Which aspects motivate users of exchange programs to treat

The aim of the bachelor thesis entitled Which aspects of exchange programs lead their users to treat is to introduce the importance and role of contact center workers in the life path of an addicted (drug addicted) client. Further focus on aspects of their treatment process through harm reduction, which can lead to lifestyle changes and associated abstinence. This was done by the testimonies of two k-center workers in the South Bohemian region, who helped to map their activities and consequently the difficulties of dependent clients. The theoretical part describes the nature of dependence, the concept of motivation and related terms such as motivational interviews and case management. This section also includes types of facilities that provide services to drug addicts (hospitals, therapeutic communities, aftercare centers, substitution treatments). The practical part contains qualitative research, which was created from interviews with four workers of k-centers. Followed by a discussion of the results obtained and the conclusion of the thesis. Conception of case management in terms of mental health center

The main goal of the Strategy for the Reform of Psychiatric Care, published in 2013 by the Ministry of Health of the Czech republic, was to improve the quality of life of people suffering from mental illness. One of the results of this strategy was the emergence of new mental health centres. These mental health centres are meant to provide individualized social and health care to people with mental illness in order to prevent or minimize their hospitalization. The service is ensured by multidisciplinary teams which include professionals such as psychiatrists, psychologists, psychiatric nurses, and social workers. Case management is supposed to be a dominant form of the service provided. The theoretical part of the thesis describes the historical context of the case management and its recent exploitation in the mental health centres in the Czech republic. The empirical part focuses on a particular multidisciplinary team applying the case management method. Through qualitative research, it aims to identify the different parts of the case management and the constraints and obstacles of its application. The analysis of the data collected brings the definition of the key problem areas the team is facing. The current practice of community outreach teams for people with severe mental illness

Community mental health teams common in many countries with developed community mental health care have been slowly advancing in recent years. It is expected that this trend will continue to grow with the reform of psychiatric care. The diploma thesis focuses on community mental health teams for the target group of people with severe mental illness, which are one of the basic pillars of mental health care abroad. The theoretical part describes the activities of these teams in the context of community mental health care and also presents these teams mainly in terms of their composition and way of work. The research part analyzes the current situation of four specific teams in terms of multidisciplinary composition and team cooperation, describes and interprets some of the specific difficulties that these services face in the beginning of multidisciplinary teamwork and captures some specific moments of their functioning. Utilizing a qualitative approach through in-depth interviews with team leaders and their founding organizations has enabled them to convey the specific context of these teams and at least partially capture the dynamics of their current state and development. The goal of the thesis is to answer the questions that come from author's professional experience in home hospice care. Could a social worker, already a part of a multidisciplinary team, be a coordinator of a case to be entrusted with a leading role in a case? Would a multidisciplinary team and a client benefit from a social work method case management which appears as a connector of health and social services? What should be the goal of a social worker as a coordinator of a case in home hospice care? The thesis begins briefly with specifics of home hospice care and defines relevant vocabulary concerning multidisciplinary team. The principle of cooperation of a multidisciplinary team in home hospice care is a complex view of a client and his/her quality of life and his/her close ones. The thesis also addresses the benefits and a role of a social worker as a profession within a multidisciplinary team of home hospice care. The aim is to define and show the necessity and a position of a social worker in multidisciplinary team of home hospice care. Further, it also touches a case management method its use in home hospice care, its advantages, and disadvantages; it states the duties of a case coordinator. Finally, the paper compares two home hospice care providers in Vysocina County with the findings described above.

Case management in multidisciplinary care of family with disabled children

Summary: The main objective of this thesis is to describe the limits of existing methods and case management in providing health and social services in the Czech Republic of multidisciplinary care for a family with a disabled children. The work is divided into two parts. First - theoretical part - summarizes the theoretical issues, the basic terminology, case management, case manager, describes the role and method of use of case management in social and health services in the Czech Republic and abroad. There is also summarized the issues of families with disabled children, concept of multidisciplinary care for families with disabled children and the legislative framework governing the health and social care for these entities. The second, empirical part, qualitative research focuses on mapping the experience of families with disabled children in the multidisciplinary care of their children in practice, indications, course, professional representation of multidisciplinary care.
