Nursing care of the woman with the threat of premature birth

Abstract Premature labour is still a serious problem in Czech healthcare system. The rate is 5-6 %. It has not only health but also family, social and economic impact. A healthy mother and physiological labour of a newborn are the basic goals of perinatal medicine and the whole perinatal care. According to the literature the most common complications during pregnancy are epidemiological, iatrogenic and serious health factors of a mother. Although we know the most common causes of premature labours, we do not talk enough about nursing of women with these problems. The effort of the thesis is to bring it closer. The topic of the thesis is processed on the basis of a qualitative research by means of non-standardized interview and five casuistries, which were processed by means of a functional model by M. Gordon. Everything proceeded at the department of risk pregnancy in ON Příbram. The research group was created by 5 pregnant women in various weeks of pregnancy who were concerned with the problems of impending premature labour. The results of the qualitative research were compared with the compiled statistics of premature labours in ON Příbram. The data was collected from labour books from 1985 {--} to 1988 and from 2005 to 2008. According to the statistics the increasing prevalence of premature labours was found out. From the results of the research emerged that 80 % of informants have sufficient awareness during prenatal care and 20 % none. Further, 100 % of informants have sufficient awareness on impending premature labour during hospitalization. For 100 % of informants with these problems is the biggest problem the fear for unborn child. 80 % of informants find no negatives in nursing and 20 % find negatives in disunity of information from the doctors and their often changes. On the basis of the research it is possible to state hypothesis: The clients visiting prenatal care and if they are hospitalized in ON Příbram have enough information on impending premature labour. The biggest problem in hospitalized women with impending premature labour is the fear for unborn child. Informants find no negatives in nursing carried out by midvives. The results of the research are intended for present and future nursing teams dealing with these problems and might aid to improve the quality of nursing women with impending premature labour.
