Hypertensive organ injury on the basic of 5/6 nephrectomy in REN2 transgenic rats

Hypertension is a major determinant of rate of progression of CKD. Treatment of systemic hypertension is now generally accepted as the most important clinical strategy to slow the progression of CKD to ESRD. Renin-angiotensin (RAS) and endothelin (ET) systems are important contributors to the pathogenesis of hypertension. The interaction between the RAS and ET systems also plays a crucial role in the development of hypertension- associated end-organ damage. Antihypertensive drugs that block the renin-angiotensin system (RAS) are now believed to provide class- specific renoprotective actions beyond their blood pressure lowering effects. Our first study was performed to compare the organ-protective effects of a RAS-dependent and a RAS-independent antihypertensive therapy in heterozygous (mRen-2)27 transgenic rats (TGR) after 5/6 renal ablation (5/6 NX). Normotensive transgene-negative Hannover Sprague-Dawley (HanSD) rats after 5/6 NX served as controls. RAS-dependent and -independent antihypertensive therapies induced equal reductions of BP and almost identical cardio- and renoprotective effects on the progression of CKD after 5/6 NX, in both HanSD and TGR. These findings suggest that the cardio- and reno-protection, at least in rats after 5/6 NX, is mainly mediated through BP- dependent mechanisms....
Implementation of Immunohistochemical and Molecular-Genetic Methods in Differential Diagnosis of Urogenital and Gynecologic Tract Lesions

This thesis focuses on gynecopathology. It consists of a collection of seven papers published in pathology journals with impact factor. Introduction section contains selection of examples showing scientific application of molecular genetic methods. Further on the aims of individual research projects are described. The first project comprises histomophologic study of skin endometriosis addressing "mullerian" differentiation. A case report of a rare tumor namely borderline papillary serous tumor of the fimbriated end of the fallopian tube follows with molecular genetic analysis of KRAS, BRAF and p53 gene mutation status. Prospective longitudinal study on high grade squamous dysplasia (HSIL) of the cervix in HPV vaccinated women, so called DAV (dysplasia after vaccination), aims to elucidate pathogenesis of this phenomenon. Two other studies focus on incidence of fumarate hydratase deficient leiomyomas of the uterus and hereditary leiomyomatosis and renal cell carcinoma syndrome (HLRCC). The aim of those studies is to improve our diagnostic capability and increase detection rate of the patients with HLRCC syndrome. Finally a new subtype of HSIL namely bizarre cell dysplasia is described in two separate studies. Conclusion remarks contemplate the role of molecular genetics in surgical pathology.
Unusual tumors of the skin and soft tissue

This doctoral thesis describes unusual skin and soft-tissue tumors, which were the basis of the postgraduate study of Ladislav Hadravský, MD at Medical faculty in Pilsen of Charles University in Prague during 2013 - 2016. It contains documented cases of skin and soft-tissue tumors related to hereditary syndromes, unusual morphology, rare biological behavior, minor causal association with the respective disease, or different phenotypes. These cases were published in journals with the impact factor and in peer-reviewed journals. Regarding skin tumors, the study focused on sebaceous tumors of the skin, which may occur within Muir-Torre syndrome. In the retrospective study of sebaceous skin tumors, two unusual cases were found: the case of aggressive extraocular sebaceous carcinoma on the scalp in a patient with Muir-Torre syndrome and the case of multiple sebaceous skin tumors in a patient with MUTYH-associated polyposis of the colon mimicking Muir-Torre syndrome. As far as soft-tissue tumors are concerned, the study aimed at the morphological comparison of cases of myxoinflammatory fibroblastic sarcoma and pleomorhic hyalinizing angiectatic tumor.
Tumors of Urogenital Tract, Molecular Genetic Characteristics and Impact on Biological Behavior

The Ph.D. thesis is a collection of seven commented articles on the topic of molecular-genetic of urogenital tumors. The spectrum of urogenital lesions is very wide, there is lots of new insights in field of urogenital pathology. Therefore, presented Ph.D. thesis is focused only on small part of urogenital neoplasms - renal cell carcinomas characterized by papillary growing pattern. The introduction presents different entities with papillary morphology, there are specified basic morphological, immunohistochemical and molecular-genetic characteristic of these lesions, their differential diagnosis is commented. In the results, there are presented recently published studies dealing with newly described entities of renal neoplasias, heterogenity of molecular genetic studies in individual tumorous groups is established, prognostic significance is commented, typical pitfalls are introduced. In the conclusions, the limitations of the use molecular-genetic methods are discussed and clinical outcome and significance is evaluated.

Adipocytokines are a group of adipose tissue-derived cytokines that have been discovered since early nineties. The role of selected adipocytokines in the prostate carcinoma and in selected joint diseases was evaluated. In the first part of this work the role of adiponectin and resistin in the prostate cancer development and progression was examined. Immunohistochemical expression and serum levels of these adipocytokines were evaluated in both groups of patients with benign prostate hyperplasia and prostate cancer of pathological grade pT2 and pT3, respectively. Moreover, selected metabolic, biochemical and anthropometric parameters were included into this study. Serum adiponectin levels did not differ between prostate benign hyperplasia and prostate cancer generally, but they were significantly higher in pT3 in comparison to pT2 group. In agreement with the results of serum levels, tissue immunohistochemistry showed enhanced staining intensity of adiponectin in neoplastic glands and to a lesser degree in prostatic intraepithelial neoplasia while within benign prostate hyperplasia the immunohistochemical staining was generally low. None relation of adiponectin expression to disease grade or stage was observed. In resistin no difference between benign hyperplasia and prostate cancer in respect to serum levels...
