Colorectal carcinoma (epidemiology, prevention, diagnostic and treatment)

The Czech Republic has been leading the world order of incidence and mortality of colorectal carcinoma for a long time. Despite of the fact the prevention strategy is elaborated very well, we still can't manage to diagnose most of the tumors in early stadium, when the prognosis of disease is the best. There are diagnostic and treatment of colorectal cancer on the high level in Czech Republic, but its prognosis depends on an individual patient and his approach and responsibility to come to the doctor in time with the first symptoms.

Téma své bakalářské práce Ošetřovatelská péče o pacientku s divertikulitidou jsem si vybrala proto, jelikož mi je toto téma nejbližší, vzhledem k oddělení ARO, na kterém pracuji. Nejčastější skupinu pacientů zde tvoří právě pacienti po operaci střeva. Divertikulitida ovšem není z těch onemocnění, které by se na našem oddělení vyskytovalo často, a proto jsem měla zájem, dozvědět se o tomto onemocnění nové informace. V této práci se budu snažit uvést nové poznatky do praxe a pomoci tím pacientce zjednodušit a zpříjemnit její pobyt v nemocnici a zařadit se znovu do života jako před operací. Mým cílem bylo zpracování případové studie u pacientky S.S., která byla hospitalizována na oddělení ARO po operaci střeva pro komplikace divertikulárního onemocnění tlustého střeva. Bakalářskou práci jsem rozdělila na tři hlavní části - klinickou, praktickou a ošetřovatelskou. V klinické části popisuji anatomii, fyziologii, patologii a patofyziologii tlustého střeva. Věnuji se podrobně tlustému střevu, postiženému divertikulárním onemocněním. Jeho výskytem, etiologií, patogenezí, klinickým obrazem, komplikacemi, vyšetřením, léčbou a prognózou. Nursing Care of the Patient with Colorectal Cancer

Cílem mé bakalářské práce je zpracování případové studie ošetřovatelské péče o nemocného, který byl hospitalizován na chirurgickém oddělení v jedné z pražských nemocnic s diagnózou karcinom recti. V této studii jsem se věnovala 40-ti letému pacientovi, který byl přijat k resekci rektosigmoidea. Pacient souhlasí s tím, že informace o jeho nemoci budou použity při zpracování této práce. V klinické části se věnuji charakteristice onemocnění, anatomii a fyziologii tlustého střeva a rekta. Dále se zabývám patofyziologií nádorů, klinickým obrazem nemoci a základními vyšetřovacími metodami vedoucí k diagnóze kolorektálního karcinomu, prognózou a možnostmi prevence. Dále jsou zde uvedeny základní údaje o pacientovi, údaje z lékařské anamnézy a vyšetření, která byla rozhodující indikací k operačnímu výkonu. Následuje stručný průběh hospitalizace. Ošetřovatelská část zahrnuje stručnou charakteristiku ošetřovatelského procesu, zhodnocení nemocného podle modelu Majory Gordonové, stanovení aktuálních a potencionálních diagnóz, realizaci a hodnocení ošetřovatelské péče. Informace o nemocném jsem zaznamenala do formuláře zpracované pro potřeby výuky v oboru ošetřovatelství na 3. LF UK v Praze a ošetřovatelskou anamnézu jsem zaznamenala do formuláře z nemocnice ÚVN. Diverticular Disease at the Beginning of the Millenium

Diverticular disease, colon cancer is a disease with a relatively short history and the rapid development of expert opinion among gastroenterologists and surgeons regarding its pathogenesis, conservative and surgical treatment procedures and , last but not least, prevent its occurrence. It is a disease whose frequency occurrence in the course of the twentieth century significantly increased along with the development of society and lifestyle changes. While at the beginning of the twentieth century was disease is a rare finding at autopsy , at its conclusion and the beginning of the century twenty-first became a major surgical gastroenterologickým problem. Often when I search articles hit the front on the claim experts , it is a "disease or problem of the twentieth century ." As the development of knowledge about the disease due to the rapid progress of science Rapid and information often appear to be fragmented and incomplete , as the task this thesis I have chosen to describe the current state of the most recent major knowledge of the disease in the form of recent research articles , literature and charting the historical development of the main findings of pages pathogenesis, treatment conservative and surgical procedures.
