. The respondents were selected by the so called "snowball method": that is, the sampling of the next respondents was based on recommendations of the already questioned ones. It was essential that all the respondents had been already previously diagnosed for multiple sclerosis. In the practical part of the text I present and discuss the results of my research. It has turned out that although some special parts of the procedure are well known (and also well exploited and generally used), the general knowledge of the method is very low. Thus, the full potential of the coordinated rehabilitation is not satisfactorily taken advantage of. The Thesis can be used as a source of information for the clients with multiple sklerosis and for their families, and also for the people working in the individual stages of coordinated rehabilitation. The results can serve as an aid for enhancing the the knowledge of the clients.
Comparison of selected aspects in the coordinated rehabilitation in Czech children's homes and in German children's homes

In the provided care for children placed in substitute residential facilities in the Czech Republic are in the context of European Union made demands to change some key aspects of its current structural frame. Aim of this master thesis is to provide in this context, through the analysis of mainly bibliographic sources, a complex overview of the system of substitute residential care in two EU countries, the Czech Republic and the Federal Republic of Germany. The thesis contains an insight into the activities of both systems, as well as comparatively processed answers to two initial research questions. The results of the research work on the answer to the question "What are the differences in the existing service network, regarding the work with the children placed in selected Czech and German children's homes?" show, that the established frame of the provided services is similar, but the qualitative and quantitative offer of the services is in practice reflected by different economic situations in both countries. The result of the second comparative research, answering the question "How are in the chosen countries determined the means of children rehabilitation in the selected children's homes?", shows, that the coordinated rehabilitation is in all its parts realized in both countries as consistent with the fulfilment of "the child's best interest" as possible. The most significant difference in reaching the optimal harmony of all segments of coordinated rehabilitation in these countries makes the application of support means to the work with the family, to the care for youth preparing for leaving the institution, to the realization of complex multidisciplinary care, or to fulfilment of an individual approach and planning. It is therefore desirable, that the Czech Republic, as well as the Federal Republic of Germany, orient themselves not only on the quality of provided help and care for the children placed in children's homes, but also on the future of young people, who are in the process of leaving the institution. This can be achieved by legislatively well-set structural system of substitute residential care, in which is through suitable service provided a valuable support in the context of coordinated rehabilitation. This master thesis can for the people interested in this topic expand the view on how the same mission regarding the children in substitute residential care is fulfilled by two advanced European countries, it can also be in this context an impulse to reflect on the form of a good practice in these institutions.
Possibilities of therapies in children with DMO applied in a special kindergarten in the Vysočina Region and in the South Bohemian Region.

This bachelor thesis deals with options of available therapy for childrens with cerebral palsy in chosen special kindergartens, where the working style is based on coordinated rehabilitation. The goal is to find out which therapies are offered by the special kindergartens and how these therapies work. In the theoretical part, I am focused on definition of a disabled person, especially person with cerebral palsy and also on the system of coordinated rehabiitation. In the practical part I describe institutions such as Kindergarten in ARPIDA center and Kindergarten and SPC Jihlava, and also research complex and therapies which are in use. During the research I used method of observation and semistructured interview with chosen children's parents, therapists a pedagogues from special kindergarten. Results of my thesis delineate options of therapies which are used for children with cerebral palsy and the way these therapies might how it work in Kindrgarten in ARPIDA center and Kindergarten and SPC Jihlava.
Koordinovaná rehabilitace z pohledu pacienta po poškození mozku a členů multidisciplinárního týmu

Abstract This diploma thesis aims to describe coordinated health rehabilitation of people after severe brain injury provided by multidisciplinary team within home environment. It comprises both theoretical and practical part. The theoretical part presents the history and development of coordinated rehabilitation services and the function of multidisciplinary team. Furthermore, it characterizes a brain injury in more details. The practical part was carried out by means of qualitative research strategy semi structured interview. The first part of the research was aimed at the interviews with clients after brain injury and cerebrovascular accident. The interviews started after three months from entering the project and were realized by social workers. Its main propose was to evaluate the care of multidisciplinary team. Clients were questioned about work, time, communication and co-operation of individual experts involved in the process of recovering. The second part of the research used a method called focus group, where 12 members of the team were involved. The goal of the research was to describe how the multidisciplinary team works from both the point of view of a patient after the brain injury and the members of the multidisciplinary team within the project GAJU. The GAJU project is entitled "Coordinated rehabilitation of patients with brain injury (reg. number GAJU 138/2016/S)". The selection of patients was based on the communicative skills criteria e.g. skills to participate in the directed conversation based on the FIM and WHODAS and the follow-up questions concerning social issues, physiotherapeutic and ergo therapeutic methods and techniques. To achieve the aim of the diploma thesis two research questions were defined: "How is the function of multidisciplinary team perceived by clients involved in GAJU project?" and "How is the function perceived by the members of multidisciplinary team involved in GAJU project who put the coordinated rehabilitation into practice?" It was found out that after the discharge from a hospital being with family members was the most important. Besides, multidisciplinary team who regularly commuted to clients helped them. The members of the team were physiotherapists, ergo therapists, social workers and doctors. In view of the fact that physiotherapist attended the client the most frequently, the function of this expert is perceived as crucial during recovery. No less important is ergo therapist who is useful concerning motor activity and presenting various tools and aids leading to improvement. Social worker is apprehended rather as a psychological or informative support. Positively valued is attending clients at home, interconnection of the team members and the progress of the client from all points of view. On the other hand the lack of working experience of some team members is pointed out. Additionally, some clients were not satisfied that the data were collected by means of questionnaires.
Possibilities of Implementing the Concept of Coordinated Rehabilitation of People with Alzheimer'S Disease

This thesis is focused on concept of coordinated rehabilitation within individuals in care of Alzheimer's disease. Objective of the thesis is to find out the possibilities of coordinated rehabilitation in the current concept of care for people with Alzheimer's disease. The thesis divided to two parts. The first part is focused on Alzheimer's disease, which describes what Alzheimer's disease is, its history, course, epidemiology, its risk factors, symptoms of how Alzheimer's disease is diagnosed and how it is treated. Another part of the theoretical part is dementia, where it is briefly described, what is the disease and other forms and division of dementia. The third and final part of the theoretical part is coordinated rehabilitation. This part describes what is a coordinated rehabilitation, its individual components and the conclusion of a specific activity that is used in a coordinated rehabilitation for people suffering from Alzheimer's disease. The research part of this work is focused on its objective and research questions, the methodology, results, discussion and conclusion are described. The qualitative research method was used in the research part. The technique for obtaining data for this research was a semi-standardized interview that was carried out with residential and outpatient staff providing services to people suffering Alzheimer's disease. The informants were selected by the method of deliberate selection. The data was then processed in Atlas.ti 7. In order to achieve the objective of this work, which is already mentioned above, two research questions have been determined. The first research question was focused on the differences in perception and the possibilities of using components of coordinated rehabilitation in residential and outpatient care for people with Alzheimer's disease. The second research question was focused on how the system of coordinated rehabilitation for people suffering Alzheimer's disease is most limited. The results of my work show that care for people suffering from Alzheimer's disease is insufficiently applied within the coordinated rehabilitation because the vast majority of practitioners who deal with such ill persons every day have no knowledge of this concept.
