Impact of pharyngofixation and timing in cleft palate repair surgery on the development of chronic adhesive otitis media and cholesteatoma in cleft palate patients.

Introduction: A significant number of pediatric cleft patients suffer from OMS and its consequences as hearing loss, adhesive retraction changes and cholesteatoma. The aim of our retrospective study was to find out whether these effects can be minimized by selecting an operative technique to correct the palate - using pharyngofixation or by surgery timing . Material and methods: Retrospective study involving163 patients - 325 ears (1x external auditory canal atresia), operated at the age of 5 months - 8 years, follow up time 36-84 months. Results: 125 (38%) ears had an epitympanic retraction and 45 (14%) had a superior posterior quadrant retraction, 10 (3%) had cholesteatoma, together 5 epitympanic and 5 sinus cholesteatoma. No significant relationship was found between the use of pharyngofixation and the severity of the otologic finding, respectively of retraction rates in the epitympanum and the posterior superior quadrant (p 0.53). Analogous no significant relationship was found between the surgery timing and the severity of the otologic finding (p 0.48) Conclusion: In our findings, the use of pharyngofixation and timing of palate correction surgery is not related with the severity of subsequent otologic findings. Surgical treatment of retraction pocket as a prevention of secondary cholesteatoma

The topic of his thesis Retraktion pockets as Prevention of secondary cholesteatoma I chose based on their interest in this issue, resulting in the third degree Medical School. The issue of retraction pockets and cholesteatomas considered significant due to the relatively frequent occurrence, with no early diagnosis and appropriate treatment can lead to serious complications such as hearing loss, destruction temporal bones, etc. While the most common cause of hearing loss in infants and younger children with cleft lip and palate is very clearly an inflammatory process otitis media secretoria with an incidence of approximately 90% in older children, adolescents and adults dominate rather its consequences in the form of chronic otitis, and adhesive - retraction processes in 20-40% or perforation of the tympanic membrane and mainly the most serious of these, cholesteatoma 2-10%.
Surgical treatment of retraction pocket as a prevention of secondary cholesteatoma

The dissertation deals with acquired (secondary) cholesteatoma. It arises from a retraction pocket. The process is conditioned on tubal dysfunction that leads through secretory otitis media to the development of retraction pocket and then may result in chronic otitis media with cholesteatoma. Cholesteatoma is a pearl shaped formation located in the middle ear cavity with aggressive behavior to surrounding structures. It may lead to serious life-threatening complications. Therefore all measures preceding its formation are important. These measures concern: follow-up of risk patients, appropriate treatment preceding the development of acquired cholesteatoma or therapeutic operation of uncontrollable or problematic retraction pocket, performed as tympanotomy, atticotomy, or atticoantrotomy according to the size and localization of the pocket as a prevention of the formation of cholesteatoma. The dissertation is completed with a group of patients who underwent middle ear surgery at the ENT clinic of FNKV. This group of patients has a high proportion of patients with cleft (44%), often connected with tubal dysfunction and so they represent a risk group and preventive measures are justified.

The thesis is focused on the marketing communication on a chosen secondary technical school. The thesis comprises of a theoretical and a practical part. The aim of the theoretical part is to set up marketing terminology and to describe its use in the given school system. Comparative analysis and synthesis of the literature resources were used to build the theoretical part. The practical part is focused on the evaluation of the marketing communication of the chosen secondary technical school. The data were acquired using questionnaire aimed at the first year students of the chosen secondary technical school. The aim of the questionnaire was to mine the knowledge on the decision making process of the secondary technical school choice; what contributed to the choice and how the respondents perceived the chosen secondary technical school marketing communication. The data were evaluated using simple statistical methods. The final outcome of the theoretical and practical parts was a definition of a set of recommendations for the applied education, which was designed based on the questionnaire data.
