Cardiac failure in pre-hospital care

The primary topic of study for this bachelor's thesis is prehospital emergency care of congestive heart failure. The theoretical section details the anatomy of the heart and its conductive system to allow for later discussion of the pathophysiology of heart failure. Types of heart failure, associated symptoms, therapeutic approaches, and complications are also described. The practical section addresses two distinct aims. The first studies how well emergency first responders understand the pathophysiology of left-sided heart failure and the associated treatments. The second focuses on right-sided heart failure. We utilized qualitative methods for data gathering and analysis. Data collection was performed via structured interviews of first responders from the Southern Bohemia District in Tabor and Jindrichuv Hradec. The results were grouped into twelve qualitative categories, which allowed us to complete both aims of the thesis. We demonstrate that emergency first responders have good knowledge of the basic pathophysiology of congestive heart failure including individual types. Understanding of heart failure etiologies is less common. There is also inconsistent mastery of the presenting symptoms and therapeutic approaches. In many instances, there was significant variability between individuals in the extent of their knowledge. Practical experience with diagnosis of congestive heart failure in the field was also variable, but it was consistently one of the most feared diagnoses for the first responders. The results of this thesis can serve as a starting point for the design of an instructional tool to be used for emergency first responder training in congestive heart failure pathophysiology, associated symptoms, and acute therapy. The goal of such a tool would be to provide more uniform understanding of this potentially life-threatening process and to allow for the delivery of higher quality prehospital care.
